Malignant Purkinje ectopy induced by sodium channel blockers.

BACKGROUND: Sodium channel blocker (SCB) infusion is used to unmask the electrocardiographic pattern of Brugada syndrome. The test may also induce premature ventricular complexes (PVCs) in individuals without Brugada pattern, the clinical relevance of which is little known. OBJECTIVE: The purpose of this study was to describe the prevalence of short-coupled (Sc) PVCs induced by ajmaline or flecainide in patients with suspected or documented severe ventricular arrhythmias. METHODS: We reviewed the SCB tests performed in 335 patients with suspected ventricular arrhythmias and structurally norm

Sodium channel Nav1.8: Emerging links to human disease.

The NaV1.8 sodium channel, encoded by gene SCN10A, was initially termed sensory neuron-specific (SNS) due to prominent expression in primary sensory neurons including dorsal root ganglion (DRG) neurons. Early studies on rodent NaV1.8 demonstrated depolarized voltage dependence of channel inactivation, a slow rate of inactivation, and rapid recovery from inactivation. As a result of these biophysical properties, NaV1.8 supports repetitive firing in response to sustained depolarization. This article reviews recent studies that reveal multiple links of NaV1.8 to human disease: (1) It has recently

Sodium Channel Mutations and Pyrethroid Resistance in Aedes aegypti.

Pyrethroid insecticides are widely used to control insect pests and human disease vectors. Voltage-gated sodium channels are the primary targets of pyrethroid insecticides. Mutations in the sodium channel have been shown to be responsible for pyrethroid resistance, known as knockdown resistance (kdr), in various insects including mosquitoes. In Aedes aegypti mosquitoes, the principal urban vectors of dengue, zika, and yellow fever viruses, multiple single nucleotide polymorphisms in the sodium channel gene have been found in pyrethroid-resistant populations and some of them have been functiona…

Glycosylation-dependent activation of epithelial sodium channel by solnatide.

Dysfunction of the epithelial sodium channel (ENaC), which regulates salt and water homeostasis in epithelia, causes several human pathological conditions, including pulmonary oedema. This is a potentially lethal complication of acute lung injury at least partially caused by dysfunctional alveolar liquid clearance, which in turn impairs alveolar gas exchange. Solnatide (named TIP-peptide, AP301), a 17 residue peptide mimicking the lectin-like domain of TNF has been shown to activate ENaC in several experimental animal models of acute lung injury and is being evaluated as a potential therapy fo

Could a defective epithelial sodium channel lead to bronchiectasis.

BACKGROUND: Bronchiectasis is defined as a permanent dilation of the airways arising from chronic bronchial inflammation/infection. In 50% of cases, no etiology can be identified. Recently, the role of the epithelial sodium channel ENaC has been pointed out in the pathophysiology of cystic fibrosis, a disease due to mutations in the CFTR gene and causing bronchiectasis in the airways. Moreover, it was found that transgenic mice overexpressing ENaCbeta present cystic fibrosis-like lung disease symptoms. Our aim was to evaluate if a defective ENaC protein could be involved in the development of

Immunochemical analysis of the sodium channel in rodent and human eye.

The presence of the amiloride-sensitive sodium channel (ASSC) in ocular tissues was studied with the aid of a polyclonal antiserum raised against the 14 amino acid peptide QGLGKGDKREEQGL. This sequence corresponds to the region 44-58 of the alpha subunit of the channel, termed ENaC, cloned from rat colon. The antibody titers, measured by the ELISA technique, rose to 1∶2560 4 weeks after immunization, and this bleed was used in all subsequent experiments. Immunoblotting with the polyclonal anti-alphaENaC serum, revealed a major band of 82-86 kDa in extracts prepared from whole bovine or r

Epithelial sodium channel activity is not increased in hypertension in whites.

Abnormal renal sodium transport causing excess reabsorption of sodium may be one mechanism that causes high blood pressure. For example, increased activity of epithelial sodium channels in the distal tubule is the cause of high blood pressure in Liddle's syndrome, a rare familial form of hypertension. We have shown that the increase in sodium channel activity can be detected in the nose using transepithelial potential difference measurements in 1 family with Liddle's syndrome. We therefore used nasal potential difference measurements to look for increased sodium channel activity in white patie…

[Recent progress in sodium channel structures relevant to their functions].

Efforts to reveal specific aspects of protein structure with channel function have been carried out for more than a decade. Recent progresses in this field have been achieved by site-directed mutagenesis of the sodium channel DNA as well as other techniques. Among them I briefly review four topics in this article: (1) extracellular mouth of the ion conducting pore and ion selectivity filter which are correlated to the site of tetrdotoxin binding, (2) cytoplasmic mouth of the ion pore which is correlated with the binding site of local anesthetics, (3) inactivation gate, parts of which are relat

Membrane topology of the amiloride-sensitive epithelial sodium channel.

The amiloride-sensitive epithelial sodium channel (ENaC) is involved in fluid and electrolyte absorption across a number of epithelia, and cloning of several ENaC subunits has begun to facilitate investigation of the structure, function, and regulation of this channel. Analysis of the amino acid sequence has revealed two potential membrane-spanning domains, but little else is known about the structure of ENaC. To investigate the membrane topology of one subunit, alpha rENaC, we used in vitro transcription, translation, and translocation into microsomal membranes. This generated a glycosylated

Linkage of atypical myotonia congenita to a sodium channel locus.

