Classification of Color-Coded Scheimpflug Camera Corneal Tomography Images Using Deep Learning.

PURPOSE: To assess the use of deep learning for high-performance image classification of color-coded corneal maps obtained using a Scheimpflug camera. METHODS: We used a domain-specific convolutional neural network (CNN) to implement deep learning. CNN performance was assessed using standard metrics and detailed error analyses, including network activation maps. RESULTS: The CNN classified four map-selectable display images with average accuracies of 0.983 and 0.958 for the training and test sets, respectively. Network activation maps revealed that the model was heavily influenced by clinica

Knowledge, Attitudes, and Practices Regarding Refractive Eye Surgeries Among Adults in the United Arab Emirates.

Background and aim Refractive errors (REs) are a leading cause of visual impairment globally. Surgical correction methods like laser-assisted in situ keratomileusis (LASIK), laser-assisted subepithelial keratectomy (LASEK), and small-incision lenticule extraction (SMILE) are available in the United Arab Emirates (UAE), but public acceptance and understanding remain inconsistent. This study aims to assess knowledge, attitudes, and practices (KAP) regarding refractive surgery among adults in the UAE and to explore demographic and experiential factors influencing surgical uptake and perception. Physiologic analysis of corneal healing after epikeratophakia.

The evaluation of epithelial permeability, stromal transparency, and endothelial cell density is essential to determine the resumption of normal function in each corneal physiologic unit after refractive surgical procedures. The authors report the results of a prospective study conducted in 55 consecutive patients undergoing epikeratophakia using prelathed, lyophilized tissue lenses. Epithelial permeability was evaluated by means of fluorophotometry preoperatively and 1, 2, 4, 8, 12, 24, and 52 weeks after epikeratophakia. With few exceptions, Scheimpflug photography also was performed at the …

Excimer laser correction of hyperopia, hyperopic and mixed astigmatism: past, present, and future.

The broad acceptance of "spot scanning" or "flying spot" excimer lasers in the last decade has enabled the domination of corneal ablative laser surgery over other refractive surgical procedures for the correction of hyperopia, hyperopic and mixed astigmatism. This review outlines the most important reasons why the ablative laser correction of hyperopia, hyperopic and mixed astigmatism for many years lagged behind that of myopia. Most of today's scanning laser systems, used in the LASIK and PRK procedures, can safely and effectively perform low, moderate and high hyperopic and hyperopic astigma…

Radial keratotomy: background and how to manage these patients nowadays.

In this review, we presented the principles of radial keratotomy (RK), its evolution, enhancement, and complications, and strategies to manage the consequences of RK in the present day. It is essential to understand the RK procedure f, the theoretical background that supported this surgery, the current effect on the cornea, and how to approach patients needing vision improvement. These patients are developing cataracts that need to be handled well, from the IOL calculation to the surgical procedure. Guided keratorefractive surgery is the most accurate procedure to improve these patient's visio

[Presbyopia and refractive surgery].

Presbyopia is the progressive loss of accommodation. Accommodation is a quick, precise and involuntary action which enables the eye to change the power of the young crystalline lens to see clearly at all distances. Presbyopia is and age-linked physiologic phenomenon: it results from aging of the lens, which loses its elasticity, gradually becomes rigid and loses the ability to accommodate. Presbyopia has attained a prevalence of 80% in Europe and is increasing regularly due to the aging of the population. Various surgical techniques can be proposed. Indications depend on age, ophthalmological

[Limbo-sclerotomy--a refractive procedure outside the cornea. Preliminary report].

BACKGROUND: Complications of refractive corneal procedures are a consequence of inevitable corneal deformation or thinning. We aim to reduce corneal refractive power but omit laceration on the cornea itself. METHODS: 1. Rabbits: The procedure war first studied in 16 chinchilla rabbits (32 eyes). Seven eyes were enucleated in different intervals for microscopic evaluation. In rabbits the corneal diameter increased and refractive power of the cornea was diminished by 3.0 to 6.0 diopters. Refraction was controlled after 1, 2, 6 and 12 months. 2. SURGICAL TECHNIQUE: Incision of the conjunctiva 4

Evidence-Based Guidelines for Keratorefractive Lenticule Extraction Surgery.

TOPIC: Development of evidence-based guidelines for keratorefractive lenticule extraction (KLEx). CLINICAL RELEVANCE: Keratorefractive lenticule extraction refers to various corneal refractive procedures involving removal of refractive lenticules of intrastromal corneal tissue, typically through a small incision, eliminating creation of a corneal flap. This technique has gained popularity rapidly; however, no clinical practice guidelines exist. METHODS: These evidence-based guidelines were developed following the World Health Organization guidebook using the Appraisal of Guidelines for Resea

[Refractive surgery for hyperopia].

Hyperopia is a refractive error in which light is focused behind, instead of on, the retina. Clear vision can be obtained by accommodation, but in the long run, this results in eye strain. Hyperopia can be classified as low [≤2.00 diopters (D)], moderate (2.00-4.00 D) and high (>4.00 D). Detailed preoperative evaluation is necessary and essential to obtain good postoperative results. Various surgical techniques can be proposed to correct this ametropia. The main techniques used act either by modifying the corneal curvature with the Excimer laser or by implanting a phakic intraocular lens. The

Optimizing outcomes for keratoplasty in ectatic corneal disease.

PURPOSE OF REVIEW: Advanced keratectasia often requires corneal transplantation for restoration of vision. This review discusses the surgical approach suitable for these cases including significant updates in the current literature. RECENT FINDINGS: With greater understanding of ectatic corneal disease and its associated operative challenges, several modifications in the surgical approach have been developed. Identified predictive factors for successful pneumatic dissection and intraoperative complications have informed surgical planning and decision-making. In the setting of failed pneumatic

The importance of grating orientation in contrast sensitivity following refractive surgery.

Higher-order aberrations degrade visual performance when the pupil is large, particularly in eyes that have abnormally high amounts of aberrations, such as those that have undergone refractive surgery. Geometrical optics predicts that asymmetrical aberrations such as coma can produce orientation selective effects, much like uncorrected astigmatism. Coma is also one of the main aberrations to increase following refractive surgery. Orientation may therefore be an important parameter when testing grating based contrast sensitivity, particularly in refractive surgery patients. Contrast sensitivity

Piggyback intraocular lens implantation.

The piggyback method of implanting two intraocular lenses in one eye has been successfully expanded to address pseudophakic refractive error in normal eyes and eyes that have undergone post-penetrating keratoplasty. Piggyback implantation has been combined with the use of newly available minus-power lenses to provide appropriate power for a cataract patient with keratoconus, as well as to correct pseudophakic myopia. The phenomenon of increased depth of focus in piggybacks may be explained by a contact zone between the lenses. The late complication of inter-lenticular cellular growth with resu
