Review article: Safety profile of propofol for paediatric procedural sedation in the emergency department.

The use of propofol for paediatric procedural sedation (PPS) is increasing because of its favourable pharmacodynamic profile. Its paediatric use has been limited in some ED (emergency departments) because of the perceived high rate of serious adverse events in comparison with other sedation agents. A systematic literature review of propofol use for PPS outside the operating theatre environment from 1966 to 2008 was carried out to profile the adverse event rate in comparison with other procedural sedation agents. Sixty studies with a total of 17,066 paediatric propofol sedations in a variety of…

Ritonavir May Prolong Sedation but is Unlikely to Increase the Risk of Respiratory Arrest in Patients Requiring Intravenous Midazolam for Procedural Sedation.

Intravenous midazolam is frequently used for procedural sedation. Use of ritonavir containing antivirals in patients requiring procedural sedation with intravenous midazolam is postulated to increase the risk or prolong the consequences of exposure related adverse events. The primary objective of this study was to characterize interaction of ritonavir with IV midazolam. The secondary objective was to define the time course over with the interaction of ritonavir with IV midazolam resolves following cessation of ritonavir. Physiologically based pharmacokinetic modeling was used to conduct clinic…

Sedation of children for technical procedures: current standard of practice.

We sought to define the current standard of care for children undergoing sedation for painless diagnostic procedures by sending questionnaires to 284 pediatric residency program directors in North America. From the 89 responses, we determined that departments of pediatrics set sedation policies for children in most institutions, often with formal written guidelines for these procedures. Most require that children have some form of cardiorespiratory monitoring while under sedation and that they are attended by individuals trained in cardiorespiratory resuscitation until the child is fully recov

A Questionnaire Study on Patient Perspectives of Sedation for Ophthalmic Procedures.

Introduction Ophthalmic procedures, such as cataract and refractive surgery, are prevalent in the United States and are often associated with significant preoperative anxiety among patients, which can impact patient comfort and surgical outcomes. Sedation is used in the vast majority of these procedures to reduce anxiety and improve patient cooperation. However, there is limited information on patient-reported experiences and perspectives. This study thus aimed to explore the opinions of patients on sedation for their eye procedures. Methods We administered an anonymous Qualtrics survey to gat

Chloral hydrate as a sedating agent for neurodiagnostic procedures in children.

BACKGROUND: This is an updated version of a Cochrane Review published in 2017. Paediatric neurodiagnostic investigations, including brain neuroimaging and electroencephalography (EEG), play an important role in the assessment of neurodevelopmental disorders. The use of an appropriate sedative agent is important to ensure the successful completion of the neurodiagnostic procedures, particularly in children, who are usually unable to remain still throughout the procedure. OBJECTIVES: To assess the effectiveness and adverse effects of chloral hydrate as a sedative agent for non-invasive neurodia

Effects of Cannabis Use on Sedation Requirements for Endoscopic Procedures.

CONTEXT: Cannabis (or marijuana) became legal for recreational use in Colorado in 2012, and this legislation change has created both challenges and opportunities in medicine. More patients are using cannabis, and more patients are now willing to admit cannabis use than in the past, which increases the likelihood that they will be forthcoming about use during medical questioning. Cannabis use may have implications during medical care, including procedural sedation. OBJECTIVE: To determine whether regular cannabis use had any effect on the dose of medication needed for sedation during endoscopi…

Familial dysautonomia: deep sedation and management in endoscopic procedures.

OBJECTIVES: Familial dysautonomia is a rare genetic disorder that affects the development of the central nervous system, causing GI dysfunction. Because of an improved prognosis, elective surgical procedures are more common and present a unique problem to the anesthesiologist. All patients reported in the literature underwent these interventions under general anesthesia in the operating theater. We report our preliminary experience with deep sedation in the endoscopy room in patients with this rare syndrome. METHODS: Four girls (7-16 yr old) underwent percutaneous endoscopic gastrostomy inser

Is sedation necessary for hydrostatic reduction in ileocolic intussusception? Evaluating procedural outcomes and parents' preferences.

