Mucormycosis Osteomyelitis of the Hand.

Mucormycosis is a relatively uncommon but highly aggressive fungal infection caused by the Mucormycotina subgroup of fungi. Mucormycosis usually affects the skin, sinuses, or lungs of an immunocompromised host. Other risk factors for development of mucormycosis include hematologic malignancy, intravenous drug abuse, chronic steroid use, and contaminated traumatic wounds. Cutaneous mucormycosis of the hand involving the skin and soft tissues is rare but has been reported previously. In this case report, we review the clinical course of a patient with mucormycosis of the hand who was found to ha

Paediatric Osteomyelitis in Fiji.

INTRODUCTION: Osteomyelitis can lead to significant morbidity and long-term disability if early treatment is not initiated in a timely manner. For developing countries this can lead to a significant burden on the healthcare system. This study aims to describe the demographic variables, aetiology and outcomes of treatment and to calculate the incidence of paediatric osteomyelitis in Fiji. The micro-organism profile and the outcomes for treatment were analysed. METHODS: This is a retrospective review of medical records of all paediatric patients presenting to hospitals in Fiji over a 5-year per…

Emphysematous osteomyelitis of the forefoot.

Emphysematous osteomyelitis is a very rare, potentially fatal infection that requires immediate diagnosis and prompt treatment. Emphysematous osteomyelitis is usually considered whenever intraosseous gas is detected on imaging. Most organisms implicated in emphysematous osteomyelitis are members of the Enterobacteriaceae family or anaerobes; sometimes the infection is polymicrobial. We report a case of emphysematous osteomyelitis of the forefoot in a 33-year-old man with type 1 diabetes mellitus.

Actinomycotic Osteomyelitis of Mandible.

Actinomycosis is an infection of filamentous, gram-positive anaerobic bacteria. Actinomycosis of jaw bone is rare and the diagnosis is often difficult. The aim of this patient report is to present an actinomycotic osteomyelitis of mandible in a 30-year-old male patient. The patient presented with a chief complaint of pain and unhealed dental socket after mandibular teeth extractions. Radiographs showed ill-defined radiolucencies and perforation of buccal and lingual cortical plate; several biopsies and different therapy could not improve the lesion because of fault diagnosis and improper treat…

Chronic osteomyelitis of the pelvis.

The aim of this study was to examine the aetiology and response to treatment of a series of patients with pelvic osteomyelitis. Criteria for selection were multiple positive intra-operative cultures and/or a positive radiological diagnosis. Twenty patients met these criteria (age range: 21-78 years, mean = 46). Data was recorded on host status using the Cierny-Mader classification, neurological status, causative organisms; sensitivities were recorded and the treatment and its outcome. Pelvic osteomyelitis was frequently caused by unusual organisms; a high incidence (45%) of neurologically comp…

Cryptococcal osteomyelitis of the skull.

An otherwise healthy 65-year-old male from a rural area presented with a 1-month old non-tender scalp mass. He had a history of being stuck with a stone in the parietal region a year earlier but hadn't developed any complications. Needle aspiration of the mass revealed numerous yeast cells, which were confirmed to be Cryptoccus neoformans. This case describes a rare presentation of C. neoformans infection in a human immunodeficiency virus (HIV)-negative patient. Moreover, while osteomyelitis due to Cryptococcus is generally preceded by fungemia, in the present case it was caused by direct inoc…

Tuberculous osteomyelitis and spondylodiscitis.

Tuberculosis (TB) is no longer a disease limited to developing nations and is still a major cause of significant morbidity and mortality worldwide. The indolent clinical presentation, emergence of multidrug-resistant mycobacteria, and association with human immunodeficiency virus infection poses obstacles for early diagnosis and management. Compared with the other forms of TB, musculoskeletal involvement is relatively rare. Tuberculous spondylitis is the most common form of musculoskeletal TB and accounts for ~50% of cases. Extraspinal musculoskeletal TB shows a predilection for large weightbe

Pelvic osteomyelitis in children.

Pelvic osteomyelitis is a rare disease that presents like many other common paediatric orthopaedic conditions. The diagnosis and management of the disease is often delayed and as a consequence this may contribute to increased morbidity in affected individuals. In an attempt to improve our diagnostic accuracy, 23 cases of pelvic osteomyelitis presenting to our tertiary referral centre between 1998 and 2006 were reviewed. Case notes were reviewed to examine clinical, pathological and radiological variables attributable to pelvic osteomyelitis, as well as subsequent management of the disease. Pel…

[Classification and microbiology of osteomyelitis].

The clinical presentation of osteomyelitis is multifarious. Therefore, patients are diagnosed and treated by various specialists with many different concepts of optimal management. Originally, only the acute and the chronic presentations were differentiated. The classification of Waldvogel, which is based on pathogenetic mechanisms, is more sophisticated. Diabetic foot is classified according to Wagner, who takes into account the continuous progression from sore to ulcer to osteomyelitis to gangrene. The staging according to Cierny-Mader is the most useful for the therapeutic management by sur…

Imaging of posttraumatic osteomyelitis.

Posttraumatic osteomyelitis frequently leads to an illness characterized by chronicity and recurrences. Bone changes themselves, developed during chronic infection, and orthopedic implants heavily influence the diagnostic value of the different imaging methods. Conventional radiography remains the cornerstone of imaging of acute and chronic osteomyelitis, but other modalities such as ultrasonography, radionuclide studies, computed tomography, and magnetic resonance imaging may be necessary. The values of the techniques in different clinical settings are described.

[Subacute osteomyelitis of the acetabulum].

A 15-months-old girl presented an excentration of the hip associated with a defect image of the acetabulum. Arthrography revealed a communication between the hip joint and the acetabular defect. Bacteriological specimens of the articular fluid were negative. The radiographic images were compatible with subacute osteomyelitis. Relative immobilization using an Atlanta abduction devise was undertaken. No medical treatment was prescribed for this adolescent who had received antibiotics for polymicrobial urinary tract infections and for pharyngitis prior to detection of the orthopedic disorder. Out…

Osteomyelitis in Polynesian children.

Polynesians, including New Zealand Maori, are known to be prone to bacterial infections. We studied 85 New Zealand children with osteomyelitis requiring admission in a tertiary care hospital in a 2-year period in order to attain information regarding incidence and relative risk. During the observation period, the hospital was responsible for the healthcare of a total population of 103,900 children per annum. An increased relative risk of Polynesian and Maori children to suffer from osteomyelitis was calculated to be 3.84. The major pathogenic organism was Staphylococcus aureus. Complications s…

Subacute osteomyelitis in children.

Subacute osteomyelitis has a very anodyne symptomatology and is, therefore, difficult to diagnose. We studied 21 cases treated between 1984 and 1998 with subacute osteomyelitis of variable location and a mean diagnostic delay of 158.5 days. Of these, 10 cases could not be placed in the current classification. Diagnosis was radiologic in all cases, although in a few patients confirmation by isotopic bone scan and magnetic resonance imaging was required. Treatment was surgical in the first 11 cases to become, currently, predominantly conservative. The causal microorganism was only isolated in ni…
