Noninfectious endocarditis as a novel cardiac manifestation of glycogen storage disease type IV: a case report.

BACKGROUND: Glycogen storage disease type IV (GSD IV), Andersen disease, is a rare autosomal recessive disorder of glycogen metabolism typically associated with liver failure and cardiomyopathy. Isolated non-infectious cardiac valvular disease as a sequela of GSD IV has not been previously reported in this population. We aim to describe the first reported case of non-infectious endocarditis as a novel cardiac manifestation in a patient with GSD IV. CASE DESCRIPTION: We report the case of a 16-year-old male with GSD IV, status post liver transplant in infancy, who presented with fever, fatigue

Post-streptococcal opsoclonus-myoclonus syndrome associated with anti-neuroleukin antibodies.

BACKGROUND: Adult opsoclonus-myoclonus (OM), a disorder of eye movements accompanied by myoclonus affecting the trunk, limbs, or head, is commonly associated with an underlying malignancy or precipitated by viral infection. METHODS: We present the first two reports of post-streptococcal OM associated with antibodies against a 56 kDa protein. Two young girls presented with opsoclonus and myoclonus following a febrile illness and pharyngitis. Protein purification techniques were employed. Amino acid sequences of human neuroleukin (NLK) and streptococcal proteins were compared using the protein-

Susceptibility and characteristics of infections in patients with glucocorticoid excess or insufficiency: the ICARO tool.

OBJECTIVE: Registry data show that Cushing's syndrome (CS) and adrenal insufficiency (AI) increase mortality rates associated with infectious diseases. Little information is available on susceptibility to milder forms of infections, especially those not requiring hospitalization. This study aimed to investigate infectious diseases in patients with glucocorticoid disorders through the development of a specific tool. METHODS: We developed and administered the InfeCtions in pAtients with endocRinOpathies (ICARO) questionnaire, addressing infectious events over a 12-month observation period, to 1

Chronic scrotal pain syndrome: management among urologists in Switzerland.

INTRODUCTION AND OBJECTIVES: Management of patients presenting with chronic or recurrent pain located in the scrotum is often very challenging. Evidence-based literature and clinical practice guidelines for the management of chronic scrotal pain syndrome (CSPS) are not available. We assessed the current perception and management of chronic scrotal pain syndrome by urologists in Switzerland. METHODS: In July 2004, all the members of the Swiss Society of Urology received a questionnaire focusing on diagnostic and treatment practices for the management of chronic scrotal pain syndrome. The quest

Antibody-mediated encephalitides: distinct diseases with transdisciplinary implications.

The discovery of antibody-mediated glial and neuronal disorders has transformed the diagnosis and treatment of neurological diseases previously considered idiopathic or not recognised as immune-mediated. Antibody-mediated disorders targeting glial aquaporin-4 and myelin oligodendrocyte glycoprotein are distinct nosological entities, defined by specific targets, mechanisms, syndromes, and therapeutic implications. By contrast, antibody-mediated disorders targeting neuronal antigens (ie, autoimmune encephalitides) are still frequently analysed collectively, with data from different disorders poo

Immunotherapies for postural orthostatic tachycardia syndrome, other common autonomic disorders, and Long COVID: current state and future direction.

Postural orthostatic tachycardia syndrome (POTS), neurocardiogenic syncope, and orthostatic hypotension are the most common autonomic disorders encountered in clinical practice. The autoimmune etiology and association of these conditions with systemic autoimmune and inflammatory disorders, autonomic neuropathy, and post-acute infectious syndromes, including Long COVID, suggest that immunotherapies should be considered as a therapeutic option, at least in a subset of patients. However, the treatment of common autonomic disorders has traditionally included pharmacologic and non-pharmacologic sym

Multiple Evanescent White Dot Syndrome as Epiphenomenon of Infectious Chorioretinopathies.

PURPOSE: Multiple evanescent white dot syndrome (MEWDS) is a rare idiopathic ocular disorder often affecting young adults. Recently, a secondary form, epiphenomenon MEWDS (Epi-MEWDS), has been described in association with pre-existing retinal conditions. However, its correlation with infectious diseases remains poorly documented. METHODS: A retrospective review of six patients with infectious chorioretinopathies who developed Epi-MEWDS was conducted. Clinical presentations, imaging findings, and disease progression were analyzed. RESULTS: This study presents a case series of six patients wh

Locomotor Health in Modern Broiler Chickens: Risk Factors, Major Disorders, Assessment Methods, and Implications for Welfare and Production Efficiency.

Genetic selection for rapid growth, improved feed efficiency, and increased breast-muscle yield has substantially enhanced broiler production; however, it has also increased the functional demands placed on the locomotor system. This review critically examines locomotor health in modern broiler chickens in relation to welfare and production efficiency. Particular attention is given to the multifactorial interactions among genotype, growth rate, body weight and conformation, skeletal development, nutrition, physical activity, housing conditions, age, and infectious factors. Major locomotor diso

Long Vax in the Eye: Long Post-COVID Vaccination Syndrome Presenting with Frosted Branch Angiitis.

mRNA COVID-19 vaccines have been reported as protecting against COVID-19 and reducing its severity, and we have recognized post-vaccination symptoms recently. This research investigates the clinical trajectories of ocular disorders in a 51-year-old female who received a second dose of the BNT162b2 (Pfizer-BioNTech) mRNA COVID-19 vaccine. Exhibiting fever and blurred vision within 24 h post-vaccination, with progressive blurry vision over two months, she underwent in-depth ophthalmologic examinations, revealing intraocular cellular infiltration in anterior chamber, vitreous opacity, and frosted

Impact of a total lockdown for pandemic SARS-CoV-2 (Covid-19) on deep surgical site infections and other complications after orthopedic surgery: a retrospective analysis.

