Does Clockwise Rotation of Maxillomandibular Complex Using Surgery-First Approach to Correct Mandibular Prognathism Improve Facial Appearance?

BACKGROUND: Facial aesthetics may be optimized based on a deeper understanding of soft tissue changes after orthognathic surgery. PURPOSE: The purpose of the study was to delineate facial soft tissue changes after clockwise rotation (CWR) of the maxillomandibular complex (MMC) to correct mandibular prognathism using the surgery-first approach. STUDY DESIGN, SETTING, SAMPLE: This prospective cohort study enrolled patients over 18 years of age with skeletal Class III malocclusion in the craniofacial center. The patients were excluded with previous history of craniofacial syndrome, orthognathic…

The Influential Bony Factors and Vectors for Predicting Soft Tissue Responses After Orthognathic Surgery in Mandibular Prognathism.

PURPOSE: We sought to identify the hard tissue points and vectors that have the greatest effect on soft tissue movement after orthognathic surgery in patients with mandibular prognathism. PATIENTS AND METHODS: The present retrospective study involved patients who had undergone mandibular setback surgery with or without maxillary advancement. Multiple linear regression models were adapted to evaluate the association between the 8 hard tissue landmark (predictor variables) changes and 11 soft tissue responses (outcome variables) using the x and y coordinates assessed from superimposed pre- and …

Reduction genioplasty enhances quality of life in female patients with prognathism and maxillary hypoplasia undergoing bimaxillary osteotomy.

The structure of the chin determines facial attractiveness and is directly linked to quality of life (QoL). In patients with prognathism and maxillary hypoplasia, bimaxillary osteotomy (BIMAX) with mandibular setback does not always lead to a more slender chin or improved aesthetics. The aim of the present study was to evaluate whether QoL differed between females undergoing BIMAX alone (group I; n=30) and those undergoing BIMAX combined with reduction genioplasty (group II; n=30). Presurgical and postsurgical evaluations included cephalography, photogrammetry, and the Oral Health Impact Profi…

Periodontal status of mandibular incisors following excessive proclination. A study in adults with surgically treated mandibular prognathism.

The present study was undertaken to examine whether excessive proclination of mandibular incisors results in gingival retraction. In patients with surgically treated mandibular prognathism, 29 with more than 10 degrees proclination of mandibular incisors and 33 with minimal change in incisor inclination during presurgical orthodontic phase were selected. A total of 21 and 19 patients, respectively, could meet for a clinical follow-up examination including Visible Plaque Index, Gingival Bleeding Index, probing pocket depth, and length of supracrestal connective tissue attachment. Study models a…

Absence of mutations in NR2E1 and SNX3 in five patients with MMEP (microcephaly, microphthalmia, ectrodactyly, and prognathism) and related phenotypes.

BACKGROUND: A disruption of sorting nexin 3 (SNX3) on 6q21 was previously reported in a patient with MMEP (microcephaly, microphthalmia, ectrodactyly, and prognathism) and t(6;13)(q21;q12) but no SNX3 mutations were identified in another sporadic case of MMEP, suggesting involvement of another gene. In this work, SNX3 was sequenced in three patients not previously studied for this gene. In addition, we test the hypothesis that mutations in the neighbouring gene NR2E1 may underlie MMEP and related phenotypes. METHODS: Mutation screening was performed in five patients: the t(6;13)(q21;q12) MMEP…

Single nucleotide polymorphisms MYO1H 1001 C>T SNP (rs3825393) is a strong risk factor for mandibular prognathism.

INTRODUCTION: Mandibular prognathism (MP) is a common craniofacial disorder of Class III malocclusion that causes esthetic and functional problems. Class III malocclusion diversity is influenced by both environmental and genetic factors. Single nucleotide polymorphisms (SNPs) in genes involved in craniofacial morphogenesis, bone and cartilage development, and metabolism, could play a role as predisposing factors. The present study aimed to establish a potential association between MATN1 -1878 A>G (rs1149048), MYO1H 1001 C>T (rs3825393), and BMP-4 538 A>G (rs17563) SNPs and MP in Serbian popula…

Swallowing Functions after Sagittal Split Ramus Osteotomy with Loose Fixation for Mandibular Prognathism: A Retrospective Case Series Research.

Sagittal split ramus osteotomy (SSRO) is a standard surgical technique for patients with mandibular prognathism. However, the appropriate position of the proximal fragment is not strictly defined, and rigid fixation can induce early postoperative skeletal relapse and temporomandibular (TMJ) disorders. Loose fixation can be expected to seat the proximal bone fragments in a physiologically appropriate position, thereby reducing adverse events. Although long-term skeletal stability has been achieved using SSRO without fixation, the evaluation of preoperative and postoperative eating and swallowin…
