Multiple sclerosis is a common demyelinating disorder of the central nervous system, and neuro-ophthalmologic manifestations occur in the majority of patients. This article provides a review of the pathogenesis, epidemiology, and classification of multiple sclerosis. Neuro-ophthalmologic abnormalities associated with multiple sclerosis, including acute demyelinating optic neuritis and internuclear ophthalmoplegia, are described in detail. Current and emerging technologies designed to assess visual function in multiple sclerosis are discussed. A summary presents the appropriate evaluation and m…

Natalizumab (NTZ), defined as "the first of a new class of drugs known as elective adhesion molecule inhibitors" was developed at the beginning of 2003 to treat relapsing-remitting multiple sclerosis (MS) and was approved in the United States in November 2004. In February 2005, the production of NTZ was suspended by Producer Firms on account of the occurrence of two serious adverse events: two patients who had been taking NTZ manifested a progressive multifocal leukoencephalopathy; the patients showed progressive neurologic deterioration, initially believed to be a worsening of the pre-existin…

BACKGROUND: Multiple sclerosis is a presumed cell-mediated autoimmune disease of the central nervous system. Cyclophosphamide (CFX) is a cytotoxic and immunosuppressive agent, used in systemic autoimmune diseases. Controversial results have been reported on its efficacy in MS. We conducted a systematic review of all relevant trials, evaluating the efficacy of CFX in patients with progressive MS.
OBJECTIVES: The main objective was to determine whether CFX slows the progression of MS.
SEARCH STRATEGY: We searched the Cochrane MS Group Trials Register (searched June 2006), Cochrane Central Regi…

PURPOSE OF REVIEW: The aim of this article is to describe recent observations regarding the basis for the initiation and disease evolution of multiple sclerosis.
RECENT FINDINGS: A current debate is where and what initiates the neuroinflammatory reaction that characterizes the acute multiple sclerosis lesion. Immune sensitization to neural antigens could develop within the systemic compartment consequent to exposure to cross-reacting, possibly viral derived, peptides (molecular mimicry). Although CD4 T cells are considered central to initiating central nervous system inflammation, the actual …

Multiple sclerosis (MS) has recently been classified according to its clinical course. Despite relapses and remissions, its course is invariably progressive, and the observed progression from the remitting-relapsing to the secondary progressive form represents the accumulation of permanent damage to the nervous system. Discussions of the nomenclatural position of Schilder's, Marburg's, and Baló's diseases, ignore the fact that the unique, pathognomonic, sharp-edged plaque of MS, is also the pathologic end-result in the three variants. Devic's disease or neuromyelitis optica (NMO) is quite diff…

Fatigue is a common symptom in patients with multiple sclerosis. Rapid exhaustion and reduced exercise tolerance leads to difficulties in maintaining a normal daily life for many patients. Regular resting and short breaks can help to compensate this to a certain degree. The pathophysiology of fatigue is currently unknown. Damage of specific neuroanatomic regions or a more generalized effect of inflammatory mediators in the central nervous system could be the causes of fatigue. Some drugs (e.g. amantadine) have proven effective in therapy of fatigue. Recent therapeutic approaches have begun usi…

Diagnosis of multiple sclerosis is based on detection of a disseminated demyelinating inflammatory disorder of the central nervous system with a progressive course and(or) exacerbations. Essential data is obtained by study of the cerebrospinal fluid and magnetic resonance imaging. Other causes of white matter disorders (vascular, compressive or tumoral) are less important in differential diagnosis than are other monophasic demyelinating inflammatory disorders, such as acute disseminated encephalomyelitis, idiopathic optic neuritis and idiopathic myelitis.

The diagnosis of multiple sclerosis (MS) remains clinically determined, based on evidence for lesions disseminated over time and space. Evidence of multifocal disease can now be obtained through paraclinical tests, such as magnetic resonance imaging and evoked responses. Cerebrospinal fluid abnormalities may facilitate diagnosis. The disease is extremely protean in its manifestations, although early in the course, sensory symptoms, motor weakness, and optic neuritis occur most frequently. Once a diagnosis is established, patients should be so informed by their physician, so that they will be p…

OBJECTIVE: This article examines the cognitive and psychiatric features of multiple sclerosis. MS can manifest as a neuropsychiatric disturbance even in the absence of physical disabilities.
METHOD: Two MS patients with predominant behavioral symptoms are described, and the literature is reviewed.
RESULTS: The first patient had an interhemispheric disconnection syndrome, and the second patient had cognitive fatigue and depression. Other patients have slowed information processing speed, memory retrieval difficulty, frontal-executive dysfunction, and visuospatial difficulty.
CONCLUSIONS: MS …

We report four patients who had multiple sclerosis associated with diabetes mellitus. Multiple sclerosis and diabetes mellitus share clinical, epidemiological and immunological features. The association between these two putative autoimmune diseases may suggest a similar pathogenic mechanism.

In a cross-sectional study of 299 MS patients 22 have had fractures and of these 17 after onset of MS. The fractures most frequently involved the femoral neck and trochanter (41%). Three patients had had more than one fracture. Only 1 patient had osteoporosis. The percentage of fractures increased with increasing age and disease duration. Among 34 deceased MS patients 4 had had fractures. These findings are discussed in relation to physical and cognitive impairment in MS. A case-control study is recommended.

Multiple sclerosis reportedly coexists with disorders of autoimmune origin. The prevalence with which such disorders occur in the MS population has not been adequately investigated. We reviewed the medical records of 828 patients with definite MS and found that 4.8% had a past or present associated disorder in which autoimmune mechanisms presumably play a role. The cumulative prevalence of these disorders was no higher than that estimated for the general population. Serum from 105 patients, without clinical evidence of an associated autoimmune disorder, was tested for the presence of antinucle…

A classification of disseminated sclerosis summing up many years of follow up of 750 patients is proposed. According to this classification, the cerebral, spinal and cerebrospinal forms are distinguished. The cerebral form includes hyperkinetic and ocular disease varieties. Both childhood disseminated sclerosis and disease variety with a late debut are distinguished. Rare cases of familial disseminated sclerosis are marked. Acute disseminated sclerosis is considered only as a stage of chronic disease development.
