Degrees of acid suppression and ulcer healing: dosage considerations.

The human stomach has a normal circadian rhythm of intragastric acidity characterized by increasing acidity during the day and peaks in the early hours of the morning. Eating causes a transient decrease of intragastric acidity. Acid appears to be the permissive factor in peptic ulcer disease and to be responsible for symptoms; the patient with duodenal ulcer may secrete too much acid. Pharmacological control of gastric acid secretion will speed ulcer healing. Modern regimens, which typically use a bedtime dose of an H2-receptor antagonist, produce a pulse of decreased acidity. Intragastric aci

Managing irritable bowel syndrome in primary care.

The classic symptoms of irritable bowel syndrome (IBS) are abdominal pain, bloating and some form of bowel dysfunction. The pain is typically colicky in nature and can occur at any site although most commonly it is on the left side. The abdomen feels flat in the morning and then gradually becomes more bloated as the day progresses reaching a peak by late afternoon or evening. It then subsides again over night. Traditionally IBS is divided into diarrhoea, constipation or alternating subtypes. IBS patients frequently complain of one or more non-colonic symptoms, these include constant lethargy,

Alpha-gal allergy after a tick bite in Austria.

Tick bites can cause the alpha-gal syndrome, which is characterized by delayed anaphylactic reactions mainly to red meat and offal due to IgE antibodies against mammalian galactose-alpha-1.3-galactose carbohydrate (alpha-gal). Ixodes ricinus bites are considered the primary cause of IgE antibody responses specific for alpha-gal in Europe. This article reports on a 51-year-old Austrian male who acquired a tick bite in Austria in spring 2017, which, within 48 h, resulted in prolonged inflammation of the skin area around the bite. The patient experienced an allergic reaction 3 months later approx

Circadian disruption: New clinical perspective of disease pathology and basis for chronotherapeutic intervention.

Biological processes are organized in time as innate rhythms defined by the period (τ), phase (peak [Φ] and trough time), amplitude (A, peak-trough difference) and mean level. The human time structure in its entirety is comprised of ultradian (τ < 20 h), circadian (20 h > τ < 28 h) and infradian (τ > 28 h) bioperiodicities. The circadian time structure (CTS) of human beings, which is more complicated than in lower animals, is orchestrated and staged by a brain central multioscillator system that includes a prominent pacemaker - the suprachiasmatic nuclei of the hypothalamus. Additional pacemak

Evaluation of muscle strength assessment in adolescent polycystic ovary syndrome: a case control study.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a common endocrine disorder in adolescents characterized by irregular menstrual cycles, hyperandrogenism and metabolic disturbances. While much of the research on PCOS has focused on the hormonal and metabolic effects, the impact of PCOS on physical health, particularly muscle strength, has not been adequately studied. Previous studies suggest that hyperandrogenism may play a role in muscle strength, but the relationship between muscle function and PCOS in adolescents has not been well studied. Understanding this relationship is critical to impro

Associations of sleep patterns with metabolic syndrome indices, body composition, and energy intake in children and adolescents.

BACKGROUND: Self-reported short sleep duration is associated with greater risk for metabolic syndrome (MetS), obesity, and higher energy intake (EI). However, studies of these associations in children using objective methods are sparse. OBJECTIVES: The study aims to determine the associations for sleep patterns with MetS indices, body composition, and EI using objective measures in children. METHODS: Free-living sleep and physical activity were measured in 125 children (aged 8-17 years, BMI z = 0.57 ± 1.0, 55% female) using wrist-worn actigraphs for 14 nights. Emotional Deprivation in Children: Growth Faltering and Reversible Hypopituitarism.

Emotional deprivation can lead to growth faltering of infants and children. The mechanism(s) involved differ in that for infants, the major metabolic problem is inadequate energy intake for growth. In young children, it is likely that the emotional deprivation causes a syndrome not only of growth faltering, but with bizarre behaviors, especially with regard to food: hoarding, gorging and vomiting, hyperphagia, drinking from the toilet, and eating from garbage pails. Other disturbed behaviors include, poor sleep, night wanderings, and pain agnosia. The pathophysiology appears to be reversible h

Prevalence of Metabolic Syndrome and Its Associated Risk Factors Among Schoolchildren Aged 11-13 Years Living in Thiruvananthapuram District, Kerala, India: A Nested Case-Control Study.

