Assessment of postoperative pain following single versus multiple-visit root canal treatment: A randomised clinical trial.

Postoperative pain following root canal procedures remains a significant clinical challenge, with on-going controversy regarding optimal single- versus multiple-visit treatment regimens for mandibular molars with symptomatic irreversible pulpitis. This study compared postoperative pain, flare-ups and analgesic consumption between single-visit (n=60) and multiple-visit (n=60) root canal therapies using standardized rotary instrumentation and warm vertical obturation in 120 matched patients. Visual Analog Scale (VAS) measurements revealed similar pain trends across both groups, peaking at 6-12 h

Deposition of calcified tissue around an overextended gutta-percha cone: case report.

Root canal treatment was performed in a mandibular right second premolar with a periapical lesion and apical resorption. The root canal was prepared with K-files using the step-back technique and 3% NaOCl as an irrigant; during obturation gross overfilling of gutta-percha occurred. The tooth was permanently restored with a post and core along with a crown. Although healing of the periapical lesion occurred and the patient reported that he was symptom-free, the tooth was extracted after 4 years because of a subgingival root fracture. Following extraction the tooth was examined with SEM. The exa

Endodontic Retreatment of a Mandibular First Premolar With Unusual Four Root Canal Morphology: A Case Report.

INTRODUCTION: Mandibular premolars exhibit complex root canal systems, making them highly challenging to manage endodontically. Canal configurations vary from one to five canals. Successful endodontic treatment requires a thorough knowledge of standard anatomy and its variations. CASE PRESENTATION: This case report presents the endodontic retreatment of a right mandibular first premolar with four separate root canals in a 36-year-old male. Initial clinical and radiographic examinations showed a complex root canal anatomy. With the aid of a dental operating microscope (DOM) and cone-beam compu

Effect of Different Obturation Techniques on Sealer Penetration in Teeth with Artificial Internal Root Resorption: a Confocal Laser Microscope Analysis.

OBJECTIVE: To investigate the efficacy of different obturation techniques on sealer penetration in teeth with internal root resorption using confocal laser microscopy. METHODS: An artificial internal resorption cavity (3 mm deep and 1.2 mm in diameter) was formed in the round-shaped root canals of 45 single-rooted teeth at a distance of 7 mm from the apex, then roots were instrumented (size 40/.06). The samples were divided into three groups (n = 15) according to the obturation technique: lateral compaction (LC), warm vertical compaction (WVC) and carrier-based (CB). RESULTS: In the resorpti

Maxillary molars with morphologic variations of the palatal root canals: a report of four cases.

INTRODUCTION: The purpose of this article was to show the importance of the knowledge of the anatomy of root canals. Unusual root and root canal morphologies associated with both buccal roots of upper molars have been recorded in several studies in the literature. However, scientific information focusing on variations of the palatal root is rare. METHODS: In this report, four cases are presented involving the root canal treatment of maxillary first and second molars with unusual morphologic configurations of the palatal root canals. RESULTS: During root canal treatment, type IV and V configu

In Vitro Comparison of Void Formation Between Calcium Silicate-Based Sealers and AH Plus: A Systematic Review and Meta-Analysis.

Background: Achieving a three-dimensional seal of the root canal system is considered essential for long-term endodontic success. Because no clinically acceptable threshold for void formation has been established, comparisons with clinically validated reference materials are needed to interpret the sealing quality of newer calcium silicate-based sealers (CSBSs). AH Plus is a well-established epoxy resin-based sealer (ERBS) with documented long-term clinical performance. This systematic review and meta-analysis compared the quality of root canal fillings produced with CSBSs and AH Plus using di

Quantitative radioactive analysis of microleakage of four different retrograde fillings.

Sealing properties of four different retrograde filling materials were investigated in vitro. Radioactive isotopes were applied in the root canal, and leakage into an extraradicular fluid was measured at regular intervals. The method permitted repeated observation of the specimens over prolonged periods of time. Forty single-rooted human teeth were biomechanically instrumented and obturated using calcium-hydroxide paste. Following obturation, an apicectomy was performed and retrograde cavities were filled with four different materials: group 1, non gamma 2 amalgam (Amalcap); group 2, glass ion

Disappeared roots: a case report.

Tooth resorption is a perplexing problem for all dental practitioners. The etiologic factors are vague, the diagnoses are educated guesses and, often, the chosen treatment does not prevent the rapid disappearance of the calcified dental tissues. This becomes all the more confusing if the tooth in question is a pulpally involved young permanent tooth. Presented in this report is the case of an upper first young permanent molar that underwent complete root resorption following root canal therapy and obturation.