INTRODUCTION: The necessity for procedural sedation during hydrostatic reduction for ileocolic intussusception in children is being debated. This study aimed to compare outcomes of procedures with and without sedation, as well as parental preferences. METHODS: This retrospective study (2019-2021) reviewed the medical records of children treated for ileocolic intussusception at two hospitals-one that did not provide sedation (non-sedation, NS) and one that did (with-sedation, WS) during hydrostatic reduction. The primary outcomes encompassed success rate, perforation, and recurrence. Parental …

Oral Ketamine and Midazolam for Procedural Sedation in the Pediatric Emergency Department: A Retrospective Study.

INTRODUCTION: Needle-free procedural sedation (PS) is an attractive option for children presenting to the emergency department (ED) who require a painful procedure, as it avoids inflicting additional pain either with intravenous line placement or intramuscular injection. While use of oral (PO) ketamine has been reported in the literature, limited information is available to guide ED-based use in children. METHODS: This is a descriptive study of the patient experience receiving PS with the following regimen: PO ketamine 6 mg/kg (max of 200 mg) with or without PO midazolam 0.5 mg/kg (max 15 mg)…

An exploratory study of distractions during the induction phase of pediatric procedural sedation with propofol.

INTRODUCTION: Distractions are a leading cause of disturbance to workflow during medical care. Distractions affecting the anesthetic team in the operating room are frequent and have a negative impact on patient care one-fifth of the time. The objective of this study was to evaluate the frequency, source, target, and impact of distractions during the induction phase of pediatric procedural sedation outside the operating room. METHODS: Distractions were analyzed during propofol induction for oncology procedures from 45 video recordings. Distraction was defined as any event that disturbs or has …

General anesthesia vs procedural sedation for failed NeuroThrombectomy undergoing rescue stenting: intention to treat analysis.

BACKGROUND: There is little data available to guide optimal anesthesia management during rescue intracranial angioplasty and stenting (ICAS) for failed mechanical thrombectomy (MT). We sought to compare the procedural safety and functional outcomes of patients undergoing rescue ICAS for failed MT under general anesthesia (GA) vs non-general anesthesia (non-GA). METHODS: We searched the data from the Stenting and Angioplasty In Neuro Thrombectomy (SAINT) study. In our review we included patients if they had anterior circulation large vessel occlusion strokes due to intracranial internal caroti

Comparative safety profile of chloral hydrate versus other sedatives for procedural sedation in hospitalized infants.

BACKGROUND: Given the limited available evidence on chloral hydrate safety in neonatal populations and the discrepancy in chloral hydrate acceptance between the US and other countries, we sought to clarify the safety profile of chloral hydrate compared to other sedatives in hospitalized infants. METHODS: We included all infants <120 days of life who underwent a minor procedure and were administered chloral hydrate, clonidine, clonazepam, dexmedetomidine, diazepam, ketamine, lorazepam, midazolam, propofol, or pentobarbital on the day of the procedure. We characterized the distribution of infan

Procedural sedation in dentistry: a scoping review and proposal for an ad hoc informed consent.

BACKGROUND: The healthcare aspects of dentistry are framed within a legal environment. Dentists are obliged to obtain their patients’ written informed consent (IC) before undertaking any procedures. A failure to fulfill IC obligations or to fulfill such obligations correctly can have legal repercussions related to professional liability. Procedural sedation (PS) in dental patients requires an independent health treatment. This is usually successfully provided by authorized sedative drugs that induce anxiolysis and sedation, allowing the patient to tolerate uncomfortable procedures while mainta…

Nonoperating Room Anesthesia and Procedural Sedation in Interventional Radiology: A Collaborative Framework for Safe Growth.

Interventional radiology (IR) has become one of the fastest expanding areas of procedural care, with increasing demand for image-guided diagnostic, vascular, oncologic, and emergency interventions. This growth has created parallel demands for both procedural sedation (PS) and anesthesiology services, the so-called nonoperating room anesthesia (NORA) within the IR suite. These services should not be viewed as competing models, but as different points on a continuum in which the level of sedation or anesthesia is matched to the patient, procedure, location, level of care, clinicians' qualificati…
