To aim of the present paper was to introduce a novel fixation technique for the treatment of inferior pole fracture of the patella. We performed a prospective observational study of consecutive cases of inferior pole fracture of the patella that were treated at our institution between January 2018 and June 2019. The patients include three men and one woman, with an average age of 47 years (range: 42-59 years). All patients were treated with the novel rim plating fixation technique for preserving the inferior pole of the patella. During the surgery, a 2.4 mm straight locking compression plate w

CASE: A 14-year-old boy underwent open reduction and internal fixation (ORIF) of a tibial tubercle avulsion fracture. Seven weeks postoperatively, the patient slipped in the bathroom and was found to have a superior patellar pole periosteal sleeve avulsion injury.
CONCLUSION: We describe a rare case of periosteal sleeve avulsion fracture of the superior patella pole after tibial tubercle avulsion fracture ORIF in an adolescent male patient. The etiology of this injury pattern may be due to altered tensile forces transmitted through the extensor mechanism related to the previous surgery. Ortho

RATIONALE: Upper pole sleeve fractures of the patella are rare in adolescents; however, they are serious injuries that require early diagnosis and treatment.
PATIENT CONCERNS: We present a rare case of a 15-year-old girl who suffered a sleeve fracture at the superior pole of the right patella. The patient had a history of dislocation of the patella 2 weeks ago. Physical examination showed tenderness on palpation over the upper pole of the patella and absence of active movement of the knee accompanied by swelling and joint effusion. A plain radiograph showed an avulsed fragment of the superior

OBJECTIVES: Diagnosis of Sinding-Larsen-Johansson disease may not be an easy task. Several sport-related conditions affect the distal pole of the patella in the adolescent, and treatment varies considerably. The article describes a patient that had radiographic features of Sinding-Larsen-Johansson disease associated with an atypical acute presentation.
METHODS: Case report and literature review.
RESULTS: A 10-year-old boy presented with a sudden pain after a noncontact soccer injury. He had tenderness and swelling over the patella. Radiographs showed minimally displaced distal patellar ossic

We present the case of an 18-year-old male who presented following a high-energy motor vehicle accident with direct trauma to both flexed knees. Clinical examination revealed bilateral open anterior knee wounds with exposed bone fragments and intact distal neurovascular status. Plain radiographs confirmed bilateral displaced transverse patellar fractures (Gustilo-Anderson type II). The patient underwent urgent single-stage open reduction and internal fixation of both patellae under general anesthesia. Thorough debridement and pulsed-saline lavage were performed bilaterally, followed by anatomi

For extensor mechanism deficiency in the osteoarthritic knee, mesh reconstruction of the extensor mechanism is an accessible option. Studies have shown that its rates of infection and graft failure are noninferior compared to allograft reconstruction. This case report presents a 51-year-old patient who presented with chronic fracture nonunion of the inferior pole of the patella, associated with pain, extension lag, and osteoarthritis. Since the anterior knee was previously treated with partial-thickness skin graft, soft tissue reconstruction was performed prior to mesh reconstruction of the ex

BACKGROUND: Open reduction and internal fixation (ORIF) represents a standard surgical technique for the management of patellar fractures, with tension band fixation serving as the commonly employed method. To enhance fixation strength, wires are frequently threaded through the medullary canal of the bone. This study aims to evaluate the potential benefits of utilizing transosseous wire fixation in the treatment of patellar fractures and its impact on patient outcomes.
METHOD: In this study, 223 patients with patellar fractures who had undergone open reduction and tension band surgery at our

BACKGROUND: A patellar avulsion fracture is a unique fracture type in children with an extremely low incidence rate in knee joint injuries, most of which are unilateral. Cases of simultaneous bilateral fracture are rare. Due to the physiological and anatomical characteristics of patellar sleeve fractures in children, obvious avulsed bony fragments are seldom seen with plain radiography after trauma; thus, this fracture type is often misdiagnosed or missed on first evaluation. Early diagnosis of patellar sleeve fracture is the key to successful treatment.
CASE REPORT: This paper presents a cas

INTRODUCTION: Gap non-union patella fractures are rare but can be particularly challenging for the orthopedic surgeon. We report a case of a 12 cm gap non-union patella fracture in a patient with osteoporosis and its surgical management, functional outcome, and implications on clinical practice.
CASE REPORT: A 73-year-old fully independent female with a background of osteoporosis was referred to our outpatient knee clinic. She reported a fall from stairs following an initial soft-tissue injury to the left knee 6 months prior. She has had progressive decline in mobility since the injury and wa…

Treatment of patella fractures is fraught with complications and historically poor functional outcomes. A fixation method that allows for early mobilization and decreases symptomatic hardware rates will improve knee range of motion, postoperative functional status, and reoperation rates. The purpose of this study was to evaluate the functional outcomes after locked plate osteosynthesis of patella fractures at a Level 1 trauma center. A retrospective case series was conducted of patients who underwent open reduction internal fixation (ORIF) of a patella fracture using a locked mesh plating tech…

Nonunion and delayed union of patella fractures are rare conditions that are fraught with challenges. There exists limited evidence in the literature to guide the clinician in the management of these complications. Therefore, the purpose of this study was to perform a systematic review of the available evidence on the management of nonunion and delayed union of patella fractures. A total of five publications which met our criteria were identified and formed the basis of this study. The decision making in the treatment of this condition is based on the functional demands of the patient, the fac…

Bilateral simultaneous fractures of the patellae were first documented in the literature in the early 19th century, and they remain uncommon today. The patella is the largest sesamoid bone in the body and plays an important role in the knee extensor mechanism. Isolated bilateral patellar fractures following trauma are exceedingly rare (2-3% of all patellar fractures) and most commonly occur with dashboard injuries or various pathological disorders. We report a case of a 43-year-old healthy African male who presented with bilateral simultaneous extensor mechanism disruption following knee traum

Patellar fractures are common injuries that compromise the extensor mechanism of the knee. Tension band wiring (TBW) is a widely accepted surgical technique for simple mid-pole fractures, but is associated with complications like wire migration and breakage. This case report presents a modified TBW technique designed to improve biomechanical stability and reduce complications. A 51-year-old female patient presented with right knee pain and swelling following a fall. Radiographs confirmed a transverse patella fracture. Surgical fixation was performed using a modified TBW technique involving tri…

OBJECTIVE: Most inferior pole fractures of the patella are comminuted. Therefore, an ideal treatment method has not been determined. We have presented a modified tension band fixation technique-the Krachow suturing, Nice knot combined with tension band fixation-and reported the results of the procedure.
METHODS: A total of 16 inferior patellar pole fractures were treated at our institution between January 2019 and October 2020, 15 of which underwent treatment with the modified tension band fixation technique consisting of Krachow suturing with Nice knots combined with tension band fixation. T

Vertical stress fractures of the patella are rare, with only a handful of bilateral cases reported. Stress fractures in the athletic community are often due to repetitive strain and submaximal loading, with minimal recovery time. An Olympic marathon runner in her 30s presented with right anterolateral knee pain. MRI revealed an acutely displaced vertical fracture of the lateral patella. After failure of non-operative treatment, she underwent surgical excision of the fracture fragment and lateral patellofemoral retinacular repair. Despite return to light training pain-free 3 weeks after surgery
