Krause approach to pineal tumour with intraoperative oculomotor nerve monitoring.

BACKGROUND: The supra-cerebellar infratentorial approach to pineal region tumours is versatile and safe corridor to lesions located below the deep veins. Monitoring of the extra-ocular muscle pathways using the evoked compound muscle action potential can lead to safer resections. TECHNICAL NOTE: To describe the use of electrooculography and a three handed retractor less method for pineal region tumour surgeries. MATERIAL AND METHODS: Intraoperative electrooculography uses recording done from two channels (horizontal and vertical)by inserting disposable subdermal needle electrodes along the p

Chronic oculomotor nerve palsy secondary to a sphenoid-cavernous sinus lesion in an adolescent: a case report and literature review.

BACKGROUND: Oculomotor nerve (cranial nerve III) palsy in children and adolescents is uncommon and typically suggests an underlying structural lesion rather than microvascular ischemia. Sphenoid sinus disease represents a rare cause of ocular cranial neuropathies and most frequently affects the abducens nerve. Consequently, isolated chronic oculomotor nerve palsy secondary to sphenoid sinus pathology is an unusual clinical presentation. CASE PRESENTATION: We report a 16-year-old male who presented with long-standing right ocular deviation and progressive limitation of eye movements beginning

PURPOSE OF THE STUDY: A collective summary of the pathogenesis, character and clinical significance of damage to the oculomuscular innervati … , who died following major head trauma. RESULTS: Few authors focus on damage to the innervation of oculomotor muscles, the diagnosis of damage being difficult due to the usually poor clinical condition of such patients, the interdisciplinary nature of the resulting damage, and their delayed ophthalmological evaluation. The oculomotor (III) nerve is the nerve most f …

[Principles and results of treatment in acquired paralysis of III, IV and VI nerves].

Between 1979-1994 120 patients with acquired paralysis of cranial nerves of ocular muscles were treated: 33 cases with paralysis of oculomotor nerve, 43 cases with paralysis of trochlear nerve, 44 cases with paralysis of abducens nerve. The majority of our patients were males (84-70.0%) aged 21-40 years (65-54.2%). The paralysis was most frequently caused by traffic accidents (45 cases--37.5%) and assaults (26 cases--21.7%). The most common symptom of paralysis was diplopia (109 patients--90.8%). Only 12 persons (10.0%) were admitted during the first month of paralysis and visual disorders cau

A prospective evaluation of cholinergic supersensitivity of the iris sphincter in patients with oculomotor nerve palsies.

To evaluate the frequency and clinical correlates of preganglionic cholinergic supersensitivity of the iris sphincter, I prospectively tested 31 consecutive outpatients with oculomotor nerve palsies by comparing their pupil diameters in darkness before and 30 minutes after topical application of pilocarpine 0.1%. Supersensitive pupil responses were observed in the involved eye in five of 11 patients with compressive palsies, four of five patients with traumatic palsies, in the two patients with congenital palsies, and in none of the patients with ischemic palsies. Supersensitivity was statisti

A Case of Epstein-Barr Virus Encephalitis and Orbital-Face Inflammation.

Epstein-Barr virus (EBV) can cause follicular conjunctivitis, keratitis, oculoglandular syndrome, meningitis, and encephalitis. We report a 54-year-old Hispanic male who presented with right pupil-involved complete ophthalmoplegia, orbital and masticatory muscle inflammation, trigeminal enhancement, and new corneal infiltrate highly suggestive of EBV. Labwork was negative except for positive EBV polymerase chain reaction (PCR) in serum. Magnetic resonance imaging (MRI) of his brain and orbits with contrast showed enhancement of the right ganglion of the trigeminal nerve, oculomotor nerve, all

First case of presumed trigemino-oculomotor synkinesis in a dog.

An 11-year old, intact male Border Collie was referred with a history of subacute and progressive left eye exophthalmos and mydriasis associated with reduced pupillary light reflex, ventrolateral strabismus, and absence of physiologic nystagmus in the left eye. Neuroanatomical localization was consistent with a left oculomotor neuropathy, involving the general somatic and visceral parasympathetic efferent components. Computed tomography and magnetic resonance imaging of the head were performed. Imaging findings were consistent with an infectious-inflammatory process involving the left retrobul

Preserved eye movements in adults with spinal muscular atrophy.

