Androgen in postmenopausal women.

Menopausal symptoms, bone loss, changes in lipid profiles and reduction of insulin sensitivity due to an abrupt decrease in circulating estrogen level are well known in women during the menopausal transition. On the other hand, the effect of androgen on women's health has not been fully elucidated. Circulating levels of testosterone and dehydroepiandrosterone sulfate (DHEA-S) gradually decrease with age in postmenopausal women, although transient increases have been observed during the menopausal transition. High testosterone level has been suggested to be associated with increased risk of car

Bisphosphonates for postmenopausal osteoporosis.

Bisphosphonates are effective in reducing bone turnover, increasing BMD and reducing fracture risk in postmenopausal women with osteoporosis. The licensed bisphosphonates exhibit some differences in potency and speed of onset and offset of action. These differences mean that different agents may be more advantageous in different situations. Uncertainties still exist around the optimum duration of treatment and treatment holidays, how best to use bisphosphonates with anabolic treatments, and the benefits of treatment in patients who do not have a BMD T-score below -2.5.

Chemoprevention in postmenopausal women.

Several large, prospective trials have evaluated tamoxifen compared with placebo for breast cancer risk reduction in women at increased risk of breast cancer. Combined results from these trials demonstrate that tamoxifen at 20 mg/d reduced the incidence of breast cancer by 38%. The risk of developing breast cancer is the primary determinant of net benefit, with greater benefits accrued to women with the highest risk of breast cancer. Age and the presence of risk factors for increased toxicity also have an effect on the net benefit associated with tamoxifen. The greatest clinical benefit from t

The woman with postmenopausal bleeding.

BACKGROUND: Postmenopausal bleeding is a common complaint from women seen in general practice. OBJECTIVE: This article outlines a general approach to such patients and discusses the diagnostic possibilities and their management. DISCUSSION: The most common cause of postmenopausal bleeding is atrophic vaginitis or endometritis. However, as 10% of women with postmenopausal bleeding will be found to have endometrial cancer, all patients must be properly assessed to rule out the diagnosis of malignancy. Most women with endometrial cancer will be diagnosed with early stage disease when the progno

Treatment of postmenopausal osteoporosis.

Osteoporosis is a skeletal disease characterized by low bone mass and microarchitectural deterioration with a resulting increase in bone fragility and hence susceptibility to fracture. Calcium and vitamin D are the most commonly used therapies for osteoporosis, although their efficacy in osteoporotic fracture prevention remains uncertain. Biphosphonates are the most frequently prescribed medication for treatment of osteoporosis and are often considered as first-line therapy for the treatment of osteoporosis. Currently, hormone replacement therapy is only approved by the Food and Drug Administr

Postmenopausal tuberculosis endometritis.

Tuberculosis remains a global health problem, primarily in developing countries with inadequate health services. A significant portion of tuberculosis in these settings is extrapulmonary, including tuberculosis of the genitourinary tract. Patients with genital tuberculosis are usually young women detected during work up for infertility. After menopause, tuberculosis of the endometrium is a rare possibility probably because of the decreased vascularity of the tissues. We present a case of endometrial tuberculosis with postmenopausal vaginal bleeding.

[Pathophysiology of postmenopausal osteoporosis].

Osteoporosis is a huge public health problem because it is directly related with fractures. A number of genetic and environmental factors are interweaving in the complex pathophysiology. These factors determine peak bone mass and the degree (rapidity) of bone loss later on. Understanding of the molecular mechanism of bone formation and remodeling, as well as understanding of factors responsible for bone quality (strength) have been significantly improved over the last few years. Thorough knowledge of postmenopausal osteoporosis pathophysiology is the prerequisite for effective prevention and t

Treatment of postmenopausal osteoporosis.

