[The participation of RANK, RANKL and OPG in tumor osteolysis].

It was recently shown that physiological bone remodeling depends on the dynamic balance of two cytokines that are predominantly secreted by osteoblasts. RANKL promotes the differentiation of osteoclastic precursors and the activation of osteoclasts, whereas osteoprotegerin (OPG)inhibits RANKL action. During the development of many tumors, enhanced osteolysis results in pathological bone destruction. Tumor-associated osteolysis is characterized by the degradation and inhibition of osteoprotegerin (OPG) and increased RANKL expression and secretion by tumor tissue. The resulting RANKL/OPG imbalan…

Impediment of NEMO oligomerization inhibits osteoclastogenesis and osteolysis.

The transcription factor NF-kappaB is essential for osteoclastogenesis and is considered an immune-modulator of rheumatoid arthritis and inflammatory osteolysis. Activation of NF-kappaB subunits is regulated by the upstream IkappaB kinase (IKK) complex which contains IKKalpha, IKKbeta, and IKKgamma; the latter also known as NF-kappaB essential modulator (NEMO). The role of IKKalpha and IKKbeta in the skeletal development and inflammatory osteolysis has been described, whereas little is known regarding the role of NEMO in this setting. Typically, signals induced by RANK ligand (RANKL) or TNF pr…

Osteolysis in patients with a metal-on-metal hip arthroplasty.

BACKGROUND: Despite design and manufacturing improvements in contemporary metal-on-metal hip replacements, the problem of wear particles persists. The local and systemic biological consequences of this ionic debris have been the subject of much investigation and it has become clear that cell-mediated delayed-type hypersensitivity reactions are a discrete mode of osteolysis in susceptible patients. METHODS: We have carried out a consecutive series of 125 primary hip replacements using the Metasul (Zimmer, Warsaw, IN, USA) bearing couple. The radiographs, periprosthetic tissue and retrieved imp…

Massive osteolysis due to failure of a unipolar endoprosthesis.

Unipolar endoprostheses are commonly used for the treatment of displaced femoral neck fractures in the elderly. Failure due to polyethylene wear, which has been well documented in bipolar endoprostheses, is not a commonly reported problem with unipolar prostheses. We present 2 cases of a particular unipolar design that failed due to polyethylene wear. We report these cases to promote awareness of this particular reason for failure in this design. We also question the need for polyethylene in any prosthetic, when it is not indicated for the bearing surface.

Wear particles, periprosthetic osteolysis and the immune system.

The immune system modulates many key biological processes in humans. However, the exact role of the immune system in particle-associated periprosthetic osteolysis is controversial. Human tissue retrieval studies, in vivo and in vitro experiments suggest that the immune response to polymer particles is non-specific and macrophage-mediated. Lymphocytes may modulate this response. However direct lymphocyte activation by polymer particle-protein complexes seems unlikely. However, metallic byproducts may complex with serum proteins and lead to a Type IV, lymphocyte-mediated immune reaction. In pred…

[Involvement of immunogenetic factors in the development of periprosthetic osteolysis].

Aseptic loosening and osteolysis are the most frequent causes of total hip or total knee arthroplasty failure. Osteolysis is induced predominantly by polyethylene particles that are produced by adhesive wear of the prosthesis. The particles trigger a complex host's reaction varying in intensity even in response to the same number of particles. These differences indicate that individual predisposition may have an important role in the pathogenesis of osteolysis. The major key mediators of wear-induced osteolysis include the cytokines RANKL, TNF-a, IL-1, IL-6 and IL-8. The inter-individual diffe…

Familial expansile osteolysis--not exclusively an adult disorder.

Familial expansile osteolysis (FEO, MIM174810) is a rare syndrome which was observed world-wide in only three kinships and in two unrelated American individuals. We report a patient with familial expansile osteolysis from the Czech Republic, not related to the previously reported cases. This patient's extraordinary clinical course does not conform to the ordinary. Her radiographic bone involvement was unusually extensive, involving most of the peripheral skeleton and the skull. This case documents that familial expansile osteolysis is not only a disease of adults but does occur in childhood.

Comparison of VEGF-producing cells in periprosthetic osteolysis.

The pro-angiogenic cytokine vascular endothelial growth factor (VEGF) has been implicated in periprosthetic osteolysis and subsequent aseptic loosening of implants following total hip arthroplasty (THA). The goal of this study was to investigate whether increased VEGF at the bone-implant interface is secondary to a greater number of VEGF-producing cells or to increased VEGF production by individual cells. Real time polymerase chain reaction (RT-PCR) techniques were used to assess the expression of VEGF mRNA (isoforms 121, 165, 189) in periprosthetic tissues from revision THAs. Immunofluorescen…

The central role of wear debris in periprosthetic osteolysis.

Periprosthetic osteolysis remains the leading complication of total hip arthroplasty, often resulting in aseptic loosening of the implant, and a requirement for revision surgery. Wear-generated particular debris is the main cause of initiating this destructive process. The purpose of this article is to review recent advances in our understanding of how wear debris causes osteolysis, and emergent strategies for the avoidance and treatment of this disease. The most important cellular target for wear debris is the macrophage, which responds to particle challenge in two distinct ways, both of whic…

Atraumatic osteolysis of the distal clavicle: MR findings.

OBJECTIVE: The purpose of this study was to describe the MRI appearance in atraumatic osteolysis of the distal clavicle (AODC). MATERIALS AND METHODS: We retrospectively evaluated MRI, medical records, ancillary diagnostic imaging studies and clinical course in five men and two women (mean age, 39 years) in whom the final clinical diagnosis of AODC was established. None of the patients had significant shoulder injury, but all participated in activities involving repetitive strain of the acromioclavicular (AC) joint. In three of these patients, we performed follow-up MRI (ranging from 5 1/2 to…

Foramen magnum impaction in a case of acro-osteolysis.

A case of a rare, non-familial, generalized, congenital, osseous dysplasia is described with features which have previously been described under the name 'acro-osteolysis'. Basilar invagination followed defective bone formation in the skull and this produced occipital headache and progressive neurological deterioration with involvement of the lower cranial nerves and cerebellar function. Hydrocephalus and valvular impaction of the cerebellum in the foramen magnum were demonstrated. Partial relief was obtained by foramen magnum decompression; death supervened from infection and respiratory inad…
