Hepatic splenosis is a nodular implant of normal spleen tissue in the liver. This innocent liver nodule is frequently misinterpreted as a malignancy. Almost all hepatic splenoses have been associated with a clinical history of splenic trauma or prior surgery. This report describes two cases of hepatic splenosis. In both patients, the nodular lesions were initially thought to be liver malignancies and they were ultimately assessed by histology. The spleen plays a significant role in the defence against infections, especially infections caused by encapsulated bacteria, such as Streptococcus pneumoniae. In case of (functional) asplenia, a fulminant sepsis with a high mortality rate may develop in a short time. INTRODUCTION: Inflammatory pseudotumours are benign lesions of unknown aetiology that can appear almost in any organ, sometimes along with infectious agents like Epstein-Barr virus. A retrospective study of spleen findings in 42 victims of drowning and a comparison group of 42 cases of asphyxiation due to other causes (hanging, ligature strangulation and manual strangulation), that were matched for sex, age, body weight and build, was performed. A case of inflammatory pseudotumour of spleen in which plasma cells were the predominant cell type, is described. It presented as multiple nodules in the spleen of a 37 year old woman who died of myocardial infarction, and was an incidental finding at necropsy. The history of a Moroccan girl is described with splenomegaly, lymphadenopathy and pancytopenia after a holiday in her native country. Bone marrow smears were considered negative for Leishmaniasis in four different laboratories. All other diagnostic options could also not be confirmed. Reexamination of the bone marrow smears in a laboratory for tropical diseases revealed Leishmania donovani organisms. Treatment with sodium antimony gluconate was successful. Epidemiology, symptoms and diagnostic problems are discussed.

Scintiscans of the spleen were performed in 62 hospital patients under observation for abdominal contusion. No abnormality was detected in 82% of the cases, and the course of events showed that the spleen was intact. Lesional images were found in the remaining 18%. Injury to the spleen was excluded by other investigations in 10% (false positive results) but confirmed in 8% who underwent laparotomy. This fast and non aggressive method can be recommended outside emergencies to exclude rupture or haematoma of the spleen.

Spontaneous rupture of spleen, is a rare abdominal catastrophe, during pregnancy and labour, associated with 100% maternal and foetal mortality if not diagnosed in time. An awareness about this rare entity on the part of an obstetrician leading to timely diagnosis, prompt splenectomy and proper replacement of blood provides good prognosis for mother and foetus. A rare case of spontaneous rupture of NORMAL spleen during labour with delivery of a live baby by lower segment caesarean section followed by splenectomy is presented and the literature is reviewed.
