The development of the neurosurgery workforce in Austria over the past quarter century: is more always better?

BACKGROUND: The neurosurgical workforce has expanded markedly across Europe, often accompanied by declining operative exposure per surgeon. Austria, with one of the highest physician and hospital bed densities in the OECD, provides an important case study to assess whether workforce expansion has translated into proportional service provision and maintained training opportunities. METHODS: We performed a retrospective, nationwide analysis of official health statistics from Statistik Austria covering 1997-2023. Data included numbers of practicing neurosurgeons, all specialist physicians, popul

Identifying pediatric neurosurgery training priorities in Paraguay: a novel quadrant analysis.

BACKGROUND: The landscape of pediatric neurosurgical training in Paraguay is largely unknown, partly due to the absence of pediatric fellowship training in the country. Correspondingly, the aim of this study was to identify priorities for future training initiatives using a novel quadrant analysis based on current trainee perceptions. METHODS: Paraguayan neurosurgical trainees were surveyed regarding their familiarity with multiple pediatric neurosurgical diseases (n = 21), procedures (n = 24), and technologies (n = 10). Familiarity was scored on a discrete numerical scale. The mean and stand

Venous Thromboembolism Risk and Thromboprophylaxis in Pediatric Neurosurgery and Spinal Injury: Current Trends and Literature Review.

Although the entities of venous thromboembolism (VTE), deep venous thrombosis, pulmonary embolus, and thromboprophylaxis in adult patients undergoing brain tumor and spine surgery, traumatic brain injury and elective neurosurgical procedures are widely elucidated, the same is not valid when pediatric patients are under consideration. An attempt to review the peculiarities of these patients through a comprehensive bibliographic review is undertaken. We performed a narrative summary of the relevant literature dedicated to pediatric patients, centered on traumatic brain injury, the general incide

Fetal neurosurgery.

Among fetal surgical procedures, neurosurgery stands out due to the number of cases and the possibility of developing new procedures that can be performed in the fetal period. To perform fetal neurosurgical procedures, there is a need for specialized centers that have experts in the diagnosis of fetal pathologies and a highly complex obstetrics service with specialized maternal-fetal teams associated with a pediatric neurosurgery center with expertise in the diverse pathologies of the fetus and the central nervous system that offers multidisciplinary follow-up during postnatal life. Services t

Conservative treatment of severe limited mouth opening after transtemporal craniotomy.

Limited mouth opening (LMO) is a common complication of the transtemporal craniotomy that is often involved in the temporalis and temporomandibular joint. The occurrence of prolonged LMO was rare. Risk factors for such limitation after these neurosurgical procedures are defined, and the strategies of treatment and prevention are further discussed. This article presents 12 patients with severe LMO after transtemporal neurosurgical procedures. Four patients received myofunctional training combined with the neuromuscular blocking in the temporalis, which methods may be more effective for LMO withi

Neurosurgery in the malignant hyperthermia-susceptible patient.

Malignant hyperthermia is a seemingly rare genetic myopathy. Hypermetabolic crisis accompanied by a rise in body temperature to as high as 44 degrees C is its hallmark. Malignant hyperthermia is usually triggered by potent inhaled anesthetics or depolarizing muscle relaxants. Because of the extraordinary risk of death in patients who are susceptible, neurosurgeons may be reluctant to operate on these patients. Three such patients were referred to the Neurosurgery Service and the UCLA Malignant Hyperthermia Center after neurosurgical procedures aborted for first episodes of malignant hypertherm

Neurosurgical Infections.

Patients in the neurointensive care unit often undergo life-saving neurosurgical interventions that can be associated with serious complications. Infection is a common and sometimes fatal complication of such procedures. Infection may occur not only in the setting of major cranial and spinal surgery, but also with common minor procedures that utilize neurosurgical devices, such as placement of external ventricular drains, ventriculoperitoneal shunts, and deep brain stimulators. In this article, we review the epidemiology and microbiology of these infections, and discuss their general and proce

An Evaluation of the Practice for the Requisition of Blood Products and its Utilization in Neurosurgical Patients Undergoing Elective Surgery at a Tertiary Care Hospital.

BACKGROUND: Neurosurgical procedures are associated with profound blood loss that necessitates need for intraoperative and postoperative blood transfusion. Excessive ordering of blood based on physicians' habitual practice may lead to unintentional misuse of blood bank services. For the optimal use of blood resources, transfusion practices have to be appropriate. AIMS: The aim of this study is to study the cross match to transfusion ratio and to review the blood utilization practices (transfusion index and maximal surgical blood order schedule) in elective neurosurgical procedures. Pediatric Neurosurgical Care in a German Field Hospital in Afghanistan.

Background and Study Objectives Children are commonly found among the injured in war settings. Prognosis often depends on the presence of brain injury. Not all neurosurgery departments of German Armed Forces hospitals, however, have experience in the care of children with neurosurgical conditions. Against this background, we assessed the group of pediatric neurosurgical patients in a German (Role 3) field hospital in Mazar-e-Sharif, Afghanistan. Patients/Materials and Methods We analyzed the operative logbooks from January 1, 2008, to December 31, 2013, to assess the number of neurosurgical pr

Evaluation of the Performance of StypCelTM Absorbable Hemostat for Intraoperative Hemorrhage Control in Neurosurgery: A Multicenter, Single-Arm Study in Riga, Latvia.

Background and Objectives: Intraoperative bleeding during neurosurgical procedures poses a significant risk by increasing morbidity and mortality, obscuring the surgical field and prolonging operative time and hospitalization. Effective hemostasis is therefore essential, frequently necessitating the use of topical hemostatic agents. This study aimed to evaluate the performance of a plant-derived oxidized regenerated cellulose (ORC) hemostatic agent StypCel™ Absorbable Hemostat (Medprin Regenerative Medical Technologies Co., Ltd.) in various neurosurgical interventions, including intracranial t

Methods and results of neurosurgical treatment of cerebral palsy.

UNLABELLED: Treatment of spastic syndrome and muscular dystonia in patients with cerebral palsy is a complex clinical problem. Effectiveness of conservative treatment is not high enough. Modern neurosurgical techniques for spastic syndrome and dystonia are divided into destructive interventions and surgical neuromodulation. Their effectiveness is different and depends on the form of disease, severity of motor disorders and age of patients. OBJECTIVE: To evaluate the effectiveness of various methods of neurosurgical treatment of spasticity and muscular dystonia in patients with cerebral palsy.