Reattachment of subgingivally fractured central incisor with an open apex.

A case report of a 6-year-old girl with a fractured maxillary left central incisor with an open apex is presented. The procedure used to repair the fracture included flap surgery with an intrasulcular incision and endodontic treatment. The patient was called for 3 months regular follow-up to check the root formation. At the end of 32 months just before the root was obturated by guttaperka, she fractured the same tooth. Flap surgery was repeated and the tooth was restored. The root canal was obturated with a root filling paste and guttaperka as the apex was closed. Examination 10 months after t

Management of Radix Paramolaris With a Distal Canal in the Mandibular First Molar: A Case Report.

Mandibular first molars, also called six-year molars as they appear at around six years of age, are the first permanent teeth to erupt in the oral cavity. They are the teeth most commonly affected by dental caries. Anatomically, the tooth presents with two roots and three canals. In very rare cases, presence of an extra root or a supernumerary root has been associated with the tooth. When this root is present lingual to the distal root, it is called radix entomolaris whereas when it is present buccal to the mesial root, it is called radix paramolaris. The presence of veiled canals is possible

[Advanced research on root canal therapy for primary teeth].

Primary tooth root canal therapy is a treatment performed on primary teeth diagnosed with pulpitis or periapical periodontitis. This procedure requires perfect instrumentation, disinfection, and filling of root canals to eliminate infection, control inflammation, relieve pain, prevent pathological effects on inherited permanent tooth, and prolong primary tooth preservation. This paper reviews the research history on primary tooth root canal treatment and summarizes the progress on primary tooth root canal treatment, including anatomical morphology, root canal preparation, root canal disinfecti

Cementum regeneration in replanted teeth with differing pulp treatment.

Cementum regeneration on exposed dentinal root surfaces of replanted teeth not subjected to replantation. The implication was that secondary reattachment of replanted teeth was largely dependent on the status of supporting alveolar tissues. The role of the replant appeared essentially passive, reacting as a substrate for cementum deposition that may be more or less acceptable to the regenerating periodontal tissues as dictated by the extraoral history. Although the pattern of cementum deposition was unaltered by replantation, the quantity of new cementum deposited on exposed dentin did appear

Influence of different drying protocols on bond strength of epoxy resin- and calcium silicate- based root canal sealers in nonsurgical retreatment.

BACKGROUND: The condition of root canal dentin and the drying protocol applied prior to obturation may influence the bonding performance of root canal sealers, particularly in retreatment cases where dentin characteristics are altered. This study aimed to evaluate the effect of different drying protocols on the push-out bond strength of epoxy resin- and calcium silicate-based sealers in primary root canal treatment and nonsurgical retreatment. METHODS: A total of 192 extracted human mandibular premolars were prepared and randomly allocated into groups according to drying protocol (paper point

Internal resorption treated with mineral trioxide aggregate in a primary molar tooth: 18-month follow-up.

Mineral trioxide aggregate (MTA) repair of a mandibular second-primary molar tooth with an inflammatory resorptive defect in the coronal third of the root canal is reported in this case. After the root canals were obturated with zinc oxide eugenol sealer, MTA was applied to the resorption cavity. The coronal cavity was then restored with a stainless steel crown. At an 18-month follow-up, the tooth was asymptomatic clinically and radiographically. According to this result, it can be concluded that MTA may be a suitable material for use in the treatment of internal resorption in the coronal thir

Calcium ion diffusion from calcium hydroxide-containing materials in endodontically-treated teeth: an in vitro study.

The aim of this in vitro study was to determine the amount and duration of diffusion of calcium ions from both a calcium hydroxide-containing root canal sealer and an intracanal medicament, through the apical foramen and the dentinal tubules of endodontically-treated teeth. The root canals of 88 freshly extracted single-rooted teeth were prepared using the modified double flared, balanced force technique with patency filing. The teeth were divided into four test groups of 20 teeth each and a control group of eight teeth. One group was dressed with a non-setting calcium hydroxide while the othe

Apical surgery with calcium hydroxide capping of the exposed dentine: a case report.

Among a variety of biomaterials that have been reported to be ideal for dental repair, calcium hydroxide has been shown to have excellent long-term biocompatibility in the pulp and periapical areas. Here we report an alternative method employing calcium hydroxide for periapical surgery in a patient who developed internal apical resorption after traumatic injury, which negatively affected the quality of the cleaning, shaping and filling of the root canal. Obturation of the root canal as far as the middle third was followed by apicoectomy and ultrasonic retropreparation, and then retrograde root
