Fifteen patients (eight males, seven females; age range: 23-76 years) presenting with acute suppurative cholangitis underwent endoscopic retrograde cholangiography and sphincterotomy within 1-10 days of hospitalization. Cholangitis was due to common duct stones in all patients; all but one of them had their gall-bladders in situ. All of them had fever, jaundice, abdominal pain, leucocytosis and deranged liver function while 26.6% were in shock, 13.3% in coma and 40% in azotaemia. Of a total of 1791 biliary tract operations from 1977-1984 transduodenal sphincterotomy was performed in 6.3% of 1720 primary and in 63.4% of 71 secondary operations, altogether in 154 cases. In many centers endoscopic sphincterotomy is replacing surgery, which has never been an ideal treatment for retained common duct calculi. We attempted endoscopic sphincterotomy in 70 patients, succeeding in 60 (85%). Sixty-one patients had choledocholithiasis (58 postcholecystectomy), 7 had papillary stenosis, 1 carcinoma of the papilla of Vater, and 1 hydatid disease. This paper reports the results of endoscopic sphincterotomy for bile duct stones in 50 patients. A design for a spincterotomy handle and a suitable stainless steel diathermy wire are described. Sphincterotomy was achieved in 45 patients (90%) and complete stone clearance in 42 (84%); this usually required two endoscopic examinations. Complications occurred in nine patients. This review summarizes the past history and presence of endoscopic papillotomy (EPT, sphincterotomy) in the treatment of biliary and pancreatic tract diseases. The indications of the different techniques used, and the comparison of the effectiveness of EPT with that of the traditional surgical treatment is also discussed. Based on the results, EPT is an up-to-date technique with good effectiveness, low risk for patients and widening indication range. EPT in the future, will play a determining role in the effective treatment of biliary and pancreatic tract diseases.

The presence of pancreatitis does not constitute a contraindication to endoscopic sphincterotomy in the management of common duct calculi. On the contrary, experience with 10 cases of gallstone pancreatitis suggests that endoscopic sphincterotomy can be carried out safely with little disturbance to the patient, and that it greatly simplifies the management of these cases. In the frail and elderly patient little further may be required.
