Barriers to diaphragm use: the views of advanced practice nurses.

BACKGROUND AND METHODOLOGY: Women have used the contraceptive diaphragm for decades. Although use has recently declined, the diaphragm may find a new role in STI/HIV and dual-prevention programmes when microbicides become available. We developed a questionnaire to examine seven provider issues identified as possible barriers to diaphragm use among advanced practice nurses (APNs) specialising in women's health. The perceived degree to which each issue represented a barrier was examined. Non-parametric correlations were calculated between diaphragm fitting history, demographic and practice chara

The pathophysiology of emphysema: considerations for critical care nursing practice.

Emphysema is caused by exposure to cigarette smoking as well as alpha(1)-antitrypsin deficiency. It has been estimated to cost the National Health Service (NHS) in excess of 800 million pounds per year in related health care costs. The challenges for Critical Care nurses are those associated with dynamic hyperinflation, Auto-PEEP, malnutrition and the weaning from invasive and non-invasive mechanical ventilation. In this paper we consider the impact of the pathophysiology of emphysema, its effects on other body systems as well as the impact acute exacerbations have when patients are admitted t

Family practice nurse views on barriers to immunising children.

New Zealand (NZ) has low immunisation coverage for infants and children compared to many other westernised countries. Barriers to improving uptake are multifactorial, with health professional knowledge and attitudes identified as important modifiable factors. In NZ practice nurses give most childhood vaccinations in the primary health care setting. This study explored aspects of 150 family practice nurse views, knowledge and experience about immunisation. Qualitative and quantitative text data were obtained through randomised computer assisted telephone surveys and converged using a triangulat

Evaluation of an action research project in ophthalmic nursing practice.

AIM: This paper reports the evaluation phase of an action research project that promoted face-down posturing of patients following vitreo-retinal surgery for macular hole to enhance patient outcomes. The evaluation phase identified areas of practice needing further development from the perspectives of those involved with the care of patients. BACKGROUND: To achieve best results following surgical repair of macular hole, patients are required to posture face down for several weeks. As a consequence, patients complain of severe back and neck ache and find it difficult to persist with the postur

The genetic family history assessment in gastroenterology nursing practice.

Genetic factors influence the risk for disease and the overall health of persons throughout their lifespan. The systematic collection of family history information in a three-generation format is the most important approach for the identification of individuals with a genetic susceptibility to most common diseases, and applying genetic concepts in healthcare. The detailed family history can play a critical role in diagnosis, lay the foundation for accurate risk assessment, and be used to develop subsequent education, individualized monitoring and management of the disease, and prevention measu

Primary pulmonary hypertension: a review for advanced practice nurses.

Primary pulmonary hypertension (PPH) is a rare entity of unknown etiology, manifested as narrowing and obstruction of the pulmonary arteries leading to elevated pulmonary artery pressures. Left untreated, these high pressures can lead to right ventricular dilation, right heart failure, and death within 3 to 5 years. Over the last decade, advances have been made in the diagnosis and treatment of PPH. Advanced practice nurses and other expert nurses can use and interpret diagnostic and therapeutic medical regimens currently available to identify and treat this rare condition.

Women with gastrointestinal symptoms: implications for nursing research and practice.

Irritable bowel syndrome and functional bowel disorder are diagnoses used to describe chronic GI symptoms for which no overt pathological condition can be identified. Symptoms are more common in women and are frequently followed in gastroenterology clinics. The purpose of this article is to provide an overview of the research linking GI symptoms and reproductive cycling and to discuss implications for practice. GIGl symptoms such as stomach pain and nausea are highest during menses compared with other cycle phases; also, stool consistency is loosest at menses. This pattern is present in contro

Marfan's syndrome in pregnancy: implications for advanced practice nurses.

Marfan's syndrome is a genetic disorder that often affects the cardiovascular, pulmonary, ocular, and musculoskeletal systems. When a woman with Marfan's syndrome becomes pregnant, the hormonal stresses and changes to the cardiovascular system can put the mother at risk for serious complications. Advanced practice nurses need to understand the maternal changes associated with Marfan's syndrome as well as the genetic factors involved in order to provide holistic care.

Spirituality in nursing theory and practice: dilemmas for Christian bioethics.

Moral strangerhood is due in part to competing worldviews. The profession of nursing is experiencing a paradigm shift which creates ethical dilemmas for both Christian nurses and Christian patients. Nursing's new focus on spirituality and spiritual care presents itself as broadly defining a desired state or patient outcome -- spiritual integrity -- supposed to be applicable to all patients of all faiths. Analysis of nursing's definition of spirituality reveals assumptions and values consistent with an Eastern/New Age worldview which may cause hostility towards Christian patients stereotyped as

The Cardiac Arrhythmia Suppression Trial: Implications for nursing practice.

BACKGROUND: Care of patients with ventricular arrhythmia after myocardial infarction requires careful nursing management, including assisting with arrhythmia monitoring and testing. Because ventricular premature depolarization is a known risk factor for sudden cardiac death, it was hypothesized that the suppression of asymptomatic or mildly symptomatic ventricular premature depolarization would improve survival in these patients. OBJECTIVE: To review the Cardiac Arrhythmia Suppression Trial findings and provide implications for nursing practice for patients after myocardial infarction. METHO

Nursing practice. Stress and attitudes towards the work environment.

This paper discusses a survey carried out within four wards in an elderly services' unit. There were two main aims of the study. One was an attempt to determine the degree of burn-out present in the wards, and the second was to ascertain the nursing staff's perceived areas of stress within their work environment. Clarke (1) relies heavily on the transactional view of stress, while Selye (2), suggests that coping with stress involves the individual in direct action in changing the demands put upon him or her. This direct action is aided by information on how to cope with stressors and, therefor

Obstetricians and nurse-midwives: The team approach in private practice.

Much has been written about the need for help for the overworked obstetrician. Two obstetricians delivering over 500 babies annuallay had insufficient time for their gynecology practice. Two nurse-midwives were employed and in the first year they delivered 61 per cent of those patients who delivered vaginally. All normal patients had the privilege of choosing their accoucheur; patients, nurse-midwives, and obstetricians were very well pleased with the team approach.

Utilization of nurse-sensitive indicators in nursing practice: A comparative qualitative study in German and U.S. hospitals.

BACKGROUND: Nurse-sensitive indicators represent data that can be used in a learning health system to systematically generate knowledge, identify opportunities to improve nursing practice and enhance patient care. AIM: To analyze and compare the utilization of nurse-sensitive indicators in nursing practice between selected hospitals from Germany and the U.S. METHOD: A qualitative study was conducted, comprising 17 semi-structured interviews with 19 nurses carried out from June to September 2025. Purposive sampling was applied, including nine participants from four hospitals in Germany that w

Invisible Care: Nurses' Lived Experiences of How the Practice Environment Shapes Nurse-Patient Relationships.

AIM: To explore how the nursing practice environment shapes nurse-patient relationships, identifying key organizational challenges and opportunities for nursing management and policy. DESIGN: A qualitative hermeneutic phenomenological design was used to explore the lived experiences of nurses in a hospital setting. METHODS: Data were collected through in-depth, face-to-face interviews and analyzed using Van Manen's hermeneutic phenomenological method. Data saturation was reached after 20 interviews. RESULTS: The findings reveal that nurse-patient relationships are fundamentally shaped by or
