A simple technique employing a penoscrotal tourniquet in the investigation and management of impotence due to venous leakage is described. Fifteen patients were investigated using a constriction band and intracorporeal injection of papaverine. Colour doppler ultrasound studies demonstrated that the tourniquet abolished venous leakage in 11 cases and there was a close correlation between clinical and ultrasonographic findings. Eighty-two patients were evaluated for erectile failure with a comprehensive history, physical examination, hormonal testing, noninvasive Doppler examination (PBI), and nocturnal penile tumescence (NPT). From February 1989 to June 1989, 49 patients consulted our OPD for the complaint of impotence. The average age was 46.9 +/- 11.2 y/0. We applied intracorporeal injection of papaverine (60 mg) by color duplex sonography (Acuson 128) for evaluation. The authors examined 68 patients with different forms of copulative disorders. Intracavernous papaverine injection is often used in the diagnosis and treatment of male impotence. Prolonged erection and/or priapism are well known local complications. Systemic side-effects characterized by discomfort and dizziness due to rapid escape of the drug into the vascular circulation also can occur in patients with venous leakage. Thus, venous leakage should be ruled out prior to intracavernous injection of papaverine in the treatment of impotence. Ligation of the deep dorsal vein and the small veins around the tunica albuginea will lessen the problem.

A study on the hypotensive action of combined injections of papaverine and octadine (guanethidine) was carried out in acute experiments on 90 anaesthetized cats. It was shown that with combined injections of octadine (in doses exceeding by 22% ED100) and of papaverine (in doses inferior by 22% to ED100) the hypotensive action was potentiated. A change of the said proportions led to a diminished hypotensive effect. INTRODUCTION: Intra-arterial papaverine (IAP) is used to treat symptomatic cerebral vasospasm following aneurysmal subarachnoid hemorrhage (SAH). IAP, however, can increase intracranial pressure (ICP). In this study we examined whether IAP alters brain oxygen (BtO2).
