Systematic review of outcomes following observational and operative endoscopic middle ear surgery.

OBJECTIVES: Middle ear surgery increasingly employs endoscopes as an adjunct to or replacement for the operative microscope. We provide a systematic review of endoscope applications in middle ear surgery with an emphasis on outcomes, including the need for conversion to microscope, audiometric findings, length of follow-up, as well as disease-specific outcomes. DATA SOURCES: PubMed, Embase, and Cochrane CENTRAL database. METHODS: A literature review was performed using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis recommendations. Articles were categorized based on s

Treatment of age-related hearing loss in dogs with the vibrant soundbridge middle ear implant: short-term results in 3 dogs.

BACKGROUND: Age-related hearing loss (ARHL), or presbycusis, is the most common form of acquired hearing loss in dogs. Middle ear implants have been used successfully in people with ARHL who cannot benefit from conventional hearing aids. HYPOTHESIS: Audibility improves in dogs with ARHL after implantation of the Vibrant Soundbridge (VSB) middle ear implant. ANIMALS: Three Beagle dogs with ARHL, mean age 11.1 years. METHODS: The dogs were assessed pre- and postoperatively by brainstem-evoked response audiometry (BERA), otoscopy, and computed tomography scans of the ears. A VSB middle ear imp

Asymmetrical surface scanning registration for image-guided otologic surgery: A phantom study.

OBJECTIVE: To develop a registration procedure to achieve a higher degree of registration accuracy in image-guided otological surgery, paying particular attention to the registration centroid. METHODS: A head phantom was used to measure the target registration error (TRE) at measurement points at various depth from the surface of the head. The surface-matching registration was performed using a commercially available surgical navigation system. We registered the phantom using only one ear of either side (right 100% - left 0%, or right 0% - left 100%) or using both ears with variable ratios (r

Endoscopic ear surgery: A case series and first United Kingdom experience.

AIM: To present the United Kingdom's first case series of 70 otological cases of endoscopic and non-endoscopic ear surgeries. METHODS: Prospective case series incorporating a range of endoscopic procedures performed using a 4 mm, 18 cm rigid endoscope, performed by a single surgeon at a single centre. Primary outcome measures included mean average pre and post-operative air-bone gap hearing thresholds and duration of surgery. RESULTS: Thirty-eight patients underwent endoscopic assisted ear surgery and 32 underwent non-endoscopic assisted ear surgery. In both surgical groups, there was a sign

[Chorda tympani in the practice of an otosurgeon. Literature review].

The chorda tympani is one of the most traumatic anatomical structures during surgical interventions on the middle ear. The problem of intraoperative damage to the chorda tympani remains underestimated by otosurgeons due to its insufficient coverage in the medical literature. A search and analysis of modern scientific publications from publicly available information resources was conducted, including review articles, original research, monographs, observations of clinical cases, and textbooks from domestic and foreign authors. The anatomical and topographic features that contribute to the injur

[Advances in the surgical treatment of cholesteatoma of the middle ear].

Middle ear cholesteatoma, as a common ear disease, has been further understood in terms of its molecular mechanism, classification and staging, and it is generally believed that surgical treatment is the first choice, but there is no final conclusion on its surgical choice. Microscopic surgery includes complete or open radical mastoidectomy with or without reconstruction of the middle ear cavity, and their pros and cons have been debated for decades. In the past decade, a variety of surgical methods have been developed, including various improvements and combined operations that combine the ad

Analysis of aerosol production and aerosol dispersion during otologic surgery.

BACKGROUND: As a respiratory disease, the transmission of Coronavirus disease (COVID-19) is mainly caused by small droplets and aerosols. Healthcare personnel are particularly exposed during otologic surgery given the continuity with the nasopharynx, where the viral load is high, and the use of high-speed instruments. The purpose of the present study is to test a model of droplet dispersion produced in the performance of a drilling procedure on human bone to provide information about its distribution and size of the deposit in similar conditions to those of an operating theatre, to design diff

Does microscopic experience influence learning curve in endoscopic ear surgery? A multicentric study.

OBJECTIVE: The aim of the present study was to illustrate the learning curve of endoscopic type-1 tympanoplasty comparing experts in microscopic otology versus neophyte surgeons. METHODS: Eight ear surgeons, from tertiary referral centers, who had performed at least 30 endoscopic type 1 tympanoplasties were included in the study. Demographic data and medical records regarding the first 30 endoscopic type-1 tympanoplasties were retrospectively collected by each surgeon. A 14-questions survey focused on subjective aspects of the learning curve was administered. Surgeons were divided in two grou

[Maurice Sourdille, otologist of Nantes, famous and unknown].

Fenestration was the first surgical operation to improve deaf persons affected by otosclerosis. Its story is on a par with Maurice Sourdille's. Not only this author had perfected this surgery even before the use of antibiotics and audiometry but he was a forerunner in the field of ear surgery and in medical otology.

Endolymphatic sac revision for recurrent intractable Meniere's disease.

Meniere's disease can be observed uniquely in revisions of the conservative surgical procedure for endolymphatic sac enhancement, which preserves the labyrinth, unlike destructive procedures. Genetic anatomic abnormalities in patients lead to malabsorption of endolymph and symptoms of Meniere's that are reversed by endolymphatic sac enhancement, but disabling symptoms eventually may recur in a few cases and require revisional surgery. We observe developing pathophysiologic conditions intraoperatively and have modified our techniques to accommodate the redeveloped pathogenesis we observe and av