BACKGROUND: A total lockdown for pandemic SARS-CoV-2 (Covid-19) entailed a restriction of elective orthopedic surgeries in Switzerland.  While access to the hospital and human contacts were limited, hygiene measures were intensified. The objective was to investigate the impact of those strict public health guidelines on the rate of intra-hospital, deep surgical site infections (SSI), wound healing disorders and non-infectious postoperative complications after orthopedic surgery during the first Covid-19 lockdown. METHODS: In a single-center study, patients with orthopedic surgery during the f

Encephalitic Arboviruses of Africa: Emergence, Clinical Presentation and Neuropathogenesis.

Many mosquito-borne viruses (arboviruses) are endemic in Africa, contributing to systemic and neurological infections in various geographical locations on the continent. While most arboviral infections do not lead to neuroinvasive diseases of the central nervous system, neurologic diseases caused by arboviruses include flaccid paralysis, meningitis, encephalitis, myelitis, encephalomyelitis, neuritis, and post-infectious autoimmune or memory disorders. Here we review endemic members of the Flaviviridae and Togaviridae families that cause neurologic infections, their neuropathogenesis and host

Posttransplantation lymphoproliferative disorder in a renal allograft recipient.

As a complication of immunosuppressive therapy, solid organ and bone marrow transplant recipients have an increased risk of developing post transplant lymphoproliferative disorder (PTLD). Epstein-Barr virus (EBV) infection and the degree of immunosuppression have been identified as risk factors for the development of this complication. The clinical presentation includes a spectrum of disorders ranging from a benign infectious mononucleosis-like syndrome with fever and cervical adenopathy to multiorgan failure leading to death. Although, T-cell lesions have been described, PTLD is usually assoc

Adverse effect of lockdowns during the COVID-19 pandemic: increased incidence of pediatric crisis admissions due to eating disorders and adolescent intoxications.

UNLABELLED: During the COVID-19 pandemic, countries imposed (partial) lockdowns that reduced viral transmission. However, these interventions may have unfavorable effects on emotional and psychological well-being. The aim of this study was to quantify possible adverse effects of the COVID-19 pandemic on psychological wellbeing in children and adolescents. Hospital admission data between January 2017 and September 2021 from eight general hospitals in the Netherlands was collected, comparing the incidences of sub-categorized psychological diagnoses, more specifically eating disorders, intentiona

Acute Ataxia in Children: A Review of the Differential Diagnosis and Evaluation in the Emergency Department.

Acute ataxia in a pediatric patient poses a diagnostic dilemma for any physician. While the most common etiologies are benign, occasional individuals require urgent intervention. Children with stroke, toxic ingestion, infection, and neuro-inflammatory disorders frequently exhibit ataxia as an essential-if not the only-presenting feature. The available retrospective research utilize inconsistent definitions of acute ataxia, precluding the ability to pool data from these studies. No prospective data exist that report on patients presenting to the emergency department with ataxia. This review exa

Clinical evaluation of pediatric olfactory disorders: a review from etiology to management.

Olfactory perception plays a fundamental role in nutrition, emotional development, and social behavior, yet olfactory disorders (OD) in children remain largely underrecognized and understudied. This mini review summarizes current evidence and proposes a structured clinical approach for the evaluation and management of pediatric OD. Etiologies are diverse, encompassing congenital syndromes such as Kallmann and CHARGE, post-infectious and post-traumatic forms, inflammatory airway diseases, and structural or iatrogenic causes. Accurate diagnosis begins with a detailed medical history and comprehe

Epstein-Barr Virus and Cytomegalovirus Infections of the Liver.

Epstein-Barr virus (EBV) and cytomegalovirus (CMV) infections are common and are associated with a variety of liver manifestations. EBV and CMV infections, in immunocompetent hosts, commonly manifest as acute hepatitis, with severity varying from asymptomatic, self-limited icteric hepatitis to acute liver failure. Atypical manifestations, such as cholestasis, chronic hepatitis, precipitation of acute-on-chronic liver failure, and autoimmune hepatitis, are reported with EBV infection, whereas cholestasis, portal vein thrombosis, and Budd-Chiari syndrome are reported with CMV infection. Acute Sensory-Motor Axonal Neuropathy in a 57-Year-Old Male Presenting With Paresthesia and Distal Muscle Weakness.

Guillain-Barré syndrome (GBS) is a relatively uncommon post-infectious, immune-mediated neurologic disorder with an incidence of 0.5-2/100,000. It is usually preceded by an infection that evokes an immune response that cross-reacts with peripheral nerve components via molecular mimicry. The presentation of this disorder has several forms, including acute inflammatory demyelinating polyneuropathy (AIDP), acute motor axonal neuropathy (AMAN), acute motor-sensory axonal neuropathy (AMSAN), and Miller Fisher syndrome (MFS). The case we describe is of a 57-year-old male presenting with sensory feat
