A macromolecular approach to excitation phenomena: mechanical and thermal changes in nerve during excitation.

(1) Nerve fibers and cells were shown to swell when they develop action potentials. Both in the squid giant axon and in the garfish olfactory nerve, the peak of swelling coincides with the peak of the action potential recorded from the site of mechanical recording. Substitution of univalent cations for the bivalent cations (Ca) bound to multi-anion sites in the membrane macromolecules is considered to be at the base of the phenomenon of swelling. (2) Measurements of heat production by the garfish olfactory nerve indicate that the rate of temperature rise is maximal at the peak of the action po

Electrical signs of impulse conduction in spinal motoneurons.

An analysis has been made of the electrical responses recorded on the surface and within the substance of the first sacral spinal segment when the contained motoneurons are excited by single and repeated antidromic ventral root volleys. A succession of negative deflections, designated in order of increasing latency m, i, b, d, has been found. Each of those deflections possesses some physiological property or properties to distinguish it from the remainder. Indicated by that fact is the conclusion that the successive deflections represent impulse conduction through successive parts of the moton

Electrophysiology of Neuromuscular Disorders in Critical Illness.

Neuromuscular disorders-predominantly critical illness myopathy (CIM), critical illness polyneuropathy (CIP), and critical illness neuromyopathy-commonly occur in intensive care unit (ICU) patients. Given the limitations of the neurologic examination in the setting of ICU-acquired weakness, electrophysiology is very helpful in localizing the cause of weakness and defining its etiology. In CIM, motor unit potentials have typical myopathic features, but motor unit potential activation may be limited. In addition, there are features of sarcolemmal inexcitability reflected in low motor responses t

Clinical Reasoning: A Patient With Progressive Sensory Neuropathy: Using Nerve Ultrasound and Skin Biopsy to Refine Diagnostic Reasoning.

A 74-year-old man presented with a 5-year history of progressive feet numbness followed by gait unsteadiness. Neurologic examination revealed absent tendon reflexes, markedly reduced vibration sense in the lower limbs with sensory ataxia, and preserved motor strength. The clinical picture was consistent with a chronic sensory-predominant neuropathy causing sensory ataxia. Nerve conduction studies demonstrated a sensory axonal polyneuropathy with absent sensory nerve action potentials and preserved compound motor action potentials. Extensive screening for potential acquired causes of polyneurop

Nonsystemic Vasculitic Neuropathy Presenting as Inflammatory Demyelinating Polyradiculoneuropathy.

BACKGROUND AND AIMS: Nonsystemic vasculitic neuropathy classically presents as a painful asymmetric axonal neuropathy, multiple mononeuropathies or radiculoplexus neuropathy. Demyelinating electrophysiologic features are rare and may lead to misdiagnosis. We describe a patient with biopsy-proven vasculitic neuropathy presenting as a severe symmetric demyelinating polyradiculoneuropathy. METHODS: Clinical, electrophysiologic, imaging, laboratory, and histopathologic findings are presented. RESULTS: A 57-year-old man developed acute ascending weakness, sensory loss, facial weakness, and arefle

Do nerve conduction studies or ultrasound imaging correlate more closely with subjective symptom severity in carpal tunnel syndrome?

INTRODUCTION/AIMS: Previous studies have reported weak correlations between neurophysiological measurements and subjective severity of symptoms in carpal tunnel syndrome, with Pearson r ≤ 0.26. We hypothesize that this resulted in part from patient-to-patient variability in the assessment of subjective severity using tools such as the Boston Carpal Tunnel Questionnaire. To compensate for this, we aimed to assess within-patient differences in symptom and test result severity. METHODS: In our study we used retrospective data from 13 005 patients with bilateral electrophysiological results and 7

Utility of Median-to-Ulnar Sensory Comparative Nerve Conduction Study to Increase Diagnostic Accuracy in Mild Carpal Tunnel Syndrome.

Electro-physiological diagnosis of mild Carpal Tunnel Syndrome (CTS) based on traditional median motor and sensory studies are often inconclusive. Therefore, we wanted to investigate the utility of Median-to-Ulnar Sensory Conduction studies in diagnosis of mild CTS. Data from Nerve conduction study (NCS) of 82 cases with symptoms suggestive of CTS from September 2017 to October 2020 attending electrophysiology department of Mount Adora Hospital, Sylhet, Bangladesh was selected for study. About 54(66.0%) out of 82 symptomatic patients were diagnosed as CTS by the conventional method, and 28(34.

Carpal Tunnel Syndrome: Rapid Evidence Review.

Carpal tunnel syndrome (CTS) is caused by compression of the median nerve as it travels through the carpal tunnel. Patients commonly experience pain, paresthesia, and, less often, weakness in the distribution of the median nerve. Provocative maneuvers, such as the Phalen test and Tinel sign, have varying sensitivity and specificity for the diagnosis of CTS. Thenar atrophy is a late finding and highly specific for CTS. Although patients with a classic presentation of CTS do not need additional testing for diagnosis, electrodiagnostic studies can confirm the diagnosis in atypical cases, exclude

Electrodiagnostic Assessment of Radiculopathies.

This article discusses the electrodiagnostic assessment of radiculopathy. Relevant anatomy initially is reviewed followed by discussion surrounding the approach to nerve conduction studies and needle electrode examination when it comes to radiculopathy evaluation. Pitfalls of the electrodiagnosis versus clinical diagnosis of radiculopathy and the definitions of acute versus chronic, and active versus inactive, are reviewed.