We performed linkage analysis in a pedigree segregating an allele for autosomal dominant, painful myotonia that is potassium sensitive and responsive to acetazolamide. This allele was tightly linked to a skeletal-muscle, sodium channel locus which is now a candidate for the site of the mutational defect in acetazolamide-responsive myotonia congenita. Since this sodium channel locus is completely linked to the disease allele in all hyperkalemic periodic paralysis and paramyotonia congenita pedigrees studied, the molecular alteration causing acetazolamide-responsive myotonia congenita is likely

Molecular properties of the sodium channel: a receptor for multiple neurotoxins.

Sodium channels are the receptors for more than 10 distinct classes of biological toxins which act at five or more receptor sites. Brevetoxins and ciguatoxins act at neurotoxin receptor site 5. This receptor site is located on the alpha subunit of the sodium channel, and peptide segments in domain IV of that subunit are important in brevetoxin binding.

[Treatment of ventricular arrhythmia resistant to sodium channel blockaders].

The efficacy of antiarrhythmic agents of various groups (Na(+)- and Ca(2+)-channel blockers, cordarone) was comparatively evaluated in the same patients with ventricular arrhythmias. Cordarone and Na(+)-channel blockers were equally effective, whereas verapamil was significantly less potent. Then the antiarrhythmic efficacy of verapamil and cordarone was assessed individually in rhythm disturbances responsive to Na(+)-channel blockers and resistent to their action. The efficacy of verapamil was significantly higher in patients resistant to Na+ blockers than in those responsive to them. The act

Pumiliotoxin alkaloids: a new class of sodium channel agents.

Pumiliotoxin B (PTX-B) and a variety of congeneric alkaloids and synthetic analogs stimulated sodium flux and phosphoinositide breakdown in … , unlike local anesthetics, had little or no effect on binding of [3H]batrachotoxinin A benzoate to sodium channels. Thus, it appears likely that some "inactive" congeners bind to the PTX-B binding site, but do not activate sodium channels. In the absence of scorpion venom the stimulation of phosphoinositide breakdown in synaptoneurosomes was consonant with the s …

Interaction of brevetoxin A with a new receptor site on the sodium channel.

Measurements of neurotoxin-activated 22Na influx in neuroblastoma cells and neurotoxin binding in synaptosomes were used to define the site and mechanism of action of brevetoxins on sodium channels. Brevetoxin A alone did not enhance the sodium permeability of neuroblastoma cells, but markedly enhanced persistent activation of sodium channels by veratridine, aconitine and batrachotoxin, which act at neurotoxin receptor site 2. Enhancement of batrachotoxin action was accompanied by a 4.3-fold increase in the binding of [3H]batrachotoxinin A 20-alpha-benzoate to receptor site 2 on sodium channel…

The sodium channel and intracellular H+ blockage in squid axons.

Sodium channels in plasma membranes can be blocked by a large variety of toxins and local anaesthetics. This property, however, is not confined to relatively large molecules. For instance, extracellularly applied small ions like hydrogen may also prevent the passive transport of permeant cations across open Na+ channels. A typical feature of this phenomenon is that the blocking action of hydrogen is gradually relieved by increasing the voltage applied across the membrane. Although in the frog skeletal muscle there is no clear evidence for a similar intracellular action, we report here for the …

Sodium ion channels as therapeutic targets in epilepsy: a drug discovery perspective.

Traditional broad-spectrum sodium channel blockers have long served as the mainstay of epilepsy therapy, yet their clinical utility is constrained by dose-limiting adverse effects and the risk of exacerbating certain genetic epilepsies. Targeting the specific Nav subtypes, including Nav1.1, Nav1.2, Nav1.3, and Nav1.6, has emerged as a promising therapeutic strategy to mitigate these liabilities. In this Perspective, we highlight the structure, physiological functions, and their pathological roles in epilepsies, and analyze the clinical status of Pan-Nav inhibitors and the recent advances in dr

CONCLUSIONS: Antifibrotic drugs partially prevent TGF-β1-induced suppression of epithelial sodium channels, preserving epithelial ion homeostasis. Restoration of ENaC expression may represent a novel mechanism by which antifibrotic therapy mitigates sodium-associated lung fibrosis.

The analgesic paracetamol metabolite AM404 acts peripherally to directly inhibit sodium channels.

Paracetamol has been used for decades to relieve mild-to-moderate pain. Its analgesic effect is mainly attributed to its metabolite, AM404, acting on cannabinoid receptors or TRPV1 channels in central nervous system (CNS) neurons. Here, we show that AM404 is produced by primary sensory neurons. It inhibits sodium current in nociceptor neurons, blocking action potential (AP) generation and reducing nocifensive behavior in naïve and inflamed rats. We demonstrated that this analgesic effect of AM404 is mediated by its direct inhibition of nociceptive voltage-gated sodium channels (NaV) 1.8 and 1.…

Nociceptor sodium channels shape subthreshold phase, upstroke, and shoulder of action potentials.

Voltage-gated sodium channels (VGSCs) in the peripheral nervous system shape action potentials (APs) and thereby support the detection of sensory stimuli. Most of the nine mammalian VGSC subtypes are expressed in nociceptors, but predominantly, three are linked to several human pain syndromes: while Nav1.7 is suggested to be a (sub-)threshold channel, Nav1.8 is thought to support the fast AP upstroke. Nav1.9, as it produces large persistent currents, is attributed a role in determining the resting membrane potential. We characterized the gating of Nav1.1-Nav1.3 and Nav1.5-Nav1.9 in manual patc…
