Clinical Features and ICHD Headache Diagnoses for Patients With Prominent Craniofacial Pain Referred by a Rhinologist to Headache Specialists.

OBJECTIVES: Most patients diagnosed with "sinus headache" are misdiagnosed and mistreated. These patients are often referred to otolaryngology for sinus disease evaluation. However, collaborations between rhinologists and headache specialists for "sinus headaches" have not been investigated. This study aimed to report the clinical features and headache diagnoses of patients referred to headache specialists for prominent craniofacial pain. METHODS: We conducted a retrospective study of patients presenting with craniofacial pain to rhinologists and subsequently referred to a headache specialist

Involvement of the Ipsilateral Tongue, an Intraoral Structure of Referred Pain due to Entrapment of the Greater Occipital Nerve.

This study reports a rare case of referred pain in the trigeminal nerve distribution caused by entrapment of the greater occipital nerve (GON). Notably, the pain extended to the ipsilateral tongue, an unusual intraoral involvement. GON entrapment can lead to sensitization in secondary nociceptive neurons within the trigeminocervical complex (TCC), which receives signals from both trigeminal and occipital nerves, causing referred facial pain. A 55-year-old female presented with chronic left temporo-occipital pain, along with pain in her left periorbital area, ear canal, gum, and a 20-year histo…

Referred pain from the trochlear region in tension-type headache: a myofascial trigger point from the superior oblique muscle.

BACKGROUND: Tension-type headache (TTH) is a prototypical headache in which myofascial trigger points (MTrPs) can play an important role. To our knowledge, MTrPs in the muscle tissues of the trochlear region, ie, the superior oblique muscle (SOM), have not been previously mentioned, and a referred pain pattern from this region has never been reported. OBJECTIVE: To describe the referred pain from the trochlear area based on the examination of MTrPs in the SOM in patients with episodic and chronic TTH (CTTH). DESIGN: A blinded, controlled study. METHODS: The trochlear region was examined in …

The anatomic and physiologic basis of local, referred and radiating lumbosacral pain syndromes related to disease of the spine.

Conscious perception and unconscious effects originating from the vertebral column and its neural structures, although complex, have definite pathways represented in a network of peripheral and central nervous system (CNS) ramifications. These neural relationships consequently result in superimposed focal and diffuse, local and remote conscious perceptions and unconscious effects. Any one or combination of somatic and autonomic signs and symptoms may potentially be observed in a particular patient. This variety and inconsistency may mislead or confuse both the patient and the physician. A clea

The Area of Pressure-Induced Referred Pain Is Dependent on the Intensity of the Suprathreshold Stimulus: An Explorative Study.

OBJECTIVE: To investigate the pain referral area (number of pixels) and extent (vector length) as elicited from increasing intensities of pr … or 60 seconds from low to high intensity or vice versa. The resulting location, area, and extent of referred pain as drawn by the participants on a digital body chart were extracted and expressed in pixels. The extent of pain was defined as the vector length extending from the ipsilateral earlobe to the most distal location of the pain. RESULTS: The referred p …

Hemifacial Trigeminal Pain Referred from Occipital Neuralgia Due to Compression of the Greater Occipital Nerve by the Occipital Artery.

Although pathologic vascular contact between the occipital artery and the greater occipital nerve (GON) at the crossing point in the nuchal subcutaneous layer can cause occipital neuralgia, referred hemifacial trigeminal pain from chronic occipital neuralgia owing to this cause is extremely rare.A 61-year-old female patient with left-sided occipital neuralgia for 4 years presented with a new onset of left-sided hemifacial pain. Decompression of the left GON from pathologic contacts with the occipital artery resulted in immediate relief for hemifacial pain and chronic occipital neuralgia. The p

Referred pain areas of active myofascial trigger points in head, neck, and shoulder muscles, in chronic tension type headache.

Our aim was to analyze the differences in the referred pain patterns and size of the areas of those myofascial trigger points (TrPs) involved in chronic tension type headache (CTTH) including a number of muscles not investigated in previous studies. Thirteen right handed women with CTTH (mean age: 38 ± 6 years) were included. TrPs were bilaterally searched in upper trapezius, sternocleidomastoid, splenius capitis, masseter, levator scapulae, superior oblique (extra-ocular), and suboccipital muscles. TrPs were considered active when both local and referred pain evoked by manual palpation reprod…

Somatosensory hypersensitivity in the referred pain area in patients with chronic biliary pain and a sphincter of Oddi dysfunction: new aspects of an almost forgotten pathogenetic mechanism.

BACKGROUND AND AIMS: Somatosensory hyperalgesia in the referred pain area (RPA) in patients with acute or chronic abdominal pain syndromes may result from the convergence of nerve fibers from visceral and somatic tissues at the spinal and supraspinal levels. Chronic biliary pain in patients with the postcholecystectomy syndrome (i.e., biliary hypersensitivity) may be explained by persistent hyperexcitability of neurons in the central nervous system (CNS). The aim of this study was to evaluate the cutaneous neural sensory perception in the RPA in patients with chronic postcholecystectomy biliar…

Cost-utility of computed tomography in patients with atypical chest pain clinically referred for invasive coronary angiography: randomised controlled trial.

BACKGROUND: Computed tomography (CT) is as safe as invasive coronary angiography (ICA) in the incidence of major adverse cardiovascular events in patients with atypical chest pain. However, the cost-utility of CT and ICA in healthcare after long-term follow-up is still unknown. METHODS: A prespecified cost-utility analysis (CUA) of 329 patients with atypical chest pain from a single-centre randomised trial compared CT and ICA. The CUA was conducted from the health sector perspective up to a 3-year follow-up using quality-adjusted life years (QALYs) from the EQ-5D-3L questionnaire. Costs were

High Prevalence of Previously Undiagnosed Axial Spondyloarthritis in Patients Referred With Anterior Uveitis and Chronic Back Pain: The SpEYE Study.

OBJECTIVE: To reduce the diagnostic delay in axial spondyloarthritis (axSpA), guidelines recommend referring patients with acute anterior uveitis (AAU) and chronic back pain (CBP) to a rheumatologist. This observational study in daily practice evaluated the prevalence of previously unrecognized axSpA in patients with AAU who were referred by ophthalmologists because of concurrent CBP. METHODS: All patients with AAU referred with CBP (≥ 3 months, age of onset < 45 yrs) from 5 ophthalmology clinics underwent rheumatologic assessment, including pelvic radiographs. Patients with previously diagno

Application and utility of a clinical framework for spinally referred neck-arm pain: A cross-sectional and longitudinal study protocol.

BACKGROUND: The clinical presentation of neck-arm pain is heterogeneous with varying underlying pain types (nociceptive/neuropathic/mixed) and pain mechanisms (peripheral/central sensitization). A mechanism-based clinical framework for spinally referred pain has been proposed, which classifies into (1) somatic pain, (2) neural mechanosensitivity, (3) radicular pain, (4) radiculopathy and mixed pain presentations. This study aims to (i) investigate the application of the clinical framework in patients with neck-arm pain, (ii) determine their somatosensory, clinical and psychosocial profile and …
