Home tube feeding for long-term nutritional support.

Twenty-three adult patients with protein-calorie malnutrition were referred for outpatient tube feeding. Initial nutritional support with hypercaloric supplemented diets did not prevent further catabolism and weight loss, as the mean protein intake was 35.2 g/24 hr and the mean calorie intake was 844 cal/24 hr. Outpatient tube feeding was then initiated in deliver a mean of 78.5 g protein/24 hr and 2248 cal/24 hr. Patients on tube feeding demonstrated an increase of serum albumin and total body weight compared to diet therapy alone. The patient and family members were instructed in the techniq…

Maximizing Nutrition Support Practice and Measuring Adherence to Nutrition Support Guidelines in a Canadian Tertiary Care ICU.

OBJECTIVE: New comprehensive guidelines for nutrition support (NS) in the intensive care unit (ICU) can be used to improve quality of care and benchmark current practice. The objective of this study was to (a) compare NS practices in our medical/surgical ICU (MSICU) to 18 recommendations described in the Canadian Clinical Practice Guidelines and Society of Critical Care Medicine/American Society of Parenteral and Enteral Nutrition guidelines, (b) determine the percentage of goal calories and protein delivered, and (c) identify the barriers to successful NS delivery. DESIGN: This was a prospec

ESPEN guideline on nutritional support for polymorbid medical inpatients.

BACKGROUND: Disease-related malnutrition in polymorbid medical inpatients is a highly prevalent syndrome associated with significantly increased morbidity, disability, short- and long-term mortality, impaired recovery from illness, and cost of care. AIM: As there are uncertainties in applying disease-specific guidelines to patients with multiple conditions, our aim was to provide evidence-based recommendations on nutritional support for the polymorbid patient population hospitalized in medical wards. METHODS: This update adheres to the standard operating procedures for ESPEN guidelines. We d…

Metabolism, Metabolomics, and Nutritional Support of Patients with Sepsis.

Sepsis is characterized by profound changes in systemic and cellular metabolism that disrupt normal metabolic homeostasis. These metabolic changes can serve as biomarkers for disease severity. Lactate, a metabolite of anaerobic metabolism, is the most widely used ICU biomarker and it is incorporated into multiple management algorithms. Technological advances now make broader metabolic profiling possible, with early studies identifying metabolic changes associated with sepsis mortality. Finally, given the marked changes in metabolism in sepsis and the association of worse prognosis in patients

Nutritional support of surgical patients with inflammatory bowel disease.

Patients with inflammatory bowel disease (IBD) in need of surgery are often malnourished, which in turn increases the risk for postoperative complications. Malnutrition in IBD patients who must undergo surgery is due to the disordered activity of the diseased intestine, decreased dietary intake, and adverse effects of potent medications. IBD operations predispose patients to both macronutrient and micronutrient deficiencies. If the gut can be used safely it is the preferential route for feeding, though preoperative and postoperative parenteral nutrition remains a viable alternative for severel

[Perioperative nutritional support from the viewpoint of risk management].

The nutritional condition of perioperative patients affects the results of surgical treatment. For a better prognosis, surgeons must always consider patients' nutritional status preoperatively. After first assessing the nutritional status, a regimen to improve and/or maintain proper nutrition should be selected taking symptoms and treatment procedure into consideration. Peripheral parenteral nutrition provides limited energy. If patients' nutritional status requires improvement and they must fast for a long-term, total parenteral nutrition should be administered. Parenteral nutrition is nonphy

Nutritional support after open liver resection: a systematic review.

BACKGROUND: Perioperative nutrition in patients with limited liver function after partial hepatic resection is still controversial. In particular, the significance of perioperative total enteral nutrition remains unresolved. The aim of this review is to investigate the impact of early postoperative total enteral nutrition on convalescence after partial liver resection. MATERIALS AND METHODS: In an internet-based Medline-Search (time course: 1960-08/2005) a total of five prospective, randomized controlled trials were found comparing the impact of enteral and parenteral nutrition after liver re

Energy expenditure in hospitalized patients: implications for nutritional support.

