BACKGROUND: National patient safety investigation bodies conduct system-level investigations and produce recommendations intended to drive improvement. Evidence that such recommendations translate into meaningful improvement is weak and the reasons for this are poorly understood. This paper examines how recommendations produced by the Health Services Safety Investigations Body (HSSIB), England's independent national patient safety investigation body, are produced by investigators and received by the national organisations to whom they are directed.
METHODS: A qualitative multi-method design w…

BACKGROUND: Patients treated and transported by Helicopter Emergency Medical Services (HEMS) are prone to both flight and medical hazards, but incident reporting differs substantially between flight organizations and health care, and the extent of patient safety incidents is still unclear.
METHODS: A qualitative descriptive study based on in-depth interviews with 8 experienced Norwegian HEMS physicians from 4 different bases from February to July 2020 using inductive qualitative content analysis. The study objectives were to explore the physicians' experience with incident reporting and their…

Healthcare-associated infections (HAIs) affect at least 300,000 patients annually in the UK and represent a significant, yet largely preventable, burden to healthcare systems. Hand hygiene by healthcare workers (HCWs) is the leading prevention measure, but compliance with good practice is generally low. The UK National Patient Safety Agency surveyed the public, inpatients, and HCWs, particularly frontline clinical staff and infection control nurses, in five acute care hospitals to determine whether they agreed that a greater level of involvement and engagement with patients would contribute to…

We present a case of allergy to a hospital thermally-printed red plastic allergy alert bracelet in a 48 year old lady admitted to the day surgery unit. Two hours postoperatively, an intensely itchy area of erythema and oedema was seen extending from her left wrist distally to the fingers. The bracelet was removed and the rash resolved overnight without further complication. A diagnosis of contact dermatitis was made, secondary to exposure to an agent within the bracelet. We discuss the safety implications for surgical patients unable to wear an identification bracelet and the steps that may be

OBJECTIVES: to analyze the characteristics of reports of serious incidents and deaths, as well as the improvement actions implemented, considering organizational learning in patient safety management.
METHODS: documentary, descriptive, and quantitative research, conducted in a private electronic system for adverse event reports of an accredited organization. Descriptive and inferential statistics were used for association analysis.
RESULTS: reports were mostly from hospital services (98.8%), from the Southeast region (76.7%), from level 1 accredited institutions (36.7%), made after 30 days o…

BACKGROUND: The study focused on how health information management (HIM) professionals in Australia contribute to patient safety.
OBJECTIVE: The aim was to develop a normative framework that illustrates how the values, beliefs, attitudes and behaviours of HIM professionals influence patient safety across various governance domains.
METHOD: Utilising findings from mixed-method data collection, the research drew on insights from HIM professionals and patient safety experts, using grounded theory strategies for data analysis.
RESULTS: The results are organised within a house-themed normative f…

PURPOSE OF REVIEW: This narrative review aims to summarize and assess recent research in sustainable anesthesia and patient safety.
RECENT FINDINGS: Recent evidence characterizing volatiles as per- and polyfluoroalkyl substances, supports transitioning towards intravenous anesthesia where clinically appropriate. Potential ecotoxicity caused by propofol is minimal, and total intravenous anesthetic techniques are safe and effective. Reusable anesthetic equipment, such as laryngeal mask airways, breathing circuits, and patient warming devices, reduces greenhouse gas emissions and does not impact…

Background: Sterile processing departments (SPDs) are foundational to perioperative safety and play a critical role in preventing surgical-site infections and ensuring procedural reliability. Despite this essential function, SPD professionals often report limited organizational visibility and constrained decision-making authority. Many also experience insufficient integration into formal governance structures. Empirical literature examining the relationship among reporting structures, professional autonomy, and patient safety outcomes in SPDs remains limited. Methods: A convergent mixed-method…

BACKGROUND AND OBJECTIVES: Minimally invasive (MI) pulmonary metastasectomy may improve access in frail patients who are at high-risk for complications after thoracotomy. We compared postoperative complications among frail and non-frail patients undergoing pulmonary metastasectomy.
METHODS: The Nationwide Readmissions Database (2017-2019) identified adult patients who underwent pulmonary metastasectomy. Frailty was determined using Johns Hopkins Adjusted Clinical Groups indicator. Independent propensity-score matches were performed for the open and MI cohorts. Odds ratio (OR) and incidence ra

OBJECTIVES: To identify employee burn-out and assess its impact on patient safety culture.
METHODS: This cross-sectional study was carried out amongst healthcare providers (HCP) of Tehsil Head Quarter Manawan Hospital, Lahore from April 1st till 30th, 2023, who had been working for at least one year and directly involved in patient care. Two questionnaires were used; the Maslach Burnout Inventory (MBI) to assess level of burnout, and Agency for Healthcare Research and Quality (AHRQ) patient safety culture survey. After obtaining informed consent, 59 participants were enrolled in this study.

OBJECTIVE: To evaluate prescribing practices for the anti-Xa reversal agent, andexanet alfa, to identify challenges in ordering and administering this medication, and to offer recommendations to improve patient safety.
PATIENTS AND METHODS: This retrospective study reviewed all adult patients treated with andexanet alfa (AA) at a single institution between January 1, 2018, and March 31, 2020. We identified ordering and administration benchmarks based on recommendations from previous clinical trials on AA. We then reviewed these medical records to determine compliance with these benchmarks. We…

Patient prescriber agreements, also known as opioid contracts or opioid treatment agreements, have been recommended as a strategy for mitigating non-medical opioid use (NMOU). The purpose of our study was to characterize the proportion of patients with PPAs, the rate of non-adherence, and clinical predictors for PPA completion and non-adherence. This retrospective study covered consecutive cancer patients seen at a palliative care clinic at a safety net hospital between 1 September 2015 and 31 December 2019. We included patients 18 years or older with cancer diagnoses who received opioids. We

Training house staff in patient safety and quality improvement (PSQI) requires multidisciplinary collaboration between program directors, graduate medical education, and hospital safety and quality leadership. A heavy clinical workload and limited protected time hinder trainees from engaging in a meaningful PSQI experience during their years of post-graduate training. This is further exacerbated by the lack of subject experts who are available to mentor young physicians. For pulmonary and critical care trainees who are actively involved in the management and care coordination of high-acuity pa…

PURPOSE OF REVIEW: Treatment of inflammatory bowel disease (IBD) in the elderly requires special attention to treatment efficacy while considering drug safety, other medical comorbidities, and the patients' risk for treatment related adverse events. In this article, we reviewed the indications and safety of the newer IBD therapies in the older IBD patient beyond anti-TNF agents, thiopurines, and corticosteroids.
