Respiratory mechanical effects of surgical pneumoperitoneum in humans.

Pneumoperitoneum for laparoscopic surgery is known to stiffen the chest wall and respiratory system, but its effects on resting pleural pressure in humans are unknown. We hypothesized that pneumoperitoneum would raise abdominal pressure, push the diaphragm into the thorax, raise pleural pressure, and squeeze the lung, which would become stiffer at low volumes as in severe obesity. Nineteen predominantly obese laparoscopic patients without pulmonary disease were studied supine (level), under neuromuscular blockade, before and after insufflation of CO2 to a gas pressure of 20 cmH2O. Esophageal p…

Pneumoperitoneum after esophageal cryoablation in a patient with a PEG.

There is an increasing use of cryotherapy in the gastrointestinal tract for ablation of a variety of lesions. There is only a single reported case of perforation with spray cryotherapy, which was in a patient with Marfan syndrome. In this report, we describe pneumoperitoneum in a patient with a percutaneous endoscopic gastrostomy tube undergoing cryoablation. It is postulated that barotrauma at the percutaneous endoscopic gastrostomy site led to the pneumoperitoneum, and this can occur even with apparently adequate gas-venting procedures.

Local and systemic impact of pneumoperitoneum on prepuberal rats.

Pneumoperitoneum (PN) and the gas used to insufflate the abdominal cavity during laparoscopy seem to be responsible for local and systemic modifications. The aim of this study was to verify the effects of intra-abdominal carbon dioxide (CO(2)) and air insufflation on the peritoneum, as well as the cortico-surrenal response in prepuberal rats. Sixty prepuberal rats were divided into three groups: in the first (S, n = 36), PN was induced with CO(2), whereas in the second (A, n = 14), it was induced with filtered room air; in both conditions, insufflation lasted 30 min at a pressure of 10-12 mmHg…

[Pneumoperitoneum in surgical management of giant scrotal hernias].

Surgical treatment of giant abdominal hernias includes reduction of the hernia content and tension-free closure of the abdominal wall. Initial laparoscopy simulates the postoperative abdominal wall tension. Recognizing the need for a preoperative pneumoperitoneum in cases of chronic eventration may help to avoid "abdominal catastrophes" including bowel resection, abdominal compartment, and extended abdominal wall reconstruction. We report a 66-year-old man with an asymptomatic long-standing giant scrotal hernia who was admitted with sepsis and uremia caused by intestinal obstruction.

Percutaneous drainage of emphysematous cholecystitis associated with pneumoperitoneum.

Emphysematous cholecystitis, a relatively rare variant of acute cholecystitis, is associated with high morbidity and mortality rates. In the presence of a concomitant pneumoperitoneum, these rates may be considered even higher, approaching those of perforation of the gallbladder. The first choice of treatment in cases presenting with pneumoperitoneum is emergency laparotomy. We performed a staged procedure as a second best alternative. In a 65 year-old female patient, initial percutaneous cholecystostomy with a strict intravenous antibiotics regimen, and subsequent cholecystectomy 6 months, la…

[Effect of pneumoperitoneum on plasminogen activator activity in the abdomen].

In a study in pigs it was examined whether peritoneal plasminogen activator activity (PAA) is impaired by creation of a pneumoperitoneum (PP) with CO2 or with helium. The animals were anesthetised and laparotomised, and peritoneal biopsies were taken from the cecum, the ileum and the wound site. Then a PP of 15 mmHg was created with CO2 (Grp. 1, n = 6) or helium (Grp. 2, n = 6). Group 0 (n = 7) was left without gas insufflation. After a PP of 3 hrs and an additional observation period of 2 hrs further tissue samples were collected. In the biopsies specific PAA was determined. In all groups a d…

Sonographic detection of pneumoperitoneum in patients with acute abdomen.

We describe five patients who presented with an acute abdomen in whom pneumoperitoneum was first detected by sonography. All five subsequently were proved to have a perforated viscus. In all cases, the pneumoperitoneum was seen as an echogenic line with a posterior ring-down or reverberation artifact between the anterior abdominal wall and the anterior surface of the liver. The finding was shown best in the right upper quadrant with the patient in the left lateral decubitus position. The echoes caused by the pneumoperitoneum overlapped the echoes of the lung during inspiration, but the echoes …

Incidence and significance of pneumoperitoneum after inguinal herniorrhaphy.

Previous studies have shown pneumoperitoneum either to be extremely rare or to not occur after inguinal herniorrhaphy, and that its presence signifies a serious intra-abdominal complication. A prospective study has been carried out to consider the incidence and significance of pneumoperitoneum after herniorrhaphy for indirect inguinal hernias. In a 1-year period, 100 patients were studied. Pneumoperitoneum was detected in six patients. In five patients, the amount of free gas was minimal and was reabsorbed after 48 h. The pneumoperitoneum was large and of increasing amount in one patient. Faec…

Complications of endoscopic gastrostomy: pneumoperitoneum and volvulus of the colon.

Pneumoperitoneum caused by percutaneous gastrostomy usually follows a benign clinical course, but led to progressive volvulus of the ileocolic segment in the patient described here. This article explains relationships between pneumoperitoneum after surgical or endoscopic procedures and volvulus involving the intraperitoneally mobile intestinal loops.

