The Language Profile of Preschool Children With 22q11.2 Deletion Syndrome and the Relationship With Speech Intelligibility.

PURPOSE: Young children with 22q11.2 deletion syndrome (22q11DS) often have impaired language development and poor speech intelligibility. Here, we report a comprehensive overview of standardized language assessment in a relatively large sample of preschool-aged children with 22q11DS. We furthermore explored whether speech ability explained variability in language skills. METHOD: Forty-four monolingual Dutch preschoolers (3-6 years) with a confirmed genetic 22q11DS diagnosis participated in this prospective cohort study. Standardized tests (Clinical Evaluation of Language Fundamentals Prescho…

Agreements between speech language pathologists and naïve listeners' judgements of intelligibility in children with cleft palate.

Speech-language pathologists (SLPs) working in interdisciplinary craniofacial teams need to know how much their judgments of speech intelligibility could predict the patient's communication difficulties with non-clinical communicative partners. This study examines the relationship between experienced SLPs and naïve listeners in judgments of speech intelligibility in speakers with cleft palate. A speech perception study was conducted using speech samples from 20 speakers with velopharyngeal insufficiency (VPI) following cleft palate. Speech samples were presented to 70 naïve listeners and 10 SL…

Binaural squelch effect in unilateral otosclerosis surgery: comparison of speech intelligibility in noise before-after surgery.

PURPOSE: We aimed to evaluate the speech intelligibility benefit in noise provided by stapedotomy in the treatment of unilateral otosclerosis. METHODS: We enrolled adults suffering from unilateral conductive hearing loss and followed them up until 9 months after surgery. The patients underwent a free field speech hearing evaluation using the French Matrix test before and after stapedotomy. Speech material was sent to the front of the patients (S0) and noise was presented either at the front (N0), or at the operated ear (N-90) or at the non-operated ear (N + 90). The speech intelligibility ben…

Speech intelligibility of Parkinson's disease patients evaluated by different groups of healthcare professionals and naïve listeners.

INTRODUCTION: Speech intelligibility, how well a listener comprehends the speaker's message, is related to the listener' expertise and type of the message conveyed. There is no evidence about speech intelligibility in different groups of healthcare professionals and naïve listeners. OBJECTIVES: This study is the first to understand if there were differences in the speech intelligibility of Parkinson's Disease (PD) patients by different experienced and naïve listeners, according to the speech stimuli and dysarthria severity. MATERIALS AND METHODS: Randomly digitised audio-files (50 words and …

Frequency-lowering processing to improve speech-in-noise intelligibility in patients with age-related hearing loss.

OBJECTIVE: Modern hearing aids use various signal-processing strategies to improve speech intelligibility. In this manuscript, we studied the linear frequency transposition (LFT), a frequency-lowering algorithm, in patients with age-related hearing loss. Frequency-lowering algorithms transpose high-frequency sounds to a lower-frequency band. The study aimed to assess whether LFT could be used as a tool to improve speech intelligibility in patients with a better high-frequency preservation. METHODS: The study population consisted of 77 patients (age average 74.8 ± 12.4) wearing hearing aids wi…

The Impact of Dysphonic Voices on Healthy Listeners: Listener Reaction Times, Speech Intelligibility, and Listener Comprehension.

PURPOSE: There is currently minimal information on the impact of dysphonia secondary to phonotrauma on listeners. Considering the high incidence of voice disorders with professional voice users, it is important to understand the impact of a dysphonic voice on their audiences. METHODS: Ninety-one healthy listeners (39 men, 52 women; mean age = 23.62 years) were presented with speech stimuli from 5 healthy speakers and 5 speakers diagnosed with dysphonia secondary to phonotrauma. Dependent variables included processing speed (reaction time [RT] ratio), speech intelligibility, and listener compr…

Speech-in-noise intelligibility does not correlate with efferent olivocochlear reflex in humans with normal hearing.

CONCLUSION: According to the presented data, speech-in-noise intelligibility (SI) does not correlate with olivocochlear efferent activity - as measured by contralateral suppression (CS) of distortion product otoacoustic emissions (DPOAE) in humans with normal auditory threshold. OBJECTIVES: Literature data indicate a possible role of the medial olivocochlear efferents in speech intelligibility, especially in background noise. The objective of this study was to investigate this relationship. MATERIALS AND METHODS: SI was evaluated in three independent sessions by determining the ratio speech …

[Otoacoustic emissions, auditory evoked potentials, pure tone thresholds and speech intelligibility in cases of auditory neuropathy].

