Mechanical stress-induced apoptosis of nucleus pulposus cells: an in vitro and in vivo rat model.

BACKGROUND: Un-physiological loads play an important role in the degenerative process of inter-vertebral discs (IVD). In this study, we used an in vitro and in vivo rat model to investigate the mechanism of nucleus pulposus (NP) cells apoptosis induced by mechanical stress. METHODS: Static compressive load to IVDs of rat tails was used as the in vivo model. For the in vitro model, NP cells were tested under the physiological and un-physiological loading. For histological examination, apoptotic index study, and apoptotic gene expression, we also selected cytokines [bone morphogenetic protein (…

Adult acute calcific discitis confined to the nucleus pulposus in the cervical spine: case report.

Acute calcific discitis is a rare condition in the pediatric population and has been reported in only 2 instances in the adult population. This report describes a case of acute calcific discitis that uniquely presented in the adult cervical spine. A 22-year-old woman presented with the chief complaint of sudden-onset neck pain. Nonsurgical management, including nonsteroidal antiinflammatory drugs, provided moderate symptom relief. Radiography revealed nucleus pulposus calcification at the C2-3 level. Contrast-enhanced MRI did not reveal any additional abnormalities. Further nonsurgical managem…

Cytokine inhibition and time-related influence of inflammatory stimuli on the hyperalgesia induced by the nucleus pulposus.

INTRODUCTION: The symptoms of lumbar disc herniation, such as low back pain and sciatica, have been associated with local release of cytokines following the inflammatory process induced by the contact of the nucleus pulposus (NP) with the spinal nerve. MATERIAL AND METHODS: Using an animal experimental model of intervertebral disc herniation and behavioral tests to evaluate mechanical (electronic von Frey test) and thermal (Hargreaves Plantar test) hyperalgesia in the hind paw of rats submitted to the surgical model, this study aimed to detect in normal intervertebral disc the cytokines known…

HDAC1 maintains metabolic homeostasis in nucleus pulposus cells via post-translational modification interactions with YBX1.

Intervertebral disc degeneration is characterized by the metabolic dysregulation of nucleus pulposus cells, which is regulated by complex post-translational modifications, manifesting as extracellular matrix remodeling and inflammatory microenvironment formation. We demonstrated the progressive downregulation of HDAC1 during disc degeneration. HDAC1 mediates the deacetylation and ubiquitination-dependent degradation of YBX1. The downregulation of HDAC1 results in YBX1 accumulation, compromising extracellular matrix synthesis in nucleus pulposus cells while enhancing the degradation processes. …

D-Galactose induces the senescence and senescence-related secretory phenotype switch of nucleus pulposus cells.

Senescence of nucleus pulposus cells (NPCs) in degenerative intervertebral discs is related to the development and progression of intervertebral disc degeneration (IVDD). The aim of this study was to establish a reliable and robust cell senescent model using D-galactose (D-Gal) to induce oxidative stress in NPCs. NPCs were isolated from Sprague Dawley rats and incubated with increasing concentrations of D-Gal. Cell viability, cell cycle, senescence-associated markers, and extracellular matrix were detected to evaluate the effect of D-Gal on NPCs. The migration and polarization of macrophage we…

Cyclic tensile loading regulates nucleus pulposus cell autophagy through mitochondrial dynamics: molecular mechanisms and implications.

BACKGROUND: Chronic low back pain is predominantly driven by intervertebral disc degeneration (IVDD), a process rooted in the dysregulation of extracellular matrix (ECM) homeostasis within nucleus pulposus (NP) cells. These cells reside in a harsh microenvironment characterized by hypoxia, nutrient scarcity, and mechanical stress, making the regulatory pathways of autophagy and mitochondrial dynamics critical for their survival and function. MAIN BODY: This review synthesizes current evidence demonstrating that cyclic tensile loading (CTL) is a decisive factor directing NP cell fate through th…

In Situ Stress-Dispersing Hydrogel Millispheres via Load Redistribution to Restore Nucleus Pulposus Metabolic Homeostasis.

Abnormal mechanical stress can disrupt the metab homeostasis of the nucleus pulposus (NP) and induce inflammatory reactions and matrix degradation, thereby driving the progression of intervertebral disc degeneration (IDD). Owing to their small particle size, traditional micron hydrogel microspheres exhibit insufficient load dispersion, and it is difficult to effectively restore the stable environment of NP metabolism destroyed by local stress concentration. This study developed a bionic hydrogel millimeter sphere, approximately 1 mm in diameter, using a dual network design (HAMA/ChSMA, HA:ChS …

UCHL3 drives ferroptosis in nucleus pulposus cells by deubiquitinating HMGB1 to activate NCOA4-mediated ferritinophagy.

Ferroptosis contributes to intervertebral disc degeneration (IVDD), yet its upstream regulatory mechanisms in nucleus pulposus cells (NPCs) remain unclear. Here, we identify a novel UCHL3-HMGB1-NCOA4 signaling axis that drives NPC ferroptosis. Transcriptomic and clinical analyses reveal that UCHL3 is upregulated in degenerative discs, with its expression correlating positively with both IVDD severity and ferroptosis markers. Mechanistically, UCHL3 deubiquitinates and stabilizes HMGB1 by cleaving Lys48-linked polyubiquitin chains, thereby promoting the accumulation of cytoplasmic HMGB1. This st…

Fibrocyte enrichment and myofibroblastic adaptation causes nucleus pulposus fibrosis and associates with disc degeneration severity.

