Iron-Rim Lesions in the Optic Chiasm in Multiple Sclerosis: A Neuropathologic Case Series.

OBJECTIVES: The aim of this study was to demonstrate the presence of iron-rim lesions (IRLs) in the anterior visual pathway in multiple sclerosis (MS). METHODS: Postmortem formalin-fixed paraffin-embedded anterior visual pathway tissues (optic chiasms) from 3 donors with MS were examined for iron using Turnbull Blue histochemistry. Demyelinated lesions were identified and staged with immunohistochemistry for myelin proteolipid protein and CD68. RESULTS: A total of 9 lesions were identified, all of which were mixed active/inactive. Iron-positive microglia and macrophages were identified at th…

Enlargement of the Optic Chiasm: A Novel Imaging Finding in Glutaric Aciduria Type 1.

Patients with glutaric aciduria type 1, without early diagnosis and initiation of preventive treatment, often develop movement disorders and … , a novel imaging finding present regardless of a previous metabolic crisis: the enlargement of the optic chiasm associated with signal abnormalities in the anterior intracranial visual structures observed in 6 of 10 patients. These optic pathway abnormalities are suggested as useful diagnostic clues for glutaric aciduria type 1, and possible pathophysiologic …

Nogo receptor inhibition enhances functional recovery following lysolecithin-induced demyelination in mouse optic chiasm.

BACKGROUND: Inhibitory factors have been implicated in the failure of remyelination in demyelinating diseases. Myelin associated inhibitors act through a common receptor called Nogo receptor (NgR) that plays critical inhibitory roles in CNS plasticity. Here we investigated the effects of abrogating NgR inhibition in a non-immune model of focal demyelination in adult mouse optic chiasm. METHODOLOGY/PRINCIPAL FINDINGS: A focal area of demyelination was induced in adult mouse optic chiasm by microinjection of lysolecithin. To knock down NgR levels, siRNAs against NgR were intracerebroventricular…

Penetration of the optic chiasm by a ruptured anterior communicating artery aneurysm. Case report.

There are few reports of anterior communicating artery aneurysms causing visual symptoms, and penetration of the optic chiasm by such aneurysms has not been reported. A 40-year-old man presented with the abrupt onset of left homonymous hemianopsia, right visual acuity disturbance (finger counting), and slight headache. Angiography disclosed a 7-mm anterior communicating artery aneurysm projecting inferiorly. After the neck of the aneurysm was clipped, the dome of the aneurysm was resected. The operation confirmed that the aneurysm had penetrated the right half of the optic chiasm and the throm…

[Heredo-familial arachnoiditis of the optic chiasm (clinical picture and results of neurosurgical treatment)].

The authors report that Leber's optic atrophy is a familial hereditary optochiasmatic arachnoiditis: 70 patients from 44 families are analysed. In 17 families a medicogenetic analysis was conducted at least in three generations and a recessive type of heredity was determined in most cases. Fifty-five patients underwent operation after ineffective nonoperative treatment. Optochiasmatic arachnoiditis was found in all patients during surgery. Visual functions improved six months after the operation in half the patients. The similarity of the clinical picture of familial hereditary optochiasmatic

[Opportunities and challenges in the diagnosis and treatment of optic chiasm lesions: a clinical and research perspective].

The optic chiasm is a critical component of the visual pathway, and lesions in the pituitary and sellar regions can cause irreversible damage to a patient's visual function, resulting in a significant decrease in their quality of life. As a result, neuro-ophthalmology evaluation is a crucial part of the multidisciplinary treatment of pituitary diseases. However, due to the significant variation in the anatomical structure of the optic chiasm and the sellar region, as well as the complexity of the injury mechanism, chiasm injury can result in diverse manifestations and severity levels, which ca…

Atrophy of the optic chiasm is associated with microvascular diabetic complications in type 1 diabetes.

