OBJECTIVE: To describe a significant but poorly understood public health problem, the authors compiled data on swimming pool drownings and near-drownings requiring hospitalization for California children ages 1 to 4.
METHODS: Data from death certificates were used to analyze swimming pool drownings, and hospital discharge data were used to analyze near-drownings.

An outbreak of otitis externa was observed to affect one third of 230 swimmers using a new swimming pool within three weeks of its opening. Pseudomonas aeruginosa was grown from the water and from all of the nine swabs taken from the infected ears of the swimmers. During the same period only six other cases of otitis externa were seen in the local general practice serving 4,000 patients. The disinfection procedures were found to be defective and after they were corrected the outbreak subsided.

In order to compare the standard swim-up semen preparation with and without test yolk buffer (TYB) incubation in intrauterine insemination (IUI), we conducted a prospective multicentre randomized trial. A total of 121 infertile couples with male factor (n = 52) or unexplained infertility (n = 69) was randomly assigned to two groups following ovulation induction. The forced swim test (FST) is a widely used animal model of depression and antidepressant drug screen. Rats are forced to swim on two test days in a restricted space from which there is no escape. On the first test day the rats attempt to escape and then become largely immobile; on the second test day the onset of immobility is more rapid. On 3 February 2004, the Vermont Department of Health received reports of acute gastroenteritis in persons who had recently visited a swimming facility. A retrospective cohort study was conducted among persons attending the facility between 30 January and 2 February. Fifty-three of 189 (28%) persons interviewed developed vomiting or diarrhoea within 72 h after visiting the facility. Swimming efficiencies of fish and cetaceans have been related to a certain synchrony between stroke cycle frequency, peak-to-peak tail/fluke amplitude and mean swimming speed. On May 6, 1998, the Olmsted County Public Health Department initiated an investigation into an outbreak of diarrheal illness that had occurred among people who swam frequently at a local swimming pool. Interviews of swimmers and microbiological testing of stool samples and swimming pool filter material were conducted to determine the source of the outbreak. Twenty-six of 206 swimmers interviewed had illnesses that met the case definition. The most common symptoms were diarrhea (100 percent), abdominal cramps (81 percent), and nausea (77 percent). An outbreak of eight cases of cryptosporidiosis in Hampshire over a period of eight weeks in the summer of 1996 was linked to use of one swimming pool. Cryptosporidial oocysts were not isolated from samples of backwash, but the presence of enterobius ova indicated faecal contamination and a case control study including the first four primary cases suggested an association with immersion in the pool. Even in small outbreaks case control studies can provide useful supportive evidence as to the possible source of infection.

An outbreak of cyptosporidiosis occurred in late April 1993 among resort hotel guests which was temporally associated with, but geographically distant from, a massive waterborne outbreak of cryptosporidiosis in Milwaukee, Wisconsin, that occurred in late March and early April of 1993. In July 1989 an outbreak of shigellosis occurred among visitors to a recreational park in Oakland County, MI. An epidemiologic investigation discovered an association between illness and swimming in a pond at the park, especially for those who had put their head underwater. The incidence of molluscum contagiosum (MC) in children attending 15 public elementary schools and 9 kindergartens in Chigasaki City was studied by questioning their parents. Special attention was paid to the relationship between MC and swimming, and the incidence of MC in the swimming group was twice as high as that of the nonswimming group. This study also revealed that the predilection for lesions was the side of the trunk and the axillae.

Effects of antidepressants and other drugs on the behaviour of rats in the forced swimming test were examined. Acute and chronic administration of antidepressants reduced the duration of immobility during the first 5 or 10 min of a 30 min test by prolonging escape-directed behaviour which appeared only during this period.
