Splenic rupture as an infectious mononucleosis complication.

Splenic rupture is a rare yet serious complication associated with an infectious mononucleosis, especially in those patients with atypical clinical manifestations or those whose presentations are nonspecific or even completely asymptomatic. Since the death in these occasions is mostly attributed to acute massive hemorrhages, an early diagnosis and treatment is of paramount importance. This report illustrates that in infectious mononucleosis, one should be aware of the possibility of an unusual clinical presentation and the enlarged spleen may rupture spontaneously or by a causal traumatic forc

Spontaneous splenic rupture complicating acute Q fever.

Q fever is usually a self-limited febrile illness that involves the lungs and the liver. Acute complications are rare. We present the case of a 30-yr-old patient with spontaneous splenic rupture during the course of acute Q fever infection. He was admitted to the hospital with high temperature and the radiological signs of an atypical pneumonia. Forty-eight hours after admission, he developed shock. Because of free intraabdominal liquid, a laparatomy was performed that revealed a tear in the enlarged spleen. A splenectomy was performed. The diagnosis of Q fever was established by a significant

Severe meningococcal septicaemia associated with splenic rupture.

OBJECTIVE: To present the first report of ruptured spleen associated with meningococcal septicaemia. CLINICAL FEATURES: A 13-year-old girl presented with an acute abdomen and clinical signs of meningococcal septicaemia. Features of her illness placed her in a high mortality group. INTERVENTION AND OUTCOME: She required cardiovascular and respiratory support in the Intensive Care Unit. Failure to initially identify the organism led to percutaneous fine-needle aspiration of fluid in the lesser sac. A laparotomy revealed free intraperitoneal blood and a ruptured spleen. During the course of her

Rupture of splenic abscess through the diaphragm.

Splenic abscess is an uncommon disease that has a high mortality rate if not treated surgically. With the aid of new diagnostic tools, such as ultrasonography, scanning and computerized axial tomography, it is possible to diagnose the entity antemortem. Even though it often has a nontypical clinical course, it may overlap other systemic diseases. We describe an unusual case of splenic abscess with rare complications, the operation and the outcome.

Fatal splenic rupture in Ehlers-Danlos syndrome.

A fatal case of Ehlers-Danlos syndrome (Type IV) is described. Autopsy revealed splenic rupture that had resulted from spontaneous haemorrhage into the splenic parenchyma. This is a previously unrecorded complication of Ehlers-Danlos syndrome and should be considered in the differential diagnosis of chest and abdominal pain in patients with this condition.

[Splenic rupture. Diagnosis, clinical aspects and therapy].

It is reported about 73 traumatic ruptures of the spleen in a period of 19 years. The most frequent cause for rupturing were traffic accidents. The most important diagnostic procedure was peritoneal lavage, a rapid and well-aimed method. Therapy of choice is splenectomy. Prognosis depends on the time from accident to operation as well as on the severity of accompanying injuries. The course and cause of lethal courses are summarized. Ten patients who died in the postoperative phase, had polytraumatic injuries with three of more injured organs.

A case of splenic torsion and rupture presenting as ruptured ectopic pregnancy.

Splenic torsion with rupture of spleen is an extremely rare phenomenon. The clinical picture mimics several common conditions which are causes of acute abdomen and so it is seldom detected pre-operatively. An 18 year old female patient was admitted with an acute abdomen and shock. The provisional diagnosis was of a ruptured ectopic pregnancy. Peri-operatively we found a spontaneous rupture of the spleen following torsion along with early intrauterine pregnancy. Splenectomy was carried out and patient recovered well. Our report confirms that this rare entity can present as an acute abdomen whic

Systemic Melioidosis With Ruptured Splenic Abscess.

Melioidosis is a severe systemic disease caused by the bacterium Burkholderia pseudomallei, commonly found in soil, ground water, and ponds of endemic regions. It is transmitted to humans via percutaneous inoculation while working in these areas without protective clothing and footwear giving rise to the disease which has a high case fatality rate. It has a wide range of clinical manifestations, varying from asymptomatic infection to localized abscess formation to fulminating disease with multiple organ involvement and even death. Currently, there are no known pathognomonic features or specifi

[Complications in the conservative managemennt of splenic rupture].

INTRODUCTION: Management of splenic rupture in haemodynamically stable children is non-surgical treatment. However, complications can occur during follow-up. Objective: to study the frequency, evolution and treatment of complications of conservative treatment of splenic rupture. Secondary objective: to evaluate the results of the American Pediatric Surgical Association (APSA) clinical guideline in conservative treatment. MATERIAL AND METHODS: Retrospective study of patients with abdominal trauma between 2010-2016. We included children under 15 years of age with splenic injury after blunt abdo

Splenic rupture as a presenting feature of endocarditis.