Osteoporosis is a major public health burden. The most devastating outcome of osteoporosis is fracture, which results in increased morbidity and mortality. These fractures most often occur in the vertebrae and indicate an increased risk of future vertebral and hip fractures. Consequently, it is important to identify patients at risk for fracture and to intervene with pharmacologic therapies, lifestyle changes, or both to reduce the frequency of the first or subsequent fracture. Moreover, because osteoporosis is a chronic condition requiring long-term therapy, factors that increase compliance a

[Postmenopausal hormone therapy].

The menopausal transition is a natural phase of every womańs life. Several symptoms emerging during the climacteric may be treated with estrogens. Hot flushes and symptoms of genital ageing may be relieved by estrogen therapy. Progestin therapy is indicated in all women with an intact uterus to avoid endometrial pathology. Given the present risk-benefit scenario, estrogen therapy is no longer a first-line choice to prevent osteoporosis. Counselling requires integration of recent knowledge provided by the first results of the Womeńs Health Initiative and other significant randomized, placebo-co

Postmenopausal osteoporosis and alendronate.

Osteoporosis is a systemic metabolic disorder associated with a decreased bone mass and resistance. Bisphosphonates suppress bone resorption and bone turnover by a mechanism that depends on their structure. They are characterized by low gastrointestinal absorption. In postmenopausal women, alendronate (ALN) reduces bone resorption markers and increases bone mineral density (BMD) in the lumbar spine, femoral neck, and total body. Individuals receiving ALN have been studied for up to 10 years with an apparent linear increase in BMD over that time period estimated at 13.7% at the lumbar spine. Tr

Breastfeeding and postmenopausal osteoporosis.

Bone loss associated with osteoporosis occurs with high frequency among the elderly and often results in debilitating fractures. A combination of lifestyle behaviors, genetic predisposition, and disease processes contributes to bone metabolism. Therefore, any discussion regarding bone health must address these factors. The impact of menopause on bone turnover has been generally well studied and characterized. Breastfeeding places significant stress on calcium metabolism and, as a consequence, directly influences bone metabolism. The most significant factors affecting bone mineral density (BMD)

[Therapy of postmenopausal osteoporosis].

The therapy of postmenopausal osteoporosis is based on a few comprehensible assumptions. High bone resorption should be reduced by treatment with bisphosphonates, raloxifene or seldom with calcitonins. After reduction of high bone turnover and in low bone turnover situations, an osteoinductive combination therapy should be started, inducing collagen type I with parathyroid hormone or fluorides. This collagen can then be mineralized by calcium, vitamin D, and vitamin D metabolites. In addition, bone resorption should be reduced during menopause with estrogens and gestagens, in the case of a rec

Management of postmenopausal osteoporosis.

Postmenopausal osteoporosis is associated with significant morbidity, mortality, reduction in quality of life, and increasing health care costs. It is estimated that 1.5 million women in the United States have one or more osteoporosis-related fractures annually. Fractures may occur at any site, but vertebral fractures are the most common. Longitudinal studies have demonstrated a decreased life expectancy associated with both vertebral and nonvertebral fractures. Once an initial fracture occurs, there is a fivefold increased risk of a second fracture within 1 year. The management of osteoporosi

[Postmenopausal osteoporosis in women].

Postmenopausal osteoporosis is now considered a major public health problem in aging women, due to the burden related to the consequent fractures. Over recent years, several pharmacological approaches were developed for the prevention and treatment of osteoporosis. Besides regular physical exercise and calcium rich diet, calcium supplementation can be suggested to both genders, after seventy years as well as systematic vitamin D supplementation in order to cope with the frequent lack observed in our country. Hormone replacement therapy is the first choice in prevention of postmenopausal osteop

Hemoperitoneum in a postmenopausal woman.

As the number of postmenopausal women increases, physicians will have more opportunities to treat elderly women with gynecological complications. This case report describes a 76-year-old, obese, multiparous woman, known to have gallstones who presented with acute abdomen complaints and was admitted for observation. Treatment was delayed until significant blood loss was recognized. At laparotomy, a ruptured ovarian granulosa cell tumor was found.
