Halo scalp ring: a form of localized scalp injury associated with caput succedaneum.

Caput succedaneum is a common birth injury attributed to cervical, uterine, or vaginal pressure on the fetal presenting part. It usually consists of diffuse soft tissue swelling and bruising, which resolve in a few days without sequelae. We have seen two patients who had an unusual alopecic "halo" ring on the scalp associated with a caput succedaneum.

Diagnostic accuracy of fetal scalp lactate for intrapartum acidosis compared with scalp pH.

OBJECTIVE: To determine the diagnostic accuracy of fetal scalp lactate sampling (FSLS) and to establish an optimal cut-off value for intrapartum acidosis compared with fetal scalp pH. METHODS: A 20-month retrospective cohort study was conducted of all neonates delivered in our institution for whom fetal scalp blood sampling (FSBS) was performed, matching their intrapartum gasometry to their cord gasometry at delivery (n=243). The time taken from the performance of scalp blood sampling to arterial umbilical cord gas acquisition was 45 min at most. Five arterial cord gasometry patterns were set…

Spongy Scalp Swelling in a Middle-Aged Female: A Case Report of Lipedematous Scalp.

Lipedematous scalp is a rare cutaneous disorder, characterized by subtle but conspicuous scalp swelling, usually associated with dysesthesia. The chronic recalcitrant nature of this condition can be extremely debilitating for the patient. We report a case of boggy scalp swelling and dysesthesia in a 37‑year‑old female present for five years. Magnetic resonance imaging (MRI) brain showed thickening of subcutaneous tissue of the scalp. Histopathological examination revealed thickened and edematous subcutaneous tissue, reaching up to the upper dermis. A diagnosis of lipedematous scalp was made. T…

A New Paradigm in the Management of Scalp Pruritus: Findings From the SCALP-PR Trial.

PURPOSE: The "Impact of scalp pruritus in dermatological consultations in Spain: The SCALP-PR trial" was initiated to address the common yet often insufficiently examined issue of scalp pruritus in dermatology. This condition leads to an uncontrollable urge to scratch, affecting the patients' quality of life and potentially causing scalp damage. This study aimed to explore the prevalence, patient profile, underlying conditions, and therapeutic approaches for scalp pruritus in Spain, and to assess the safety and efficacy profile, as well as the tolerability of a non-pharmacologic treatment. ME…

Helical tomotherapy of the complete scalp and the ipsilateral lymph nodes in a case of scalp angiosarcoma.

Scalp angiosarcoma represents a therapeutic challenge to all disciplines. This case report demonstrates the potential usefulness of helical tomotherapy (HT) as a new radiotherapeutic treatment option. A 71-year-old woman presented with a superficial angiosarcoma of the scalp, forehead, and left pre- and postauricular areas, with several nodular ulcerating and bleeding lesions. Irradiation of the gross tumor was performed with a total dose of 70 Gy in 2-Gy fractions and of the left cervical lymph nodes with 56 Gy in 1.6-Gy fractions. Good target coverage was achieved without compromising organs…

Reconstruction of massive defects of the scalp, cranium, and dura after resection of scalp neoplasms.

Massive defects of the scalp, cranium, and dura can be covered with local rotation, transposition scalp flaps. Five cases of massive defects of up to 300 square centimeters resulting from resection of scalp neoplasms were reconstructed by this technique. Excellent cosmetic and functional results were obtained in all cases. Although the emphasis in the recent literature has been on free flap coverage of these massive defects, our series demonstrates that these extensive scalp defects can be reconstructed using large local scalp flap transposition.

Using Feily's method prevented scalp necrosis in three patients incline to the scalp recipient necrosis; what is new in prevention of scalp necrosis?

Serious complications arising from surgical hair restoration are relatively uncommon following well-performed and well-planned surgery by skillful surgical techniques, good communication, and postoperative follow-up. Surgical complications often categorized as those which occur in the donor site and the recipient site. In this paper among recipient area complication we focused on recipient area necrosis that arises when an increased number of recipient grafts are utilized and de-vascularization of the scalp occurs as a result of the large wound area due to the dense packing splitting of recipi…

[Value of fetal scalp lactate sampling during labour: a comparative study with scalp pH].

OBJECTIVE: To evaluate the validity and the feasibility of fetal scalp lactate sampling during labour in the assessment of non reassuring fetal status. PATIENTS AND METHOD: A prospective observational study was conducted on 129 fetal scalp blood samplings. Scalp lactate measurements were compared to scalp pH (gold standard procedure), neonatal cord blood gas (pH and lactates) and Apgar score. Pathological values taken were for scalp lactates>5 mmol/l, scalp pH<7.20, cord arterial pH<7.10, cord arterial lactates>6,35 mmol/l, and Apgar score<7 at one and five minutes. All measurements were perf…

Horizontal and vertical sections of scalp biopsy specimens from dermatomyositis patients with scalp involvement.

BACKGROUND: The histologic findings of scalp involvement in dermatomyositis are not well characterized on account of a lack of large series. OBJECTIVE: To systematize the histologic features of scalp involvement in dermatomyositis on horizontal and vertical sections. METHODS: A descriptive, prospective, cross-sectional study recruited 31 patients with pathologically and serologically confirmed dermatomyositis in Mexico City, Mexico, from June 2014 to June 2015. A total of 36 scalp biopsy specimens from 20 patients with scalp involvement in dermatomyositis were processed as 20 vertical and 16…

Extended Posterior Auricular Artery Flap for Temporo-Parietal Scalp Defect in Previously Irradiated Scalp.

BACKGROUND: Temperoparietal scalp defects are fairly common resulting from varied array of etiology. Various reconstructive options exisit, but the aim was to achieve a reliable and simple solution keeping in mind the principle of replacing "like with like." METHODS: The authors present a 83-year-old lady who had a fall at her home and was lying unconscious for quite some time on her right side of her temple, leading to full thickness necrosis of overlying scalp. She had received surgery and prior radiotherapy for her basal cell carcinoma of the scalp at the same site of her present injury. S…

Development of a new classification and scoring system for scalp conditions: Scalp Photographic Index (SPI).

BACKGROUND AND OBJECTIVES: There has been no validated tool for objectively quantifying the overall condition and characteristics of the scalp. This study aimed to establish and validate a new classification and scoring system for evaluating scalp conditions. METHODS: The Scalp Photographic Index (SPI) using a trichoscope grades five features of scalp conditions (dryness, oiliness, erythema, folliculitis, and dandruff) on a score of 0-3. To evaluate the validity of SPI, SPI grading was performed by three experts on the scalps of 100 subjects along with a dermatologist's assessment of the scal…

Visualization of Hair Bulbs through the Scalp: A Trichoscopic Feature of Erosive Pustular Dermatitis of the Scalp.

Erosive pustular dermatitis of the scalp (EPDS) typically affects elderly people with scarring alopecia. Videodermoscopy (VD) of the scalp has never been assessed in the diagnosis of EPDS. To evaluate the trichoscopy features of EPDS, we carried out a retrospective analysis of VD images obtained in the scalp of ten patients who affected by EPDS and were compared with those obtained from a series of thirty patients who affected by other type of scarring alopecia. The most specific VD feature was thus represented by evident hair bulb in the scarring scalp, observed in all patients.
