The structure of genetic and environmental risk factors for fears and phobias.

BACKGROUND: Although prior genetic studies of interview-assessed fears and phobias have shown that genetic factors predispose individuals to … his study provides the first view of the architecture of genetic and environmental risk factors for phobic disorders and their subtypes. The genetic factors of the phobias support the DSM-IV and DSM-5 constructs of animal and blood-injection-injury phobias but do not support the separation of agoraphobia from social phobia. The results also do not show a coherent …

Changes in psychopathological symptoms during pregnancy and after delivery: A prospective-longitudinal study in women with and without anxiety and depressive disorders prior to pregnancy.

BACKGROUND: The role of anxiety and depressive disorders prior to pregnancy for changes in peripartum psychopathological symptoms has not been resolved yet. METHODS: A regional-epidemiological sample of 306 women was prospectively followed in seven waves from early pregnancy until 16 months postpartum. Lifetime DSM-IV anxiety and depressive disorders were assessed at baseline with the CIDI-V. Psychopathological symptoms (somatization, obsession-compulsion, interpersonal sensitivity, depression, anxiety, hostility, phobic anxiety, paranoid ideation, and psychoticism) were measured with the BSI

The Middlesex Hospital Questionnaire: a validity study with American psychiatric patients.

The Middlesex Hospital Questionnaire (MHQ) was used as a screening test for psychiatric disorder in 169 new outpatients. The profile obtaine … is of their MHQ total and subscale scores. The MHQ appears to be particularly useful in identifying phobic disorders, and the phobia subscale consistently discriminated between anxiety-phobic states and other diagnostic groups.

The relationship of hypochondriasis to anxiety disorders.

There is evidence from the literature of phenomenological overlap between hypochondriasis and several anxiety disorders, including specific … may exist. In fact, the clearest link would seem to be between the illness phobia subtype and other phobic disorders, although this subgroup has had little study. Thus, hypochondriasis is distinct from the anxiety disorders but, because of phenomenological similarities and extensive comorbidity, consideration should be given to classifying it among them.

The inhibitory effect of Selank on enkephalin-degrading enzymes as a possible mechanism of its anxiolytic activity.

Examination of patients with various forms of anxiety and phobic disorders (according to DSM-4 criteria) demonstrated a considerable shortening of enkephalin half-life and reduced total enkephalinase activity in the blood during generalized anxiety, but not during panic disorders and agoraphobia. This was probably related to low blood concentration of endogenous inhibitors of enkephalin-degrading enzymes in patients with generalized anxiety disorders. Heptapeptide Selank (Thr-Lys-Pro-Arg-Pro-Gly-Pro), which attenuates behavioral anxiety reactions and does not cause side effects typical of most…

Treatment of phobic postural vertigo. A controlled study of cognitive-behavioral therapy and self-controlled desensitization.

In balance clinic practice, phobic postural vertigo is a term used to define a population with dizziness and avoidance behavior often as a consequence of a vestibular disorder. It has been described as the most common form of dizziness in middle aged patients in dizziness units. Anxiety disorders are common among patients with vestibular disorders. Cognitive-behavioral therapy is an effective treatment for anxiety disorders, and vestibular rehabilitation exercises are effective for vestibular disorders. This study compared the effect of additional cognitive-behavioral therapy for a population

Phobic fear of flying in aircrews: epidemiological aspects and comorbidity.

BACKGROUND: Phobic fear of flying may affect aircrew members during any phase of their flying careers. Symptoms are beyond voluntary control and may lead patients to avoid flying and seek medical advice. METHODS: Of 1101 psychiatric files from our institute for 1985-2002, 150 represented cases of fliers who suffered from phobic fear of flying. Data collected from those files included assessment of fear-evoking situations, type of aircraft, class of aircrew duties, aircraft accident history, past medical history, age, and associated psychiatric comorbidity. RESULTS: We compared a group of 56

Conditioning experiences and phobias.

A retrospective study was conducted to examine the extent to which phobias are associated with a conditioning pathway to fear. The Phobic Origin Questionnaire (Ost and Hugdahl, Behav. Res. Ther. 19, 439-477, 1981) was administered to a sample of 91 phobic outpatients (patients with panic disorder with agoraphobia, social phobics, simple phobics). Results show clearly that conditioning experiences occur more frequently than either vicarious or informational, learning experiences, which confirms the findings previously reported by Rimm, Janda, Lancaster, Nahl and Dittmar (Behav. High-dose carbon dioxide challenge test in anxiety disorder patients.

Many investigators have shown that panic disorder patients and possibly social phobics are hypersensitive to the anxiogenic effects of inhaled carbon dioxide (CO2). In this study we administered double-breath inhalation of 35% CO2 and 65% oxygen (O2) to panic disorder patients, social phobics, and normal controls. At baseline, panic disorder patients were characterized by higher pulse, anxiety score, and evidence of hyperventilation. Panic patients and social phobics panicked more often to 35% CO2 than to room air; normal controls did not have a higher rate of panic to CO2 than to room air. Ho

[Phobic syndromes in mentally ill adolescents].