Fulminant emphysematous hepatitis with pneumoperitoneum: A case report.

Emphysematous hepatitis is a rare and fulminant gas-forming intra-abdominal infection with a high rate of mortality. Concurrent Type II Diabetes Mellitus is the most common association. It is diagnosed through CT examination with replacement of the liver parenchyma with gas without fluid-containing abscess formation. We describe the case of a gentleman in his sixties presenting with acute abdominal pain, fever and jaundice diagnosed with emphysematous hepatitis via CT with a rapidly progressive and terminal course. The imaging demonstrated large volume intrahepatic gas with pneumoperitoneum. T…

Benign Pneumoperitoneum: A Case Report and Literature Review.

Pneumoperitoneum is almost always considered as a warning sign of hollow visceral perforation and requires an urgent surgical review. However, free air within the peritoneal cavity does not always indicate a surgical emergency. In some cases, pneumoperitoneum can occur without true visceral perforation. Recognising this key difference is crucial to avoid unnecessary surgical intervention. A six-year-old girl with GNAO1-related neurodevelopmental disorder, epilepsy, movement disorder, on percutaneous endoscopic gastrostomy (PEG) tube feeds, and an implanted deep-brain stimulator presented with …

Pneumoperitoneum Associated with Pneumatosis Intestinalis in a Pediatric Patient.

Pneumoperitoneum in infancy raises concern for visceral perforation and resultant urgent surgical exploration. However, benign pneumoperitoneum-defined as free intraperitoneal air without gastrointestinal perforation-can occur rarely, especially in infants with chronic ventilatory dependence. We present an unusual case of an 8-month-old infant with severe bronchopulmonary dysplasia (BPD), tracheostomy, and gastrostomy-tube dependence, with radiographic evidence of pneumatosis intestinalis and free intraperitoneal air. Diagnostic laparoscopy was performed which identified air within the mesente…

Endometriosis presenting as catamenial pneumothorax, pneumopericardium and pneumoperitoneum.

A nulliparous woman in her late 30s with a history of pericardial patch repair for atrial septal defect and completed treatment for pulmonary tuberculosis 9 years ago presented with chest pain, breathlessness and abdominal pain. Radiological imaging revealed right-sided pneumothorax, pneumopericardium and pneumoperitoneum for which an intercostal drain (ICD) was placed. A contrast-enhanced CT of the abdomen showed a distal stomach perforation, which was managed conservatively. The ICD was removed after complete lung expansion. She returned twice with similar symptoms coinciding with her perime…

Systemic and Peritoneal Effects of Pneumoperitoneum in Pediatric Laparoscopy.

BACKGROUND: Laparoscopy is a standard procedure for both children and adults. However, the potential effects of CO2 insufflation on the peritoneum remain unclear. This multicenter study aimed to assess the plasma levels of inflammatory markers and examine tissue changes in children who underwent pneumoperitoneum (PN) and laparoscopic surgery. METHODS: From 2021 to 2023, patients who were candidates for elective laparoscopic surgery were divided into two groups based on their duration of PN: ±1 h. Serum samples were collected preoperatively (T0) and 24 h postoperatively (T1). Tumor necrosis fa…

Complicated Meckel's Diverticulum Presenting as Pneumoperitoneum in an Adolescent.

BACKGROUND The finding of pneumoperitoneum frequently leads to operative management for diagnosis and treatment. The etiology of pneumoperitoneum includes perforated viscus, such as perforated peptic ulcers, small or large intestinal perforations, appendicitis, and complicated sigmoid diverticulitis. We describe the preoperative, intraoperative, and postoperative course of a patient with perforated Meckel’s diverticulitis presenting with pneumoperitoneum. This unusual presenting finding highlights that Meckel’s diverticulum should be included in the differential diagnosis in adoles…

COVID-19: A Possible Cause of Spontaneous Pneumoperitoneum.

INTRODUCTION: Pneumoperitoneum is the presence of air within the peritoneal cavity and is mostly caused by organ rupture. Spontaneous pneumoperitoneum accounts 5% to 15% of the cases and occurs in the absence of organ damage. The pulmonary origin of pneumoperitoneum is unusual, and probably associated with mechanical ventilation and alveolar leak. In patients with coronavirus disease 2019 (COVID-19) there are some reports of air leak, like pneumothorax, pneumomediastinum, pneumoperitoneum, and subcutaneous emphysema. CASE PRESENTATION: We present the case of a 70-year-old man with COVID-19 pn…

Pneumoperitoneum from a congenital defect of the gastric musculature.

A neonate at 31 weeks of gestation presented with aggravated abdominal distention on the 2th day after birth. The birth weight was 2110 g. Radiography of the abdomen showed pneumoperitoneum. The neonate was complicated with respiratory distress, electrolyte disturbance and coagulation disorders. Laparoscopic exploration revealed an approximately 6 cm length rupture of the greater curvature, extending from cardia to proximal pylorus. After trimming necrosis tissue of the edge, the gastric wall perforation was repaired surgically. Thinning and disappearance of muscularis were showed at edge of d…
