Predictive Modeling of Clinically Apparent Shoulder Periprosthetic Joint Infection (PJI).

INTRODUCTION: The first International Consensus Meeting (ICM) definition of shoulder periprosthetic joint infection (PJI) introduced a tiered framework of PJI categories based on weighting of minor criteria for those cases not having major criteria of Definite infection. The selection of minor criteria and weightings was based on expert opinion and limited available evidence. The purpose of this study was to use prospective, multicenter data to develop a predictive model for clinically apparent shoulder PJI to inform a more evidence-based understanding of minor criteria and PJI categories. Joint angle estimation during shoulder abduction exercise using contactless technology.

BACKGROUND: Tele-rehabilitation, also known as tele-rehab, uses communication technologies to provide rehabilitation services from a distance. The COVID-19 pandemic has highlighted the importance of tele-rehab, where the in-person visits declined and the demand for remote healthcare rises. Tele-rehab offers enhanced accessibility, convenience, cost-effectiveness, flexibility, care quality, continuity, and communication. However, the current systems are often not able to perform a comprehensive movement analysis. To address this, we propose and validate a novel approach using depth technology a

Shoulder joint loading and posture during medicine cart pushing task.

Excessive physical loads and awkward shoulder postures during pushing and pulling are risk factors for shoulder pain. Pushing a medicine cart is a major component of a work shift for nurses and medical assistants in hospitals and other health care facilities. A laboratory experiment was conducted to examine the effects of common factors (e.g., lane congestion, cart load stability, floor surface friction) on shoulder joint moment and shoulder elevation angle of participants during cart pushing. Participants pushed a medicine cart on straight tracks and turning around right-angle corners. Peak s…

Shoulder surgery: from cuff repair to joint replacement. An update.

REPAIRING FULL THICKNESS CUFF TEARS.--Despite the sound rationale for repairing full-thickness rotator cuff tears, the procedure may fail to restore cuff integrity, which is indispensable to optimal cuff function. The functional role of each cuff muscle and the factors associated with anatomic failure (particularly those related to the muscles and tendons) provide a basis for rational patient selection and for determination of the best surgical strategy on a case-by-case basis. [The management of complete shoulder joint dislocation without metallic implants].

In 14 patients reduction of the acromioclavicular joint was reinforced by completely absorbable polydioxanon cord. In 3 cases parts of the cord were rejected and a fistula formed through the scar without further impairment following as a result of healing local treatment. Altogether we had 12 good and 2 poor results with respect to function, X-ray appearance and pain. The complication rate and general outcome of the method described do not differ substantially from those of other commonly used procedures. However, no second operation for removal of the metal implants is necessary.

Pulmonary embolism in shoulder arthroplasty: a National Joint Registry Study.

INTRODUCTION: In the UK, there is no specific guidance on prophylaxis for venous thromboembolism (VTE) following shoulder arthroplasty (SA). This study reviews the current incidence of pulmonary embolism (PE) and prophylaxis practices following SA in England. METHODS: Data were obtained from the National Joint Registry (NJR) for all SAs from 1 April 2012 to 31 March 2022 and linked with Hospital Episode Statistics (HES). ICD-10 codes identified incidence of PE at 30 and 90 days from arthroplasty. Incidence for PE was calculated by cohort and implant type at 30 and 90 days postoperatively. Mul

An Unusual Cause of Shoulder Pain: Sternoclavicular Joint Septic Arthritis.

Septic arthritis of the sternoclavicular joint (SCJ) is an uncommon location of septic joint infection and is often a diagnostic challenge. Patients may present with insidious, vague symptoms, such as chest pain or shoulder pain, and imaging modalities are often nonspecific, leading to a delay in diagnosis and treatment. We present a case of a young male patient initially presenting with right shoulder pain, but who then returned days later and was diagnosed with SCJ septic arthritis. This patient was treated successfully with SCJ resection and intravenous antibiotics. This case illustrates th

Anatomic Reversal of Shoulder Joint After Neck of Humerus Fracture.

We report on a singular case of a unique form of post-traumatic reversal of the humeral head after humeral neck fracture, in which the pattern of collapse resulted in the formation of a native reverse polarity shoulder. In essence, the humeral head became a socket, and the glenoid rounded to become a head with well-preserved shoulder function. To our knowledge, this is the first case of an acquired shoulder deformity that bears a remarkable functional similarity to a prosthetic reverse polarity shoulder replacement.

Periprosthetic Joint Infection of Shoulder Arthroplasties: Diagnostic and Treatment Options.

Periprosthetic joint infection (PJI) is one of the most frequent reasons for painful shoulder arthroplasties and revision surgery of shoulder arthroplasties. Cutibacterium acnes (Propionibacterium acnes) is one of the microorganisms that most often causes the infection. However, this slow growing microorganism is difficult to detect. This paper presents an overview of different diagnostic test to detect a periprosthetic shoulder infection. This includes nonspecific diagnostic tests and specific tests (with identifying the responsible microorganism). The aspiration can combine different specifi

Glenohumeral joint tuberculosis that mimics frozen shoulder: a retrospective analysis.

BACKGROUND: Although the incidence of osteoarticular tuberculosis is increasing, glenohumeral joint tuberculosis is rare and often misdiagnosed in its early stages. Our objective was to study the incidence of the initial misdiagnosis as frozen shoulder and the duration of the prediagnostic period among patients with glenohumeral joint tuberculosis. METHODS: The clinical records of 21 patients with tuberculosis of the shoulder joint were retrospectively analyzed. RESULTS: Among the 16 patients with glenohumeral joint tuberculosis, 14 (87.5%) were initially diagnosed as having frozen shoulder …

Shoulder joint kinetics during the push phase of wheelchair propulsion.

The purpose of this investigation was to quantify the forces and moments at the shoulder joint during free, level wheelchair propulsion and to document changes imposed by increased speed, inclined terrain, and 15 minutes of continuous propulsion. Data were collected using a six-camera VICON motion analysis system, a strain gauge instrumented wheel, and a wheelchair ergometer. Seventeen men with low level paraplegia participated in this study. Shoulder joint forces and moments were calculated using a three-dimensional model applying the inverse dynamics approach. During free propulsion, peak sh…

OBJECTIVE: To determine clinical and radiographic findings in and treatment and outcome of large animals with shoulder joint luxations. DESIGN: Retrospective study. ANIMALS: 5 horses, 3 goats, 1 calf, 1 sheep, 1 Himalayan tahr, 1 pot-bellied pig, 1 reindeer, and 1 white-tailed deer. PROCEDURE: Medical records and radiographs were reviewed to determine signalment, history, physical examination findings, type of luxation, treatment, and outcome. Owners and referring veterinarians were contracted for follow-up information. RESULTS: Goats, sexually intact males, and animals < 1 year old were o…

Capsular elasticity and joint volume in recurrent anterior shoulder instability.

It is commonly claimed that unstable shoulders have an altered capsular elasticity and increased joint volume. To find out if there is a correlation between anterior instability and increased joint volume, 20 patients were examined while undergoing arthroscopic surgery of the shoulder. There were four anterior subluxators, 11 anterior dislocators, and five bilateral dislocators, and in all patients the contralateral shoulder was also examined. During continuous saline infusion, a pressure/volume curve was recorded within the range of 0-180 mm Hg. Joint volume was correlated with body surface a
