BACKGROUND: Despite numerous clinical studies regarding the management of unilateral impalpable testes by laparoscopy, there is a paucity of such studies regarding the management of bilateral 'non-palpable' testes. PURPOSE: A number of variables should be assessed during laparoscopic orchiopexy, including vas anatomy. A looping vas deferens enters the inguinal canal and loops back to the abdominal cavity. This anatomical variant is not uncommonly encountered. Some groups considered this condition in their laparoscopic classification of nonpalpable testis. We report a case of spermatic cord torsion in the contralateral testicle 5 years after surgical fixation. Since diagnosis of such a lesion is difficult a high degree of suspicion is necessary. If torsion is suspected exploration is mandatory. We strongly recommend the use of a nonabsorbable reaction-producing suture (for example silk) when an orchiopexy is performed, with the point of fixation being between the visceral tunica vaginalis and the dartos muscle.

During 28 orchiopexies in 22 boys, an analysis was made of the increased length of cord that was obtained at the end of 3 aspects of the operation--after division of the cremaster, after excision of the sac and after retroperitoneal dissection. The mean gain in length of the cord was analysed in relation to the original position of the testis and was greatest in the ectopic testis and least in the testis in the inguinal canal. Each part of the operation produced worthwhile increases in length, but bore no relationship to the age of the child at operation.

Data were collected on 27 patients operated on for bilateral undescended testes. The information included age at operation, reoperations, testis size and location at first operation, testis size in adulthood, sexual behavior and presence of secondary sexual characteristics. Of 170 patients having undergone orchiopexy for unilateral cryptorchid testes, 90 responded to our inquiry. Testicular size was measured in 55 patients using an accurate and practical orchiometer (punched-out orchiometer) developed in our department.