INTRODUCTION: Spinal muscular atrophy (SMA) most prominently affects proximal limb and bulbar muscles. Despite older case descriptions, ocular motor neuron palsies or other oculomotor abnormalities are not considered part of the phenotype. METHODS: We investigated oculomotor function by testing saccadic eye movements of 15 patients with SMA. Their performance was compared with that of age-matched healthy controls. Horizontal rightward and leftward saccades were recorded by means of video-oculography, whereas subjects looked at light-emitting diode targets placed at ±5°, ±10°, and ±15° eccentr

Internal carotid artery aneurysm with incomplete isolated oculomotor nerve palsy: a case report.

BACKGROUND: Oculomotor nerve palsy is a common and well-described disease with diverse etiologies. Clinicians should quickly and correctly diagnose and treat oculomotor nerve palsy according to its characteristics and the accompanying symptoms and signs. Intracranial aneurysm is an important and frequent cause of oculomotor nerve palsy. Considering the catastrophic consequences of rupture, the possibility of an urgent, life-threatening disease should always be considered. CASE PRESENTATION: A 63-year-old Chinese woman presented with intermittent left ptosis and diplopia and painless incomplet

Transnasal prelacrimal approach to the inferior intraconal space: a feasibility study.

BACKGROUND: Endonasal access to the inferomedial and inferolateral intraconal space via the orbital floor has not been reported. The primary purpose of this study was to assess the feasibility of accessing the inferior intraconal space through the orbital floor via a transnasal prelacrimal approach. Secondarily, it aims to highlight anatomical relationships of neurovascular structures in this space, as a requirement to prevent complications. METHODS: Six cadaveric heads (12 sides) were dissected using a transnasal prelacrimal approach. The orbital floor, medial to the infraorbital canal, was

Combined recession-resection in the management of paralytic strabismus: simplification of the complex (a case report).

Oculomotor nerve palsy presents the greatest challenge in clinical treatment among the other cranial nerves that innervate extraocular muscle. With a complex technique of periosteal fixation using Callahan suture commonly used, a simpler and more effective technique is demanded to be proven. Here we present a case of paralytic exotropia due to oculomotor nerve palsy on a 32-year-old male along with evidence supporting our decision of surgical management. A 32-year-old male with a chief complaint of squint in both eyes was examined at an outpatient eye clinic. The patient had congenital isolate

Surgical management of oculomotor nerve palsy - a review of the literature.

We aim to review the surgical management techniques available for patients with oculomotor nerve (OMN) palsy, compare and contrast the approaches and subsequent outcomes. A search of the literature was carried out to yield all papers relevant to the topic, and a wide spectrum of surgical techniques were identified. These included: muscle shortening and lengthening procedures, muscle transposition, globe fixation and ptosis surgery. Patients often require a synergistic combination of these techniques. Strabismus surgery for OMN palsy can be approached through a variety of different techniques t

Distance between intramuscular nerve and artery in the extraocular muscles: a preliminary immunohistochemical study using elderly human cadavers.

PURPOSE: Extraocular muscles are quite different from skeletal muscles in muscle fiber type and nerve supply; the small motor unit may be the most well known. As the first step to understanding the nerve-artery relationship, in this study we measured the distance from the arteriole (25-50 μm in thickness) to the nerve terminal twigs in extraocular muscles. MATERIALS AND METHODS: With the aid of immunohistochemistry for nerves and arteries, we examined the arteriole-nerve distance at 10-15 sites in each of 68 extraocular muscles obtained from ten elderly cadavers. The oblique sections were nea

Parallel cholinergic circuit in oculomotor nucleus to control eye movements and REM sleep.

Sleep suppresses most motor output, yet rapid eye movement sleep (REMs) is marked by brief muscle twitches, including eye movements (EMs), indicating selective motor preservation. How these opposing features are coordinated remains unclear. Here we show that cholinergic neurons in the oculomotor nucleus (nIIIChAT) possess dual functional organization governing REMs termination and oculomotor execution in mice. We identify a ventrolateral periaqueductal gray-projecting nIIIChAT subset whose activity dissociates from EMs but undergoes a progressive increase prior to REMs termination. Optogenetic

[Differential diagnosis of ptosis (author's transl)].

An accurate differential diagnosis of ptosis results if one refers the functional disorders of the levator to the different levels of the motor unit: Muscle, synapsis, peripheral neuron and nucleus. Therefore ptosis can be classified into myogenic, synaptogenic and neurogenic forms. This classification is also based on electromyographic findings from the levator muscle. Myogenic forms of the ptosis are: Senile ptosis, chronic progressive ocular muscle dystrophy, acute exophthalmic myositis and affections of the levator together with general systemic myopathies. Synaptogenic forms of ptosis are
