BACKGROUND: Somatoform disorders can be understood to mimic supposedly more 'legitimate' physical disorders. To the extent that mental disorders are now also often considered legitimate, might clinicians expect to encounter the psychiatric equivalent of somatoform disorders, 'psychiatriform disorders'?
METHOD: The relevant literature on somatoform disorders is reviewed in light of the tendency for mental and physical symptoms to co-occur.
DISCUSSION: Illness attribution and behaviour may explain some of the recent rise in the prevalence of mental disorder. Hypotheses regarding the cause and

Adjustment to the inherent changes of adolescence is a challenge for many children, even healthy ones. Epilepsy may make successful adjustment even more difficult by disrupting self-control, adversely affecting the ability to conform to one's peers, and interfering with independence. Epilepsy and its treatment may affect education, employment, use of alcohol and illicit drugs, sexual activity, and pregnancy. These issues, in turn, may affect seizure control. Matching the medicine to the patient is particularly important in teenagers, a group of patients with special needs. Common seizure syndr

Malignant melanoma (MM) is a cancer with rapidly growing incidence in all age groups and-if not detected early-mostly poor prognosis. After wide excision of the primary tumour, patients without metastases are subjected to follow-up examinations. No further standard medical treatment is currently available for such patients. This results in psychosocial stress, and psychological adjustment and social support are needed-especially in patients with poor prognosis and visible scars. This article reviews recent evidence of the psychosocial correlates in melanoma disease in the context of past resea

An increase of depressive symptoms in fibromyalgia patients has been noted in a number of studies. The etiologic significance of this finding remains, however, controversial. We suggest that a subgroup of patients with this symptom combination may be pragmatically classified as suffering from somatized depression. Clinical indicators such as a family history of depressive disorders, circadian disturbances, pronounced loss of appetite or libido, and chronic psychosocial stressors should be assessed and, if present, prompt the initiation of psychiatric evaluation and treatment including pharmaco

Gilles de la Tourette's syndrome, a combination of multiple chronic tics and vocalizations, usually first occurring during childhood, is described in its history, symptomatology, genetics, etiology and therapy. Traditionally TS has been viewed either as an organic or as a psychogenic disorder. We propose an integrative concept combining both aspects. During a vulnerable phase in childhood a hypersensitivity of dopamine 2-receptors, induced by gene defects or perinatal trauma, leads to a lack of suppression of subcortical programs which discharge as tics. Tics are modified by multiple psycholog

The study examined medical records of 121 medical-surgical inpatients diagnosed with adjustment disorder by psychiatric consultants in a university hospital. Medical illness was the primary stressor, evoking the maladaptive reaction in 83 (68.6%) cases. These patients were largely free of preceding psychiatric problems, suffering protracted hospitalizations for advanced illnesses, particularly malignancy and diabetes; in contrast, the 38 (31.4%) patients whose adjustment disorder was precipitated by a stressor other than medical illness had established psychiatric histories and recurrent probl

An appreciative cooperation among dermatology, clinical psychology and psychology and psychiatry is necessary and useful. By selected facts interactions between central and autonomous nervous system on one side and the immune system on the other side were described. As far as the neutrocutaneous interrelations are concerned the cutaneous stimulation appears to be an important factor for the physical and physiologic development. In addition, the skin communicates to the environment and other people. Therefore, it plays a role in the social integration. Psychiatric diseases can affect the skin (

Engel's hypothesis of pain-prone patients having a distinct pattern of developmental psychosocial experiences was tested in a controlled design including four groups of 20 patients each: a) psychogenic pain, b) organic pain, c) psychogenic bodily symptoms, d) organic disease. On admission an open-ended interview, including childhood experiences was tape-recorded. Measures were taken to minimize observer-bias. Patients in group a) had, compared to the other groups, significantly increased prevalence of: "Parents, verbally and/or physically abusive of each other", "parents, abusive of the child"

Two breast cancer groups (mastectomised or chemotherapeutic intervention) and a control group of healthy female nurses were given a demographic questionnaire and the Minnesota Multiphasic Personality Inventory. The personality profiles of all three groups emerged as significantly different from each other on all scales with the exception of social introversion and psychopathic deviance. Both cancer groups displayed inflated scores on the clinical scale Depression. A separate series of univariate F tests revealed that the mastectomised patients were characterised by elevated scores on the clini

An association between chronic pain and depression has been recognized for a long time. However, the exact nature of this relationship remains unclear. The authors studied 71 patients for affective disorders and schizophrenia-lifetime version (SADS-L). Based on the interviews, we were able to identify 31 patients with major depression, 8 patients with minor depression and 18 with intermittent depressive disorder as defined by Research Diagnostic Criteria. Item analysis using the Hamilton Depression Rating Scale and the Montgomery-Asberg Depression Rating Scale showed that the items did not dis

College undergraduates were asked the degree to which they believed certain behaviors and characteristics are present in mentally retarded and mentally ill persons. Comparisons of responses showed that subjects clearly differentiated the concepts, although several areas overlapped. Mental retardation was characterized by physical stigmata and brain damage, developmental delays, and cognitive deficits; mental illness, by emotional lability due to environmental, hereditary, or mixed factors. How knowledge of people's perceptions of these disorders is essential for a more complete understanding o

This paper describes an example of acting out of countertransference during a brief but unexpected serious illness. My emphasis is on the regression that occurred with hospitalization and how it dovetailed with a particular patient's transference. A confluence of my residual neurosis, the patient's neurosis, her transference state, her other characteristics (that she was female, for example), plus the fact that she was the last patient contacted, all conspired with the regression from the trauma of being hospitalized to produce the countertransference reaction. This presentation is also intend

The study reported here was undertaken to establish the degree to which a person in a preclinical state of hyperthyroidism, with (by definition) euthyroid T3 and T4 levels but suppressed TRH on testing, already exhibits psychological changes and clinical symptoms. Two groups of 20 patients each, with clear clinical and preclinical hyperthyroidism (as defined by laboratory parameters), were studied, as well as a group of 20 controls. The subjects' psychological state of mind was investigated using self-rating scales, including the state-trait-anxiety inventory (STAI), "Befindlichkeits"-Skala (B

A motivational theory of depression states that depression consists of low expectations and low values for rewards. This theory accurately describes one class of depression, the affective disorders. The present study tested its accuracy for another class of depression, adjustment disorder with depressed mood. This class of depression is precipitated by environmental stresses, such as chronic illness. The focus was therefore on people (N = 48) who were experiencing terminal cancer and nonterminal cancer. Results supported each prediction generated by the motivational theory. The cancer patients

The similarities between the HRC and the DDP have been presented. The HRC has been described by many investigators in the psychoanalytic literature. Indications are that both the HRC and the DDP are working through the same mechanism. The similarities are as follows: Both types of patients are driven by a need to seek treatment from someone they will ultimately reject. A self-fulfilling prophecy develops as a cycle leading to failure. Due to lack of trust and a desire to feel important, the patient creates an atmosphere that leads to discord, hostility, anger, and frustration. Resultant discor

This paper examines whether traditional medicine promotes biopsychosocial fulfillment in African health and argues that every society has its own method of managing illness and of controlling the environment. The paper shows how each society functions within the constraints of its own unique system of social structure, language and communication, belief and customs, attitudes, behaviour and 'cultural facts' upon which health services are based. The concept, meaning and uses of traditional medicine are also discussed. It shows how traditional medicine can be used as a 'psychological opium' as a
