Vaginal breech delivery and perinatal mortality in twins.

An objective analysis of our clinical experience with the intrapartum management of twin gestations was undertaken in order to determine whether or not routine caesarean section is justified when the first twin presents by the breech. The perinatal mortality rate for breech first twins delivered vaginally was not statistically different from vertex presentations. There was no perinatal loss among babies delivered vaginally by the breech. There was no difference in perinatal outcome for the breech first twin born abdominally or vaginally. Our findings have allayed the fear that non-vertex vagin…

Perinatal mortality in term and post-term births.

Our aim was to compare the fetal mortality rate (FMR), early neonatal mortality rate (ENMR) and perinatal mortality rate (PMR) of post-term and term births, 2) to examine trends in the incidence and perinatal mortality rates of post-term and term births. We used data from Spanish Perinatal Mortality Survey of 1980, 1986, 1989 and 1992. The data include 40,863 post-term births (42 weeks and over) and 517,060 term births (37-41 weeks). Perinatal mortality rates of post-term and term births were compared. The incidence of post-term births was 7.3%. The relative risk (RR) of FMR for post-term comp…

Reducing perinatal mortality in Vila Central Hospital, Vanuatu.

This paper provides accurate figures and detailed analysis of the causes of perinatal mortality in the main referral hospital in Vanuatu in 1992, and discusses possible improvements in management. The perinatal mortality rate for the Vila Central Hospital area was 30.4 per 1000 births (with a stillbirth rate of 14 per 1000). These figures include mothers who presented for the first time in labour without any antenatal care, and cases of very low birthweight (less than 1 kg). These results were achieved with very low intervention rates. The commonest cause of death in Vanuatu was birth asphyxia…

Perinatal mortality in a northern Nigerian rural community.

87 perinatal deaths which occurred out of 1484 births in a rural area of Northern Nigeria over a 9 month period were reviewed. The perinatal mortality rate was 58.6 per 1000 births. Babies born to very young adolescent mothers aged between 10-14 years had the highest perinatal mortality of 375 per 1000 births. Statistically significant relationships were also established between perinatal mortality and parity, educational status of the mother and birth weight. The leading cause of perinatal death was birth trauma which accounted for 48% of all perinatal deaths. Strategies for the reduction of …

HIV-1 infection and perinatal mortality in Zimbabwe.

As part of a survey of the causes of perinatal mortality at Mpilo Maternity Hospital, 220 neonatal deaths and the mothers of 221 stillbirths were tested for HIV-1 antibodies. The HIV positive rate in neonatal deaths was 23.6% (95% confidence interval (CI) 18.0 to 29.2%), significantly higher than 15.4% (95% CI 10.6 to 20.1%) in stillbirths. Perinatal deaths from congenital malformations, birth asphyxia, pregnancy induced hypertension, placental abruption, and oFther non-infectious causes had similar low HIV positive rates averaging 8.1% (95% CI 3.9 to 12.3%). Deaths from septicaemia had a sign…

[Cesarean section in reducing perinatal mortality and morbidity].

The practicability of expanding indications for cesarean section in order to reduce perinatal mortality and possible prevention of perinatal losses by the correct choice of the method of delivery have been assessed through the analysis of the performance of Moscow hospitals. Perinatal mortality and morbidity can be reduced through means other than expanding indications for cesarean section. Further studies are needed to improve the management of labor and standards of neonatal care, particularly so for preterm infants.

Perinatal mortality in twins, Australia, 1973-1980: I.

The perinatal outcome of twin births in Australia is described from 1973 to 1980. Over that period, the perinatal mortality rate declined more slowly in twins than singletons. The stillbirth rate actually increased in twins due to a rise in antepartum deaths. An increasing proportion of monozygotic twins may have contributed to this trend. The overall perinatal mortality rate was 82.85/1,000 for twins and 16.49/1,000 for singletons, giving a relative risk of 5.0.

Cocaine use in pregnancy: perinatal morbidity and mortality.

With the increasing use of cocaine in the United States, there has been growing concern regarding its effects on the fetuses and neonates of pregnant cocaine abusers. Fifty-two cocaine-using women enrolled in a comprehensive perinatal addiction program were evaluated and compared with 73 women who had used narcotics in the past and were maintained on methadone during pregnancy. The groups were similar in maternal age, socioeconomic status, number of pregnancies and cigarette, marijuana and alcohol use. The cocaine-using women had a significantly higher rate of premature labor, precipitous labo

Post-perinatal infant mortality in Glasgow 1979-81.

Post-perinatal infant mortality (PPIM; deaths from the 8th day to the end of the 1st year of life) was studied in Glasgow over the 3-year period 1979-81. The 244 deaths were divided into three main categories--those determined at birth, those due to accidents and acquired disease, and cot deaths. 50% of deaths were determined at birth, and of these 46% were due to prematurity and 49% to congenital disorder. Cot deaths accounted for 44% of the total (88% of deaths not determined at birth) and a definite cause could be identified in only 10% of these. The PPIM rate was 6.1 per 1000 livebirths, a

Twin pairs, smoking in pregnancy and perinatal mortality.

Twins born in the province of Quebec during the first six months of 1972 were ascertained by reviewing all live births and perinatal death certificates. A total of 852 twins, constituting 426 pairs, were included in the study. Smoking habits of the mother during pregnancy were recorded at the hospital of birth for 88.5% of the pairs. The risk of at least one perinatal death was significantly higher for twin pairs born to smoking mothers. The Mantel-Haenszel test statistic controlling for sex of pairs, parity (0,1+), maternal age (< 25,25+ years) and years of schooling was 4.224 (p = 0.04). The

Magnitude and Determinants of Perinatal Mortality in Southwest Ethiopia.

Despite several efforts globally, the problem of perinatal mortality remained an unsolved agenda. As a result, it continued to be an essential part of the third sustainable development goals to end preventable child deaths by 2030. With a rate of 33 per 1000 births, Ethiopia has the highest level of perinatal mortality in the world. Thus, determining the magnitude and identifying the determinants are very crucial for evidence-based interventions. A community-based longitudinal study was conducted in Southwest Ethiopia among 3474 pregnant women to estimate the magnitude of perinatal mortality. …

Impact of sex on perinatal mortality and morbidity in twins.

OBJECTIVE: Twin studies offer opportunities to investigate mechanisms underlying sex-associated differences in perinatal outcomes. The objective of the study was to investigate sex-related differences in perinatal complications. STUDY DESIGN: A cohort of 16,045 twin pregnancies - 32,090 twins - was explored for obstetric complications, perinatal and infant mortality, and neonatal morbidities. RESULTS: Twin pregnancies with a female fetus had an increased risk for preeclampsia, but otherwise there were no pregnancy complications associated with fetal sex. After birth, female-female twins had

Placental abruption and perinatal mortality in the United States.

Placental abruption is an uncommon obstetric complication associated with high perinatal mortality rates. The authors explored the associations of abruption with fetal growth restriction, preterm delivery, and perinatal survival. The study was based on 7,508,655 singleton births delivered in 1995 and 1996 in the United States. Abruption was recorded in 6.5 per 1,000 births. Perinatal mortality was 119 per 1,000 births with abruption compared with 8.2 per 1,000 among all other births. The high mortality with abruption was due, in part, to its strong association with preterm delivery; 55% of the…
