Selective dorsal rhizotomy for spastic diplegia secondary to stroke in an adult patient.

BACKGROUND: Selective dorsal rhizotomy (SDR) is often recommended for children with spastic paraparesis and cerebral palsy. SDR reduces spasticity in the lower extremities for these children with spastic paraplegia. However, SDR is infrequently recommended for adults with spasticity. Spastic diplegia in adult patients can be due to stroke, brain or spinal cord injury from trauma, infection, toxic-metabolic disorders, and other causes. Although rarely considered, SDR is an option for adult patients with spastic diplegia as well. CASE DESCRIPTION: The authors describe a patient who underwent a …

Functional outcomes of childhood dorsal rhizotomy in adults and adolescents with cerebral palsy.

OBJECT: In this descriptive study the authors evaluated medical outcomes, interventions, satisfaction with life, and subjective impressions about selective dorsal rhizotomy (SDR) in older adolescents and adults who had undergone the procedure as children. METHODS: A survey was administered to older adolescents (16-20 years old) and adults with CP who had undergone SDR between 1986 and 2000 at two academic centers. The patients or their caregivers participated in telephone or clinic interviews. Subjective impressions about the SDR and a history of post-SDR medical interventions were obtained. …

Recurrent atrial tachyarrhythmia triggered by percutaneous balloon rhizotomy of the trigeminal nerve.

Stimulation of sensory branches of the trigeminal nerve is known to cause sudden bradycardia (trigeminocardiac reflex). However we report a case where percutaneous balloon rhizotomy of the trigeminal ganglion provoked atrial tachyarrhythmias during two separate treatments. On both occasions the patient was treated with antiarrhythmic drugs and reverted to sinus rhythm within days. Our case demonstrates that surgery involving the trigeminal nerve may cause variable cardiovascular effects that are often clinically significant. Possible mechanisms and management of arrhythmias in this setting are…

Percutaneous radiofrequency rhizotomy of lumbar spinal facets: the results of 46 cases.

The results of percutaneous radiofrequency rhizotomy of lumbar spinal facets in 46 patients followed at least three months (mean 15 months) are reported and compared with those reported previously. Satisfactory pain relief three months after the procedure was achieved in 36.4 percent of patients without operations and in 41.7 percent of patients, with operations other than fusion. No patient had previously undergone fusion. Treatment of low-back pain by using radio-frequency thermocoagulation of spinal facets is a simple, safe, and well-tolerated procedure. It can be used to relief of pain in …

The effect of rhizotomy on NADPH diaphorase staining in the lumbar spinal cord of the rat.

In the L6-S1 spinal segments of the rat NADPH diaphorase fiber-like staining extending along the lateral edge of the dorsal horn to the sacral parasympathetic nucleus was eliminated 1-3 weeks following dorsal-ventral L6-S1 rhizotomy. However, other NADPH diaphorase staining in the dorsal horn was not affected by rhizotomy. Rhizotomy also doubled the number (5 versus 10 cells/section) of NADPH diaphorase positive neurons in the region of the sacral parasympathetic nucleus (SPN). These data indicate that NADPH diaphorase is present in primary afferent projections to the SPN and that NADPH diapho…

Intraspinal extradural sensory rhizotomy in patients with failure of lumbar disc surgery.

Questionnaires were sent to 60 patients who had undergone an intraspinal extradural sensory rhizotomy after failure of back surgery to assess the efficacy of the procedure. Questionnaires were returned from 47 patients. The operative results were uniformly poor in improving the level of activity of the patient. However, nearly 60% of the patients obtained relief of their pain. Section of only one root, either L-5 or S-1, relieved pain in 50% of the cases. Section of both roots, L-5 and S-1, appeared more effective, since 66% of these patients were relieved of their pain. The technique of perfo…

Trigeminal Neuralgia-Step-by-Step DYNA-Computed Tomography-Assisted Balloon Compression Rhizotomy.

Trigeminal nerve balloon compression (TNBC)1-3 can provide immediate therapeutic relief to patients suffering from trigeminal neuralgia. This is a particularly effective treatment option for patients who are not eligible for surgical procedures (i.e., elderly patients or patients with multiple comorbidities) or for patients who have had an insufficient response to microvascular decompression. TNBC can also be used as a bridge treatment before stereotactic radiosurgery. Use of intraoperative computed tomography-like images using a C-arm system (DYNA-CT) imaging facilitates the TNBC procedure.4,

Long-term outcomes after selective dorsal rhizotomy: a retrospective matched cohort study.

AIM: To examine long-term outcomes of selective dorsal rhizotomy (SDR) 10 to 17 years after surgery. METHOD: Participants who underwent SDR had spastic diplegic cerebral palsy (CP), completed baseline gait analysis, and were 16 to 25 years old at follow-up. Non-SDR participants (i.e. controls) were matched on important clinical parameters at baseline but did not undergo SDR. All study participants completed six surveys assessing pain, quality of life, participation, function, and mobility. Treatment history for lower extremity surgery and antispasticity injections was tabulated. A subset of e…

Does selective dorsal rhizotomy improve bladder function in children with cerebral palsy?

PURPOSE: To investigate the efficacy of selective dorsal rhizotomy (SDR) on urinary symptoms and bladder function in cerebral palsy children. METHODS: Selective dorsal rhizotomy was performed in 56 children with spastic cerebral palsy. Intraspinal nerve root divisions over the cauda equina from L1/2 to S1/2 levels were performed. Urinary symptoms and urodynamic study (UDS) parameters before and after SDR were analyzed. RESULTS: Fifty-four out of 56 children (mean age 7.7) with SDR performed had complete urinary symptoms for analysis, of which 90 % had diplegic cerebral palsy. Fifty-one child…

Long-term functional benefits of selective dorsal rhizotomy for spastic cerebral palsy.

OBJECT: Large-scale natural history studies of gross motor development have shown that children with spastic cerebral palsy (CP) plateau during childhood and actually decline through adolescence. Selective dorsal rhizotomy (SDR) is a well-recognized treatment for spastic CP, but little is known about long-term outcomes of this treatment. The purpose of this study was to assess the durability of functional outcomes in a large number of patients through adolescence and into early adulthood using standardized assessment tools. METHODS: The authors analyzed long-term follow-up data in children wh…

Deformity of lumbar spine after selective dorsal rhizotomy for spastic cerebral palsy.

To observe the effect of selective dorsal rhizotomy (SDR) on the deformity of the lumbar spine, a retrospective review was conducted from October 1992 to December 2002 on children who had undergone SDR in our department, and in whom preoperative and postoperative spine radiographs had been obtained. The angles of hyperlordolysis were measured. The study group comprised 219 patients, in which 61 cases were followed up and examined between May 2004 and February 2005. The mean age at surgery was 6.9 years (range 3-20 years), with a mean time to recent follow-up radiographs of 6.3 years (5-9 years…