The combined findings of "normal evoked otoacoustic emissions (EOAE) and absent or markedly disturbed auditory evoked potentials from the brainstem and/or cortex" has been named auditory neuropathy in the English literature. The pure tone thresholds in these patients range from mild to severe loss. The loss of speech comprehension is out of proportion in comparison to the pure tone threshold elevation. This combination of findings suggests that cochlear function and in particular outer hair cell function, is normal in these patients but also suggests that the inner hair cell / VIII nerve funct

Effects of signal processing on intelligibility of speech in noise for persons with sensorineural hearing loss.

A person with sensorineural hearing loss accompanied by recruitment has difficulty in understanding amplified speech, especially when speech is in the presence o competition. Two groups of subjects with sensorineural hearing loss with recruitment were tested at +6 and at O dB signal-to-noise (S/N) ratio at preferred levels for unprocessed speech and for speech processed through a two-band compression/equalization of circuit. In addition, unprocessed key word identification scores were compared to those from use of a hearing aid and of a one-band compression/equalization circuit. Processing a t

Simulation of the effects of loudness recruitment and threshold elevation on the intelligibility of speech in quiet and in a background of speech.

These experiments simulated the threshold elevation and loudness recruitment associated with three different types of cochlear hearing loss: Moderate flat (condition R2), severe flat (condition R3), and moderate-to-severe sloping (condition RX). This was done to allow an examination of the effects of these factors on the intelligibility of speech, in isolation from other factors that are normally associated with cochlear hearing loss, such as reduced frequency selectivity. The simulation was performed by splitting the input signal into 13 frequency bands, and processing the envelope in each ba

Visual and Acoustic Aspects of Face Masks Affect Speech Intelligibility in Listeners with Different Hearing Statuses.

Background: When speaking while wearing a face mask, sound transmission is attenuated, and visual cues are lost due to the covered facial movements of the speaker. In this study, we investigated the extent to which different face masks alter speech intelligibility in individuals with different degrees of hearing impairment. Methods: A total of fifty participants were divided into four hearing status groups according to the degree of hearing loss: normal levels (16), mild (13), moderate (11), and severe (10). A modified version of the Audiovisual German Matrix Sentence Test (AV-OLSA) was used t…

Improving the Quality and Intelligibility of Electrolaryngeal Speech during Mobile Communication with Landline Analogous Bandpass Filtering.

The normal functioning of the larynx or voicebox gives humans the ability to converse with others effortlessly. However, if the larynx is permanently removed due to injury or illness, speech cannot be produced. The electrolarynx, a noninvasive device used to produce speech for such persons, has a motor-induced hum which hinders telephone-based communication among electrolaryngeal speakers. This adversely affects the morale of electrolarynx users, leading to abandonment of technology, psychological issues and decreased quality of life. This paper examines a technique based on bandpass filtering

Acoustic vowel analysis and speech intelligibility in young adult Hebrew speakers: Developmental dysarthria versus typical development.

BACKGROUND: Developmental dysarthria is a motor speech impairment commonly characterized by varying levels of reduced speech intelligibility. The relationship between intelligibility deficits and acoustic vowel space among these individuals has long been noted in the literature, with evidence of vowel centralization (e.g., in English and Mandarin). However, the degree to which this centralization occurs and the intelligibility-acoustic relationship is maintained in different vowel systems has yet to be studied thoroughly. In comparison with American English, the Hebrew vowel system is signific…

An overview of the HASPI and HASQI metrics for predicting speech intelligibility and speech quality for normal hearing, hearing loss, and hearing aids.

Alterations of the speech signal, including additive noise and nonlinear distortion, can reduce speech intelligibility and quality. Hearing aids present an especially complicated situation since these devices may implement nonlinear processing designed to compensate for the hearing loss. Hearing-aid processing is often realized as time-varying multichannel gain adjustments, and may also include frequency reassignment. The challenge in designing metrics for hearing aids and hearing-impaired listeners is to accurately model the perceptual trade-offs between speech audibility and the nonlinear di…

Role of intelligence tests in speech/language referrals.

This study examined the relation of the WISC-R Verbal IQ with measures of oral and written language among 190 students referred to a private educational clinic over a 5-yr. period. Correlations of Verbal IQ with scores on measures of oral language, written language, receptive language, reading comprehension, and basic reading skills were calculated for the total sample and by Grades 1-3, 4-7, and 8-11. Standard regression coefficients were used to estimate the proportion of variance explained by these five measures. Significant correlations were found for Verbal IQ with the measures, ranging f