We describe the first case of infective endocarditis presenting with spontaneous splenic rupture. Our patient, a known intravenous drug user presented with hypovolaemic shock secondary to splenic rupture. The patient was resuscitated and underwent an emergency splenectomy. Subsequent clinical examination revealed a systolic murmur and a diagnosis of mitral valve infective endocarditis was made after echocardiography. Splenic tissue, blood cultures and mitral valve tissue all cultured Enterococcus faecalis. The patient had a successful mitral valve replacement and was discharged home after 44 d

The management of splenic rupture in infectious mononucleosis.

Non-operative management of splenic trauma is now well established; however, the role of conservative management in spontaneous splenic rupture is undetermined. The leading cause of spontaneous splenic rupture is infectious mononucleosis. We report on the management of four patients with spontaneous rupture, in association with infectious mononucleosis. Three patients eventually required splenectomy, and one was successfully managed non-operatively. The comparative risks of operative and non-operative management are discussed. We believe that when splenic rupture complicates infectious mononuc

Ruptured splenic cavernous hemangioma in a neonate.

We present a case of a term neonate with hypovolemic shock after spontaneous vaginal delivery. Hemodynamic instability persisted despite resuscitation with packed red cells, fresh frozen plasma, and platelets. An ultrasound scan at 48 hours after birth followed by a computed tomographic scan demonstrated a splenic lesion and hemoperitoneum. She underwent an emergency laparotomy and splenectomy for splenic rupture. Histologic findings demonstrated a ruptured cavernous hemangioma of the spleen. Exsanguinating intraabdominal hemorrhage in the newborn infant is rare. The diagnosis and management,

[Rupture of a splenic aneurism in an emergency situation].

We report on a 36 year old patient who collapsed at home and was resuscitated by prehospital medical emergency services. He presented on scene unconscious with severe ST-elevations on the ECG and hardly palpable pulses. His previous medical history included only idiopathic hypertension and his professional background as manager of a company was associated with high stress levels. The prehospital diagnosis was myocardial infarction with cardiogenic shock. A hypovolemic shock was excluded from the differential diagnosis because of the age of the patient, lack of a precipitating trauma and incons

Splenic rupture as the presenting manifestation of vasculitis.

BACKGROUND: Although underreported, histologic splenic involvement in Wegener's granulomatosis (WG) is not unusual. Splenic rupture in association with WG, however, is rare. Only 2 cases of nontraumatic splenic rupture have been reported as the initial feature of WG. Isolated cases of splenic rupture also have been noted in rheumatoid arthritis, systemic lupus erythematosus, and polyarteritis nodosa. OBJECTIVE: To report the third case of splenic rupture as the presenting feature of WG and review the literature concerning splenic rupture in other rheumatologic diseases to better delineate a m

Ruptured splenic abscess presenting as pneumoperitoneum.

Spontaneous pneumoperitoneum follows perforation of hollow viscus; rarely, it may arise from pulmonary interstitial emphysema or intestinal inflammatory disease. We report a 30-year-old man with ruptured splenic abscess who presented with acute abdomen and had pneumoperitoneum. He was treated with splenectomy and is asymptomatic 2 months later.

[Infectious mononucleosis and the risk of splenic rupture].

Infectious mononucleosis is a common and benign disease, affecting mainly teenagers and young adults. Patients with IM are often generally advised to restrain from physical exertion for a period of 6 months. We report one case of splenic rupture in a patient with IM which required emergency laparotomy and splenectomy and one case of splenic rupture which was managed with conservative treatment. We have retrospectively examined the 37 cases of IM admitted during the period 1990-1994 and found that liver function tests return to normal within eight weeks. We therefore advise that patients with I

A traumatic splenic rupture in a healthy adult.

Splenic rupture in blunt and penetrating trauma is well documented. Other etiologies include hematologic and inflammatory disorders. The case presented documents occult splenic rupture. There was no history of trauma. Furthermore, no evidence of systemic disease or pathology could be found. Occult, spontaneous splenic rupture is rarely diagnosed, although the associated mortality/morbidity is high. The astute clinician must have a high index of suspicion relying on clinical and roentgenographic study for rapid diagnosis and management.

Ruptured arteriosclerotic aneurysm of the splenic artery.

A case of arteriosclerotic aneurysm of the splenic artery with rupture into the transverse colon resulting in secondary perforation of the cecum is presented. Symptomatology, diagnosis and treatment of aneurysms of the splenic artery are briefly illustrated. Aneurysm of the splenic artery must be suspected by the presence of uncharacteristic upper abdominal pain and the annular calcification in the left upper quadrant on roentgenogram. The diagnosis is verified by arteriography. Operation of all symptomatic splenic artery aneurysms is recommended. Asymptomatic aneurysms measuring more than 3 c