The incidence of phobias in 1731 adolescents with mental disorders was estimated as 15,2%. A clinical follow-up study included 261 patients, aged 15-17 years, with ICD-10 diagnosis of phobic disorders. One third of patients was diagnosed with organic non-psychotic disorder, while one third with a disorder of endogenous cluster, the other nosologic units were represented rarely. The ratio of male to female was 6:1. Frequency allocation in a spectrum of phobic types was as follows: the fear of darkness (23%), social phobia (20,8%), with about a half of the fears being connected with the study pr…

This category is characterized by apprehensive expectation or chronic worry focused on multiple life situations.

Phobic anxiety and clumsiness in a 10-year-old girl.

During assessment of a girl with a long-standing phobic state, it was found that she also had previously unrecognised perceptuo-motor problems, which were associated with continuing problems in balance and spatial awareness. Her phobia improved after a desensitisation programme, but the physical disorder persisted, despite physiotherapy. The authors conclude that the girl's phobic anxiety may have had its origin in her experience of the perceptual disorder during development.

Alcohol dependence and phobic anxiety states. II. A retrospective study.

Twenty-four hospitalised problem drinkers who were currently suffering from a phobic anxiety state served as subjects for this study. Each was given a detailed retrospective interview concerned with the development over time of both their alcohol dependence and phobic problems. The test-retest and inter-rater reliabilities of the data collected were found to be high. In accordance with previous experimental work, periods of heavy drinking and dependence upon alcohol were associated with an exacerbation of agoraphobia and social phobias. Subsequent periods of abstinence were associated with sub

Predictors of symptom resolution in panic disorder after one year of pharmacological treatment: a naturalistic study.

OBJECTIVE: In this naturalistic and prospective study, patients with panic disorder (PD) were treated for one year 1) to verify the rate of patients achieving the resolution of full-symptom attacks, limited-symptom attacks, anticipatory anxiety, phobic avoidance and depression; and 2) to identify the predictors of symptom resolution for each domain. METHOD: One hundred patients with PD, according to DSM-IV criteria, participated in the study. In all patients, a baseline and a follow-up with monthly evaluations of SCL-90, Ham-A, Ham-D and panic diaries were carried out over a one-year period.

[Psychotherapy in intellectual disability. Theoretical background and implementation].

BACKGROUND: Every third person with intellectual disability suffers from additional mental health problems, among others phobic disorders. Yet we do not know whether psychotherapeutic methods that are effective in the normal population are applicable to people with intellectual disabilities. PATIENTS AND METHODS: We give a survey of the development and the present state of the art of psychotherapy, particularly with regard to phobic disorders in intellectual disability. Therapeutic recommendations described in the literature will be evaluated in a case study of one patient. RESULTS: The conf…

Preattentive bias for snake words in snake phobia?

Stroop interference and skin conductance responses (SCRs) for words related to snakes, spiders, flowers, and mushrooms were studied in a group of women (n=40) with snake phobia who were randomised to a stress or no-stress condition. The 21 low-stress snake phobics showed Stroop interference for unmasked (but not for masked) snake words, compared with 21 age- and sex-matched controls. Stroop interference was not significantly different between high-stress and low-stress snake phobics. No support for stronger SCRs for masked snake words was found in snake phobics in a lexical decision task with

Menstrual cycle effects on psychological symptoms in women with PTSD.

The menstrual cycle has been implicated as a sex-specific biological process influencing psychological symptoms across a variety of disorders. Limited research exists regarding the role of the menstrual cycle in psychological symptoms among women with posttraumatic stress disorder (PTSD). The current study examined the severity of a broad range of psychological symptoms in both the early follicular (Days 2-6) and midluteal (6-10 days postlutenizing hormone surge) phases of the menstrual cycle in a sample of trauma-exposed women with and without PTSD (N = 49). In the sample overall, total psych

Investigating phobic specificity with standardized low resolution brain electromagnetic tomography (sLORETA).

The current study investigated differential sources of late event-related potentials (ERPs) in two subtypes of specific phobia using sLORETA (standardized low resolution brain electromagnetic tomography). In two experiments, 14 spider-phobic patients (and controls), and 14 dentophobic patients (and controls) were confronted with disorder-relevant and affectively neutral pictures while an electroencephalogram was recorded. Mean ERP amplitudes were extracted in the time windows of 300-450ms (P300) and 450-800ms (late positive potential, LPP). Analyses revealed that both spider phobics and dental

Medication use and the assessment of agoraphobic avoidance.

Many anxiety disorder patients who present for behaviour therapy are already taking anxiolytic medications. The present study added a new subscale to the Mobility Inventory labelled 'Without Medication' to assess possible reliance on medication for coping with phobic situations. 121 Patients with panic-related disorders were administered the scale. The results supported the reliability and validity of the existing Mobility Inventory subscales in general and of the new subscale in particular. It appears to reliably assess a clinically important domain that is not measured in traditional self-re

Work-Anxiety Rather Than Cognitive Performance Contributes to Workability Decisions in Patients With Mental Disorders.

BACKGROUND: Mental workability decisions must be based on information on person's symptom load, cognitive performance, and workplace conditions. This investigation explores in which way these factors contribute to workability in persons with common mental disorders (CMDs). METHODS: One thousand five hundred seventy CMD-patients underwent multimodal medical diagnostic. Participants filled in questionnaires on work-phobic-anxiety and general mental symptom load. They were also investigated concerning their cognitive performance. Sick leave duration, workplace problems, and subjective workabilit