INTRODUCTION: Diabetic neuropathy and diabetic eye disease are well known complications of type 1 diabetes. We hypothesized that chronic hyp … ng (MRI). Two different raters manually measured the optic tract. RESULTS: The coronal area of the optic chiasm was smaller among those with type 1 diabetes compared to non-diabetic controls (median area 24.7 [21.0-28.5] vs 30.0 [26.7-33.3] mm2, p<0.001). In participants with type 1 diabetes, a smaller chiasmatic area was associated with duration of diabetes, …

Atrophy of optic nerve fibres in compression of the chiasm. Observer variation in assessment of atrophy.

Different observers' assessment of atrophy according to a semi-quantitative method was analysed. Five observers scored the degree of atrophy on magnified black-and-white fundus photographs from normal controls and from patients with chiasmal lesions of different severity. Four of the five examiners achieved a reasonably uniform assessment of atrophy. The results of the test prompted some modification in the grading system.

Atrophy of optic nerve fibres in compression of the chiasm. Degree and distribution of ophthalmoscopic changes.

Chiasmal lesions produce a characteristic form of partial optic atrophy, subtly reflected by objective changes in the peripapillary portion of the retinal nerve fibre layer. The degrees and distributions of these changes were documented by fundus photography in a series of steady state patients with different degrees of chiasmal damage from pituitary adenoma. Semi-quantitative analysis disclosed a close correspondence with the degree of functional deficit. The state of the retinal nerve fibre layer can therefore be used as an objective indicator of the occurrence and severity of anatomical, ch

BMP Signaling Interferes with Optic Chiasm Formation and Retinal Ganglion Cell Pathfinding in Zebrafish.

Decussation of axonal tracts is an important hallmark of vertebrate neuroanatomy resulting in one brain hemisphere controlling the contralateral side of the body and also computing the sensory information originating from that respective side. Here, we show that BMP interferes with optic chiasm formation and RGC pathfinding in zebrafish. Experimental induction of BMP4 at 15 hpf results in a complete ipsilateral projection of RGC axons and failure of commissural connections of the forebrain, in part as the result of an interaction with shh signaling, transcriptional regulation of midline guidan…

Transnasal Transsphenoidal Elevation of Optic Chiasm in Secondary Empty Sella Syndrome Following Prolactinoma Treatment.

BACKGROUND: Prolactinomas are typically treated nonsurgically with a dopamine agonist. Once the tumor shrinks, adjacent eloquent structures, such as the optic apparatus, can become skeletonized and herniate into the dilated parasellar space. CASE DESCRIPTION: A 48-year-old man with a prolactin-secreting macroadenoma treated with cabergoline presented with progressive bitemporal hemianopsia. Magnetic resonance imaging showed no recurrence of disease and a stretched optic chiasm herniating into an empty sella. Elevation of the optic chiasm via a transnasal transsphenoidal approach with ALLODERM…

Optic Chiasm Involvement With Concurrent Typical Wernicke Encephalopathy Magnetic Resonance Findings: A Case Report.

The variable clinical presentation of Wernicke encephalopathy often complicates interpretation. Prompt and accurate diagnosis relies on a constellation of typical and atypical magnetic resonance imaging (MRI) findings, which are not always simultaneously present. Our case demonstrates concurrent presentation of all typical Wernicke encephalopathy MRI findings with additional signal abnormalities involving the optic chiasm and optic tract. We suggest that optic pathway involvement may be considered among several atypical MRI manifestations, reinforcing the prompt diagnosis of the potentially li…

Retinal axon misrouting at the optic chiasm in mice with neural tube closure defects.

In a new mouse mutant, circletail (Crc), failure of neural tube closure (embryonic day [E] 8-9) is associated with errors in retinal axon projection at the optic chiasm (E12-18), such that many axons normally projecting contralaterally instead grow to ipsilateral targets. Although the architecture of the chiasmatic region is altered, neurons and glia containing putative cues for axon guidance are present. The aberrant ipsilateral-projecting cells originate from a nonrandom expansion of the wild-type uncrossed retinal region. These axon pathway defects are found in two other mutants with cephal…