Fibrotic remodeling of nucleus pulposus (NP) leads to structural and mechanical anomalies of intervertebral discs that prone to degeneration, leading to low back pain incidence and disability. Emergence of fibroblastic cells in disc degeneration has been reported, yet their nature and origin remain elusive. In this study, we performed an integrative analysis of multiple single-cell RNA sequencing datasets to interrogate the cellular heterogeneity and fibroblast-like entities in degenerative human NP specimens. We found that disc degeneration severity is associated with an enrichment of fibrocy…

Muscone suppresses inflammation and senescence of nucleus pulposus via p53 signalling during intervertebral disc degeneration.

Intervertebral disc degeneration (IDD) is a major cause of chronic low back pain, the mechanism of which is still unclear. Inflammation-induced extracellular matrix metabolism (ECM) dysregulation in the nucleus pulposus (NP) and NP cell senescence are known to be the key causes of IDD. However, few drugs can reliably alleviate ECM dysregulation and NP cell senescence. Muscone, as the key natural component of musk, is a widely applied antiapoptotic and anti-inflammatory drug. We found Muscone exerts protective effects by inhibiting the expression of ECM catabolism-related genes, cell apoptosis,…

ADM ameliorates disc degeneration by inhibiting the MAPK signaling pathway and nucleus pulposus cell senescence.

BACKGROUND: Intervertebral disc degeneration (IVDD) is a leading cause of chronic low back pain. Current therapies fail to address the underlying molecular mechanisms of IVDD, necessitating novel targets. METHODS: Bioinformatics analysis identified differentially expressed genes (DEGs) from GSE186542 and GSE167199 datasets. A protein-protein interaction network was performed to find hub genes. Hub genes were validated in vitro using interleukin (IL)-1β-treated nucleus pulposus cells (NPCs). EdU, SA-β-Gal staining, and flow cytometry were applied to assess cell proliferation, senescence and ap…

SPP1 secreted by nucleus pulposus cells mediates inflammatory infiltration and contributes to intervertebral disc degeneration.

Low back pain resulting from intervertebral disc degeneration has emerged as a global public health challenge, imposing a substantial economic and societal burden. The inflammatory response of nucleus pulposus cells and the imbalance of the extracellular matrix are recognized as key drivers of intervertebral disc degeneration. In this study, we identified that SPP1 is highly expressed in degenerated nucleus pulposus tissue and is closely associated with both inflammation and extracellular matrix imbalance. Single-cell RNA sequencing of nucleus pulposus tissue revealed a distinct subtype of nuc…

Dedifferentiation-like reprogramming of degenerative nucleus pulposus cells into notochordal-like cells by defined factors.

The extensive degeneration of functional somatic cells and the depletion of endogenous stem/progenitor populations present significant challenges to tissue regeneration in degenerative diseases. Currently, a cellular reprogramming approach enabling directly generating corresponding progenitor populations from degenerative somatic cells remains elusive. The present study focused on intervertebral disc degeneration (IVDD) and identified a three-factor combination (OCT4, FOXA2, TBXT [OFT]) that could induce the dedifferentiation-like reprogramming of degenerative nucleus pulposus cells (dNPCs) to…

Single cell RNA sequencing reveals emergent notochord-derived cell subpopulations in the postnatal nucleus pulposus.

Intervertebral disc degeneration is a leading cause of chronic low back pain. Cell-based strategies that seek to treat disc degeneration by regenerating the central nucleus pulposus (NP) hold significant promise, but key challenges remain. One of these is the inability of therapeutic cells to effectively mimic the performance of native NP cells, which are unique amongst skeletal cell types in that they arise from the embryonic notochord. In this study, we use single cell RNA sequencing to demonstrate emergent heterogeneity amongst notochord-derived NP cells in the postnatal mouse disc. Specifi…

Catalytic Nanodots-Driven Pyroptosis Suppression in Nucleus Pulposus for Antioxidant Intervention of Intervertebral Disc Degeneration.

Low back pain resulting from intervertebral disc degeneration (IVDD) is a prevalent global concern; however, its underlying mechanism remains elusive. Single-cell sequencing analyses revealed the critical involvement of pyroptosis in IVDD. Considering the involvement of reactive oxygen species (ROS) as the primary instigator of pyroptosis and the lack of an efficient intervention approach, this study developed carbonized Mn-containing nanodots (MCDs) as ROS-scavenging catalytic biomaterials to suppress pyroptosis of nucleus pulposus (NP) cells to efficiently alleviate IVDD. Catalytic MCDs have…

Kongensin a attenuates intervertebral disc degeneration by inhibiting TAK1-mediated PANoptosis of nucleus pulposus cells.

Low back pain (LBP) is most commonly caused by intervertebral disc degeneration (IVDD). Pyroptosis, apoptosis, and necroptosis are crucial in IVDD pathogenesis; however, possible simultaneous occurrence in IVDD and co-regulation between the pathways and the regulatory mechanisms have not been investigated. PANoptosis is a regulated cell death (RCD) pathway with the key characteristics of pyroptosis, apoptosis, and necroptosis. This study revealed that tert-butyl hydroperoxide (TBHP) altered the expression of key proteins involved in PANoptosis in nucleus pulposus cells (NPCs). Furthermore, the…