An understanding of energy expenditure in hospitalized patients is necessary to determine optimal energy supply in the care of individuals who require nutritional support. A review was conducted of 19 studies in which resting energy expenditure (REE) had been measured using indirect calorimetry and compared with estimated basal energy expenditure (BEE) from the Harris-Benedict equation. Studies of patients with burns, head injuries, and fever were excluded because REE is known to be increased in these conditions. The studies reported data on 1256 patients with the following diagnoses: postoper…

Appropriate nutritional support in acute and complicated Crohn's disease.

Crohn's disease is frequently complicated by protein-calorie malnutrition. Four common clinical presentations of Crohn's disease include acute exacerbations or flares of disease, intestinal obstruction, fistulizing disease, and perianal disease. In this review, we examine the role of nutritional support in these clinical scenarios. Nutritional support is important for maintaining functional status and preventing loss of lean tissue. Determinants of lean-tissue loss include severity of underlying injury, baseline nutritional status, and duration of inadequate nutrition. One of the clinically us…

Issues in the nutritional support of the ventilated baby.

Extremely low birth weight (ELBW) premature infants who populate intensive care nurseries today often receive such overriding attention to their acute respiratory illness that appropriate attention to certain other adjunctive needs; adequate nutrition is often delayed. The outcome for these ELBW infants may be influenced by the intensity and length of the period of less-than-adequate nutrition. This article discusses initial fluid therapy, early postnatal intravenous amino acid administration, choice of energy substrate, and initiation and advancement of enteral feedings.

How should we monitor nutritional support: structure or function?

Changes in nutritional status have been traditionally assessed by alterations in body composition. The body's components can be divided into lean body mass and fat mass. Lean body mass is composed of water, potassium, nitrogen, and glycogen. Recent studies have shown that malnutrition and refeeding alter total body potassium earlier than, and differently from, the way in which body nitrogen is altered. These findings suggest that an early effect of nutrition occurs with membrane ion transport. In support of this concept, my colleagues and I, along with other researchers, have shown that nutrit

[Nutritional support in patients with relapsing chronic respiratory failure].

The authors review the scientific data on nutritional problems in patients with acute decompensation of COPD and present their own experience in the nutritional management of such condition. Artificial nutrition (enteral and parenteral) allowed a reduction in the duration of hospitalization, a lower incidence of infections, and a shorter weaning time from mechanical ventilation in comparison to a homogeneous group of patients treated in the same unit in an earlier period, when less attention was paid to nutritional problems. The importance of artificial nutrition in patients with acute decompe

Nutritional support in the critical care setting (Part 1).

Over the past two decades nutritional support has rapidly become an integral part of the medical care of critically ill patients. As scientific evidence accumulates supporting the important role of underlying nutritional status in determining the eventual outcome of many illnesses, aggressive nutritional intervention has become commonplace in our medical and surgical ICUs. However, nutritional support, particularly parenteral alimentation, is expensive and associated with important morbidity and even mortality. Furthermore, definite evidence of its clinical efficacy under certain specific cond…

Nutritional support of the hospitalized patient. The team concept.

Two groups of patients receiving total parenteral nutrition (TPN) were compared in a prospective study for a period of 24 months. The TPN in one group of 211 patients was exclusively managed by a nutrition support team. A variety of physicians managed the TPN of 164 patients in the other group. The complication rate of both groups was compared. The second group had a much higher incidence of all complications. The study concludes that a protocol strictly adhered to by knowledgeable persons is necessary for the proper administration of TPN in the hospitalized patient.

Evaluating oral nutritional support and the nutritional risk scores in ICI-treated metastatic solid tumours.

BACKGROUND: Malnutrition is common in metastatic cancer and may compromise outcomes. We evaluated whether oral nutritional supplementation (ONS) improves survival and compared the performance of nutritional risk scores in immune checkpoint inhibitor (ICI)-treated patients. METHODS: We retrospectively analyzed 538 adults with metastatic solid tumors who initiated ICIs between 2015 and 2024. Baseline ONS use, clinicopathological variables, and laboratory parameters were recorded. Survival outcomes were assessed after 1:1 propensity score matching (PSM). RESULTS: 246 matched patients were evalu
