Nineteen cases of histiocytic cytophagic panniculitis from our institution and from the literature were reviewed for their clinical and histopathologic features. All patients had cutaneous involvement consisting of cutaneous nodules and plaques that occasionally were ecchymotic and ulcerated and were characterized by infiltration of the subcutaneous tissue by large, benign histiocytes with cytophagic features. Thirteen of the patients had died (nine with hemorrhagic complications), four patients had a benign course with remission of the disease, and two patients responded to aggressive polyche…

We report five cases of factitial panniculitis produced by blunt trauma. Localized forms include Secrétan's syndrome and l'oedeme bleu, which are usually unilateral and involve the hand and forearm, respectively. When the lesions are more generally distributed, the self-induced nature may be less apparent. These patients may have been diagnosed as having Weber-Christian disease, pancreatic fat necrosis, or superficial thrombophlebitis. The microscopic picture of organizing hematoma may be supplemented by the appearance of amorphous polysaccharide masses commingled with iron pigment.

A young woman had a history of subcutaneous nodules, migratory arthralgia, myalgia, and lymphadenopathy. Histopathologic characteristics of the nodular subcutaneous lesion showed a massive lobular panniculitis characteristic of lupus erythematosus (LE) panniculitis. The diagnosis of LE panniculitis was established by demonstration of a positive linear basement membrane zone stain of the involved skin by direct immunofluorescence microscopy. Initial serologic findings showed negative antinuclear antibody (ANA) and anti-DNA antibody test results and a negative LE cell preparation. Subsequently, …

We report a case of subcutaneous panniculitis-like T-cell lymphoma (SPTCL), associated with macrophage activation syndrome, mimicking a lupus erythematosus panniculitis (LEP). A 29-year-old woman presented with high fever, general malaise, nausea, vomiting, and subcutaneous nodules and ulcerating lesions located on the lower extremities. The histopathology showed an infiltration of the panniculus, mostly involving fat, and periadnexial and perivascular structures consistent with lymphocytic lobular panniculitis (LLP). LLP is a shared feature of LEP and SPTCL. The immunophenotyping of the cell …

Inflammatory infiltrates of the subcutis are a particular challenge in routine histopathology as they occur relatively rarely in submitted s … osclerosis). In addition, rarer adipose tissue diseases (alpha‑1 antitrypsin deficiency, pancreatic panniculitis, morphea profunda, eosinophilic fasciitis) and vasculitides that should be considered in the differential diagnosis are discussed.

Panniculitis is associated with a variety of disease processes that can be challenging to distinguish clinically. While diagnosis often relies on histopathology, sometimes this is not sufficiently diagnostic, which requires careful consideration of clinical features. We sought to provide an overview of the most common adult noninfectious panniculitis by reviewing not only the histological distinguishing features but also the clinical characteristics of various types of panniculitis including patient history, distribution, morphology, and other clues. A narrative review of both common and rare …