INTRODUCTION: Metabolic syndrome is a cluster of conditions that includes increased blood pressure, increased blood sugar, excess body fat around the waist, and a rise in triglyceride levels which could increase the risk of developing type 2 diabetes and cardiovascular diseases. While metabolic syndrome is typically associated with adults, concern is growing about its prevalence and implications among children and adolescents. The rapid rise in childhood obesity and physical inactivity has led to an increase in risk factors among young populations, making it a pressing public health issue. Thi

Psychosocial risk and protective factors for youth problem behavior are associated with food addiction in the Generation Z.

OBJECTIVE: Food Addiction (FA) and other well-known risk behavior as substance misuse tend to co-occur and may share similar risk and protective factors. The aim of this study was to assess the association between the diagnosis/severity of FA and psychosocial domains typically related to risk behavior syndrome in a large, nationally representative community sample of Generation Z underage Italian students. METHOD: The sample consisted of 8,755 students (3,623 from middle schools, 5,132 from high schools). A short version of the Yale Food Addiction Scale 2.0 was administered to evaluate FA. Ri

Parasomnias and movement disorders of sleep.

Neurologists are often enlisted to help diagnose, evaluate, and manage a spectrum of abnormal spells during the night ranging from parasomnias to motor disturbance that span the sleep-wake cycle. Parasomnias are undesirable emotional or physical events that accompany sleep. These events typically occur during entry into sleep from wakefulness, or during arousals from sleep, and are often augmented by the sleep state. Some parasomnias, such as the rapid eye movement (REM) sleep behavior disorder may be extremely undesirable, while others such as somniloquy are often of little concern. The paras

Promoting the Mediterranean Diet in the Workplace: An Observational Cross-Sectional Study.

Background/Objectives: The Mediterranean diet (MD) is a set of customs and eating habits developed in countries bordering the Mediterranean Sea thousands of years ago. These habits are frequently proposed as a complement to pharmacological therapies for chronic diseases and could also counteract their onset. This study aimed to investigate adherence to the MD in workers from 22 companies in Latium (Italy) and to evaluate the association of diet with workers' health. Methods: Workers undergoing their health surveillance examination and routine blood tests (total cholesterol, HDL cholesterol, tr

[Contemporary criteria of the diagnosis and current recommendations for nutritional therapy in anorexia nervosa].

The basic criterion for the diagnosis of anorexia (AN - anorexia nervosa) by ICD-10 (International Classification of Diseases, version 10) is the body weight less than 15% of the expected normal body weight. According to DSM-IV (Diagnostic and Statistical Manual for Mental Disorders, version IV) the basic feature of AN is a refusal to maintain body weight equal or greater than the minimal normal weight. The prevalence of anorexia nervosa is 0.3-0.5% or even 1.3-3.7% if include pre-anorexic states (eg. the phenomenon of pro-ana). The main feature of anorexia is a reduction of caloric intake. Ac

Digital Addiction and Sleep.

In 2020, the World Health Organization formally recognized addiction to digital technology (connected devices) as a worldwide problem, where excessive online activity and internet use lead to inability to manage time, energy, and attention during daytime and produce disturbed sleep patterns or insomnia during nighttime. Recent studies have shown that the problem has increased in magnitude worldwide during the COVID-19 pandemic. The extent to which dysfunctional sleep is a consequence of altered motivation, memory function, mood, diet, and other lifestyle variables or results from excess of blu

Sleep disturbances in patients with schizophrenia : impact and effect of antipsychotics.

Difficulties initiating or maintaining sleep are frequently encountered in patients with schizophrenia. Disturbed sleep can be found in 30-80% of schizophrenic patients, depending on the degree of psychotic symptomatology. Measured by polysomnography, reduced sleep efficiency and total sleep time, as well as increased sleep latency, are found in most patients with schizophrenia and appear to be an important part of the pathophysiology of this disorder. Some studies also reported alterations of stage 2 sleep, slow-wave sleep (SWS) and rapid eye movement (REM) sleep variables, i.e. reduced REM l
