A patient with bronchiolitis obliterans organizing pneumonia (BOOP) associated with acute Mycoplasma pneumoniae infection is described. Although mycoplasmas have occasionally been associated with bronchiolitis obliterans, to the best of our knowledge, this is the first well-documented case of BOOP associated with M. pneumoniae infection. The diagnosis of BOOP was made by open-lung biopsy after fiberoptic transbronchial biopsy proved nondiagnostic. Corticosteroid therapy was administered and a dramatic improvement was observed; the patient remained without complaints during a 3-year follow-up p…

Bronchiolitis obliterans organizing pneumonia (BOOP) occurred in a 53-year-old woman with well-documented Sjögren's syndrome (SjS) and polymyositis (PM). BOOP has often been reported as a pulmonary manifestation of collagen vascular diseases, mainly rheumatoid arthritis (RA), but the association of BOOP and PM has rarely been documented. A search of the literature showed only 16 case reports of BOOP associated with polymyositis-dermatomyositis (PM-DM). It is interesting that BOOP occurred prior to PM-DM, while it is commonly believed to occur after RA.

Acute fibrinous and organizing pneumonia (AFOP) is an unusual pathological pattern which is characterized by intra-alveolar deposition of fibrin (fibrin ball) and organizing pneumonia in a scattered distribution, and the pathological diagnosis plays an irreplaceable role in the diagnosis. Most Patients cannot confirm etiology, till now, known etiology included connective tissue disease, infection, environmental and occupational exposure, drugs, organ transplant, and tumor. It can be divided into acute and subacute subtype according to the extent of progress. The most common symptoms of AFOP we…

Acute fibrinous and organizing pneumonia (AFOP) is a disease of the small airways that is characterized by deposition of fibrin within the alveolar spaces. The histological pattern is described as a variant of cryptogenic organizing pneumonia (COP). Although COP has been occasionally described in patients with HIV infection, the variant form, AFOP, has not been previously reported in such patients. This report describes an intriguing case of AFOP in a patient with HIV infection and Pneumocystis jiroveci pneumonia. AFOP was diagnosed after tapering of corticosteroid therapy. This case illustrat…

This pattern of lung injury is seen in the organizing phase of diffuse alveolar damage, organizing pneumonia (OP), acute fibrinous and organizing pneumonia, and certain types of fibrotic lung disease. Organizing pneumonia (OP) is a rare pulmonary manifestation in patients with inflammatory bowel disease (IBD). Distinguishing between a primary extra-intestinal manifestation of Crohn's disease (CD) and mesalazine-induced lung injury remains a major clinical challenge, as both conditions are steroid-responsive. We report the case of a 31-year-old woman with CD who developed progressive dyspnea and migratory pulmonary infiltrates one month after initiating mesalazine. Laboratory findings revealed marked peripheral eosinophilia (1276/mm3) and a mixed eosinophilic bronchoalveolar lavage profile (…

BACKGROUND: Amiodarone-induced pulmonary toxicity (APT) has a broad clinical spectrum, and its radiologic and histopathologic appearances vary considerably. Granulomatous lung injury, however, is rarely described and may be confused with organizing pneumonia (OP) or infection.
CASE PRESENTATION: An elderly man who had been receiving 5-month amiodarone therapy developed a 3-week history of pleuritic chest pain and progressive dyspnea. Chest computed tomography (CT) demonstrated bilateral ground-glass opacities and subpleural-predominant consolidations, with scattered reversed halo signs raisin…

BACKGROUND: Acute fibrinous and organizing pneumonia (AFOP) is a rare form of pneumonia, is characterized by the deposition of fibrin in alveoli, the formation of fibrin spheres, and deposition of fibrin in alveolar junctions and bronchioles adjacent to or adjacent to the alveoli, forming institutional loose connective tissue. The clinical characteristics of AFOP lack specificity. We report a special case of AFOP that may be associated with Candida, so as to improve our understanding and diagnosis of AFOP.
RESULT: In this patient who was early misdiagnosed with community-acquired pneumonia (CA…

A 72-year-old woman underwent a close examination because of chest computed tomography (CT) scan revealed a nodule in the left lower lobe of the lung. Positron emission tomography( PET) showed strong accumulation of fluorodeoxyglucose (FDG) in the lesion. Since lung cancer was strongly suspected, video-assisted thoracoscopic lung biopsy was performed. The lesion was diagnosed as organizing pneumonia by pathology. PET is widely used to distinguish between benign and malignant lung nodules, but FDG accumulation can also be seen in benign diseases such as inflammatory lesions. Abnormal accumulati…

Organizing pneumonia is characterized by a distinct histologic pattern in the lung interstitium and presents clinically as hypoxemia, fever, cough, and dyspnea that is not attributable to concurrent infection. Typical etiologies of this condition include inflammatory disease, malignancy, toxic inhalation, and an array of medications including the mTOR inhibitor everolimus. In this report, we describe the case of a female with tuberous sclerosis complex on everolimus therapy for renal angiomyolipomas who presented to the hospital with persistent cough, dyspnea, and fevers and bilateral lower lo…

A 70-year-old man presented to our hospital because of a cough with bright red blood for 1 month. A chest computed tomography (CT) scan show … distal airspaces and small bronchioles. These findings supported the diagnosis of focal cryptogenic organizing pneumonia. The patient was then treated with ceftazidime and prednisone. After this treatment, the patient visited the clinic and complete resolution of his respiratory symptoms and nearly complete resolution of the mass on chest CT were observed. The finding …

We reported an autopsy case of a 58-year-old-man, who developed intractable organizing pneumonia accompanying myelodysplastic syndrome with neutrophil hypofunction. Slight clinical improvement was obtained by steroid pulse therapy, but re-exacerbation occurred during steroid tapering. Infection was suspected, and we administered antibiotics and antifungal agents, while holding steroid administration. However, both the clinical findings and images deteriorated. By both steroid and cyclosporine treatment, a slight improvement was temporarily obtained, but he died. The findings of lung tissue on …

We report the case of a 62-year-old man who developed bronchiolitis obliterans with organizing pneumonia (BOOP) associated with acute Coxiella burnetii infection. The diagnosis of BOOP was made by fiber-optic transbronchial biopsy. Treatment with corticosteroids resulted in rapid clinical improvement and complete resolution of airspace opacities. To the best of our knowledge, this is the first well-documented case of BOOP associated with C. burnetii infection. This case suggests that this infection might trigger the development of BOOP.

We report five patients with chest roentgenograms showing peripheral infiltrates similar to that described for chronic eosinophilic pneumonia (CEP). While lung biopsy specimens (transbronchial and open in two and transbronchial only in three cases) revealed typical changes of bronchiolitis obliterans organizing pneumonia (BOOP) as the predominant finding in all cases, two cases had changes consistent with a resolving CEP. We speculate that (1) it may be common for BOOP to present with peripheral infiltrates, (2) chest roentgenograms showing peripheral infiltrates are not diagnostic of any spec…
