[Mitral valve atresia with normal aortic valve and pulmonary atresia. A case with pulmonary collateral vascular perfusion by systemic pulmonary arteries].

A case of mitral atresia with normal aortic valve, situs solitus, d-loop, ASD, VSD, DORV and atresia of the pulmonary valve is presented. Systemic-pulmonary collateral arteries were the single source of collateral lung perfusion. These vessels are often found in patients with tetralogy of Fallot and pulmonary atresia with VSD. The case favours the idea that these vessels are not specific for the above mentioned cardiac anomalies. It supports the theory that they represent true embryonal vessels which can persist in cases of an inadequate or completely missing connection of the sixth aortic arc…

Autologous monocusp pulmonary valve: preliminary results.

BACKGROUND: There is growing recognition that postoperative pulmonary regurgitation may result in early or late progressive right heart failure. METHOD: A technique for fashioning an autologous monocusp pulmonary valve from the wall of the pulmonary artery was developed. The monocusp valve was fashioned from the anterior wall of the main pulmonary artery, and the remaining defect was filled with autologous pericardium. The procedure was performed in 8 dogs and 5 children. RESULTS: Early follow-up and serial echocardiographic assessment in both dogs and children proved the functionality of th…

[Pulmonary hypertension in aortic valve disease].

144 surgical patients with isolated aortic valve disease were reviewed to determine the incidence and the mechanism of pulmonary hypertension (PH). The hemodynamic data revealed PH (pulmonary artery systolic pressure of 40 mmHg or more) in 18 patients (12.6%). There was a positive correlation between left ventricular end-diastolic pressure (LVEDP) and pulmonary artery pressure. After operation the pulmonary artery pressure normalized in all cases but 2 patients in early post-operative period. In patients with aortic stenosis with PH, LVEDP/LVEDVI ratio was higher than that of patients without

Congenital absence of pulmonary valve leaflets.

Congenital absence of pulmonary valve leaflets is an uncommon condition usually associated with ventricular septal defect and an obstructive pulmonary valve ring. Twenty-one patients with these malformations are described. Twenty had an associated ventricular septal defect with ventriculoarterial concordance, and one also had transposition of the great arteries, ventricular septal defect, and obstructive pulmonary valve ring. The clinical features, cardiac catheterisation findings, and angiocardiographic results are presented. Twelve patients underwent cardiac surgery. Three patients died, one…

[Surgical treatment of pulmonary valve stenosis].

"Open" pulmonary valvotomy with vena cava occlusión and normothermia was first performed by Varco in 1950, but the procedure was abandoned with the arrival of extracorporeal circulation. This paper compares two groups of patients who were operated with Varco's procedure and extracorporeal circulation. Similar data of both procedures are analyzed as well as their advantages and disadvantages. It is concluded that Varco's proceduce is useful in well selected patients with no associated anomalies in which the possibility of complications of extracorporeal circulation is avoided. Also, equipment,

Pulmonary valve vegetations detected with echocardiography.

In a patient with pseudomonas endocarditis a pulmonary regurgitant murmur developed. Sequential echocardiography demonstrated the initial absence of vegetations, the evolution of pulmonary valve vegetations with relapse and finally the apperance of vegetations on all cardiac valves. The findings were confirmed at autopsy. Echocardiography, a useful technique for evaluating mitral, aortic and tricuspid vegetations, can also detect pulmonary valve vegetations.

Pulmonary Valve Papillary Fibroelastoma: A Rare Source of Pulmonary Embolism.

Papillary fibroelastoma (PFE) is the second most common benign primary cardiac tumor, usually affecting left-sided valves. Pulmonary valve (PV) involvement is extremely rare, representing less than 8% of all PFEs. Although often asymptomatic, PFEs can present with embolic events. We report a 60-year-old man admitted with acute dyspnea, orthopnea, and lower-limb edema. Transthoracic echocardiography revealed severe left ventricular systolic dysfunction (ejection fraction: 15%), severe functional mitral regurgitation, and a mobile echogenic mass (1.0 × 0.8 cm) attached to the arterial surface of…

Quadricuspid pulmonary valve: A rare cause of pulmonary artery aneurysm.

A 17-year-old male presented to our hospital from an external center with complaints of chest pain and shortness of breath, referred with preliminary diagnoses of pulmonary stenosis and pulmonary artery dilation. Echocardiography revealed moderate pulmonary regurgitation and mild pulmonary stenosis. Coronary computed tomography (CT) angiography showed normal coronary arteries and no presence of a patent ductus arteriosus, but an aneurysmal dilation of up to 4 cm was observed in the pulmonary artery. The patient was found to have a quadricuspid (QPV) pulmonary valve, while the aortic valve appe…

Pulmonary valve cyst mimicking pulmonary artery neoplasia: a case report.

Cardiac valve cysts are rare beyond infancy. The tricuspid and mitral valve are the most common sites, with few reported cases of pulmonary valve cysts in the literature. This case illustrates a pulmonary valve cyst mimicking a tumour in a child 13 years after cardiac surgery. Although these lesions are extremely rare, they could be considered in the differential diagnosis in patients presenting with valvular mass lesions.

Pulmonary valve endocarditis: mid-term follow up of pulmonary valvectomies.

Four males aged 20-37 years (three drug addicts and one with a congenital mixed pulmonary valve lesion) were diagnosed in 1989, 1991 and 1993 with pulmonary valve endocarditis without tricuspid infection. Three patients were positive for hepatitis B, C or both, and one patient was HIV-positive. The predominant organism in blood cultures was Staphylococcus aureus. Antibiotic treatment of pulmonary valve endocarditis had failed; thus partial or total valvectomies were performed. Postoperatively, all patients were cured of infection and initial recovery was good. At mid-term follow up (5-10 years…

Risk of Infective Endocarditis Post-transcatheter Pulmonary Valve Replacement Versus Surgical Pulmonary Valve Replacement: A Systematic Review.

Pulmonary valve replacement (PVR) is the most common cardiac operation in adult patients with congenital heart disease (ACHD). It can improve right ventricular outflow tract (RVOT) obstruction, typically due to pulmonary valve stenosis or regurgitation. PVR can be performed surgically (open-heart) and through a transcatheter (percutaneous) method, which is minimally invasive and is associated with shorter hospitalization stays. However, following PVR, infectious endocarditis (IE) can complicate the recovery process and increase mortality in the long term. IE is a rare but deadly multi-organ sy…

Transcatheter pulmonary valve implantation in native pulmonary outflow tract using the Edwards SAPIEN™ transcatheter heart valve.

Percutaneous pulmonary valve implantation (PPVI) is now an accepted alternative option to conventional surgery for patients with dysfunctional conduits between the right ventricle and pulmonary artery. PPVI will reduce the total number of repeat operations in such patients. However, surgery remains the primary option in postoperative tetralogy of Fallot patients with severe pulmonary regurgitation who underwent transannular patch reconstruction of their right ventricular outflow tract (RVOT). Traditionally, an RVOT patch is considered a relative contraindication to PPVI, however, in selected p…
