Oxygen sensors in context.

The ability to adapt to changes in the availability of O2 provides a critical advantage to all O2-dependent lifeforms. In mammals it allows optimal matching of the O2 requirements of the cells to ventilation and O2 delivery, underpins vital changes to the circulation during the transition from fetal to independent, air-breathing life, and provides a means by which dysfunction can be limited or prevented in disease. Certain tissues such as the carotid body, pulmonary circulation, neuroepithelial bodies and fetal adrenomedullary chromaffin cells are specialised for O2 sensing, though most others

[Home oxygen therapy].

Oxygen therapy is one of the principal non-pharmacologic treatments for severe chronic obstructive pulmonary disease (COPD) patients. Home oxygen therapy(HOT), or long-term oxygen therapy(LTOT) for 15 hours or more per day, can improve the survival rate of severe COPD patients with beneficial effects on hemodynamic state, hematological characteristic, exercise capacity, lung mechanics, and mental state. Oxygen therapy is indicated in cases of severe chronic respiratory failure with PaO2 of 55 Torr or less, or in cases with PaO2 of 60 Torr or less in whom there is remarkable hypoxia during slee

The Universal Oxygen Connector.

The purpose of this article is to describe the benefits of using the Universal Oxygen Connector. Until now, an oxygen hose was only able to connect to a 22-mm fitting, such as those found on humidifiers used in the recovery room, and oxygen tubing was only able to connect to a Christmas tree type adapter. The Universal Oxygen Connector, manufactured and sold by International Medical, Inc (Burnsville, Minn), was developed to allow the practitioner to attach either a 22-mm oxygen hose, oxygen tubing, or a 15-mm oxygen adapter to the same connector. Patients benefit from the administration of sup

Oxygen sensing in the body.

This review is divided into three parts: (a) The primary site of oxygen sensing is the carotid body which instantaneously respond to hypoxia without involving new protein synthesis, and is historically known as the first oxygen sensor and is therefore placed in the first section (Lahiri, Roy, Baby and Hoshi). The carotid body senses oxygen in acute hypoxia, and produces appropriate responses such as increases in breathing, replenishing oxygen from air. How this oxygen is sensed at a relatively high level (arterial PO2 approximately 50 Torr) which would not be perceptible by other cells in the

Optic nerve oxygenation.

The oxygen tension of the optic nerve is regulated by the intraocular pressure and systemic blood pressure, the resistance in the blood vessels and oxygen consumption of the tissue. The oxygen tension is autoregulated and moderate changes in intraocular pressure or blood pressure do not affect the optic nerve oxygen tension. If the intraocular pressure is increased above 40 mmHg or the ocular perfusion pressure decreased below 50 mmHg the autoregulation is overwhelmed and the optic nerve becomes hypoxic. A disturbance in oxidative metabolism in the cytochromes of the optic nerve can be seen at

Hyperbaric oxygen and surgery.

Hyperbaric oxygen (HBO) therapy, that is the administration of 100% oxygen delivered under pressure, has a beneficial effect in several surgical conditions. Its use has been assessed and audited and its pharmacological effects demonstrated. It is appropriate for use in several surgical conditions as evidence-based therapy. These are: (i) gas gangrene; (ii) crush injuries, compartment syndromes and acute traumatic ischemias; (iii) enhancement of healing in selected problem wounds; (iv) exceptional blood loss anaemia; (v) necrotising soft-tissue infections; (vi) refractory osteomyelitis; (vii) r

[Pulmonary toxicity of oxygen].

Recent progress in artificial ventilation procedures sheds doubt on the toxicity of oxygen in the lung. Histological studies of the lung by light and electron microscopy were made in rats exposed to 100% oxygen at 1 ATA in spontaneous ventilation. The results are compared with the lesions observed in 19 human patients suffering from acute respiratory distress syndrome and with the data on retinopathy of prematurity. This study suggests to keep in mind that oxygen may, as well as infections or baro-traumatic factors, be involved in causing or aggravating lung fibrosis. The complexity of the mec

Oxygen delivery to tissues.

For health, well perfused tissues, oxygen uptake is determined primarily by metabolic need rather than by oxygen supply. Tissue hypoxia supervenes when tissue oxygen tension (PO2) falls below a critical point, and the point where this occurs can be predicted from the systemic oxygen delivery or extraction ratio. A growing body of evidence suggest that tissue oxygen extraction may be impaired in adult respiratory distress syndrome (ARDS) and sepsis. In these syndromes the minimum oxygen delivery needed to maintain a normal oxygen uptake appears to be increased, as tissues become hypoxic despite

Hyperbaric oxygen and scopolamine.

Scopolamine (Hyoscine), an anticholinergic compound is widely used for the prophylaxis and treatment of motion sickness and might be used with oxygen diving and hyperbaric oxygen therapy. We therefore decided to test the interaction of scopolamine with oxygen at high pressure. Thirty-six rats implanted with cortical EEG electrodes were injected subcutaneously with two doses of scopolamine (0.02 or 0.2 mg.kg-1), or the vehicle (saline), 30 min before exposure to 5 atm abs (0.5 MPa) oxygen. Electroencephalogram and heart rate were monitored continuously. Spectral analysis of the EEG was carried

Oxygen delivery and utilization.

Under normal circumstances, O 2 transport (TO 2 = cardiac output x arterial O 2 content) is regulated to provide sufficient O 2 to meet the demands of oxidative phosphorylation, quantified as the O 2 consumption (VO 2). When metabolic demands increase, TO 2 is augmented and in addition, the fractional extraction of the delivered O 2 by the tissues, the O 2ER, also increases, to levels as high as 0.80 at maximum VO 2. If TO 2 is decreased, at least in the experimental animal, VO 2 can be maintained initially by an increase in O 2ER, but eventually this mechanism is exhausted, VO 2 begins to fal

Vitreal and retinal oxygenation.

This paper reports the results of experiments carried out to understand the oxygenation of the normal retina in response to alterations in physiological conditions such as inspired oxygen concentration, elevated IOP, luminance changes and occlusion of the retinal circulation. Measurements of vitreal and intraretinal PO2 in vivo in the cat using oxygen-sensitive microelectrodes demonstrated that large PO2 gradients were set up preretinally and that arterial, venous and tissue PO2 increased when inspired PO2 was raised to 100% O2. Results showed that with an occluded retinal circulation, it was

Mixed venous oxygen.

In critically ill patients, measurement of mixed venous oxygenation is useful as an indicator of the adequacy of tissue oxygenation and as an indirect reflection of cardiac output. The flexible fiberoptic pulmonary artery catheter oximeter permits continuous in vivo measurements of the mixed venous oxygen saturation. However, in some groups of critically ill patients, such as those with adult respiratory distress syndrome or sepsis, measurements of mixed venous oxygen may be misleading and should be interpreted with caution.
