A novel optical technology based on 690 nm and 850 nm wavelengths to assist needle thoracostomy.

The sensitivity of pneumothorax diagnosis via handheld ultrasound is low, and there is no equipment suitable for use with life-threatening tension pneumothorax in a prehospital setting. This study proposes a novel technology involving optical fibers and near-infrared spectroscopy to assist in needle thoracostomy decompression. The proposed system via the optical fibers emitted dual wavelengths of 690 and 850 nm, allowing distinction among different layers of tissue in vivo. The fundamental principle is the modified Beer-Lambert law (MBLL) which is the basis of near-infrared tissue spectroscopy

A Randomized Non-Inferiority Clinical Trial of 14Fr Thal versus 28Fr Tube Thoracostomy for Traumatic Hemothorax.

BackgroundThe traditional treatment of traumatic hemothorax (HTX) is large bore chest tubes (CT) ≥28Fr. Recent evidence shows 14Fr pigtail catheters are as effective in drainage of HTX as larger CT. However, this has not been shown in 14Fr Thal tubes, a straight chest tube placed utilizing Seldinger technique.MethodsA single center, prospective randomized controlled trial was performed at an ACS verified Level 1 trauma center comparing 14Fr Thal CT (14CT) to 28Fr CT (28CT) between May 2017 and September 2021. The primary outcome was failure of drainage of hemothorax requiring additional interv

Using chest X-ray to predict tube thoracostomy in traumatic pneumothorax: A single-institution retrospective review.

BACKGROUND: Traumatic pneumothorax (PTX) is a common occurrence in thoracic trauma patients, with a majority requiring tube thoracostomy (TT) for management. Recently, the "35-mm" rule has advocated for observation of patients with PTX less than 35 mm on chest computed tomography (CT) scan. This rule has not been examined in chest x-ray (CXR). We hypothesize that a similar size cutoff can be determined in CXR predictive of need for tube thoracostomy. METHODS: We performed a single-institution retrospective review of patients with traumatic PTX from 2018 to 2022, excluding those who underwent

Clinical outcomes and prognostic factors of open-window thoracostomy for empyema: a retrospective review of 40 patients.

PURPOSE: Open-window thoracostomy remains a traditional yet important option for treating empyema. This study aimed to evaluate the clinical outcomes after open-window thoracostomy and identify any prognostic factors based on our institutional experience. METHODS: We retrospectively reviewed the medical records of 40 consecutive patients who underwent open-window thoracostomy at our institution between January 2010 and December 2020. RESULTS: The mean age of the patients was 68.3 years, and twenty-five were male. Thirty-five patients had empyema with a fistula and five had empyema without a

Incidence and predictors of complications of tube thoracostomy performed in the emergency department for non-traumatic pleural pathologies.

Tube thoracostomy (TT) is a common emergency procedure for the management of non-traumatic pleural conditions in the emergency department (ED). While effective, it carries a significant risk of complications. We aimed to assess the incidence and predictors of complications associated with TT performed in the ED for non-traumatic pleural pathologies. Retrospective cohort study involving adult patients undergoing TT in the ED of a University Hospital between 2020 and 2023 in Italy. Multivariable logistic regression was used to identify independent risk factors for complications. We enrolled 293

Therapy of Pyothorax in Cats via Small-Bore Thoracostomy Tube in Terms of Efficacy, Complications and Outcomes.

First-line therapy for cats with pyothorax consists of intravenous antibiotics, drainage of the septic pleural effusion and closed-chest lavage. Large-bore thoracostomy tubes are traditionally used for drainage, but case series indicate a comparable efficacy using small-bore tubes. In this retrospective study, we describe a new technique of sheath-guided small-bore (6 F) thoracostomy tubes in cats with pyothorax and evaluate their efficacy and complications. Additionally, we compare outcomes between two treatment groups. Placement and use of the small-bore thoracostomy tubes described here has

Limitation of tube thoracostomy in treating pneumothorax in COVID-19 infected patients. A retrospective cohort study.

BACKGROUND: COVID19 infection is caused by the highly contagious SARS-CoV-2(Severe acute respiratory syndrome coronavirus 2). The first outbreak of this infection was in Wuhan, China in December 2019. Since then, it has spread rapidly across the world, with more than 100000 new cases each day. Among those infected with SARS-COV-2 up to 20% develop severe disease requiring hospitalization. Among those who are hospitalized, one quarter will need ICU admission. Admission to the ICU is due to respiratory failure or pneumonia. The pneumonia associated with COVID19 infection may lead to respiratory

Massive Necrotizing Fasciitis of the Chest Wall: A Very Rare Case Report of a Closed Thoracostomy Complication.

We present a case study of necrotizing fasciitis (NF), a very rare but dangerous complication of chest tube management. A 69-year-old man with shortness of breath underwent thoracostomy for chest tube placement and drainage with antibiotic treatment, followed by a computed tomography scan. He was diagnosed with thoracic empyema. Initially, a non-cardiovascular and thoracic surgeon managed the drainage, but the management was inappropriate. The patient developed NF at the tube site on the chest wall, requiring emergency fasciotomy and extensive surgical debridement. He was discharged without an

Emergent needle thoracostomy in prehospital trauma patients: a review of procedural execution through computed tomography scans.

BACKGROUND: Traumatic tension pneumothoraces (TPT) are among the most serious causes of death in traumatic injuries, requiring immediate treatment with a needle thoracostomy (NT). Improperly placed NT insertion into the pleural cavity may fail to treat a life-threatening TPT. This study aimed to assess the accuracy of prehospital NT placements by paramedics in adult trauma patients. METHODS: A retrospective chart review was performed on 84 consecutive trauma patients who had received NT by prehospital personnel. The primary outcome was the accuracy of NT placement by prehospital personnel. Co

Chest X-Ray evaluation using GPT for tube thoracostomy or conservative care in non-tension spontaneous pneumothorax.

BACKGROUND/OBJECTIVE: Spontaneous pneumothorax (SP) commonly presents to the emergency department (ED), and clinicians must rapidly decide between conservative care and tube thoracostomy. Most artificial intelligence tools focus on pneumothorax detection rather than quantitative size estimation and actionable management recommendations. We aimed to evaluate whether a vision-enabled generative pre-trained transformer (GPT) model could estimate apical SP depth on chest radiographs (CXRs) and predict initial ED management. METHODS: We conducted a single-center retrospective observational study o

Efficacy of slow negative pleural suction in thoracic trauma patients undergoing tube thoracostomy-A randomised clinical trial.

INTRODUCTION: Thoracic injuries are prevalent in polytrauma patients, with road traffic accidents being a major cause. In India alone, over 400,000 people were injured in such accidents in 2022. Rib fractures, haemothorax, and pneumothorax are common chest injuries, often managed with tube thoracostomy. While standard procedures for chest tube placement are established, consensus on post-insertion management, particularly regarding negative pleural suction, is lacking. Research on this topic mostly pertains to planned thoracotomies rather than trauma cases. This study seeks to compare outcomes

Comparison of Needle Decompression to Simple (Finger) Thoracostomy in Non-Perfused Cadaveric Models with Theoretical Tension Pneumothorax.

OBJECTIVES: Tension pneumothorax is treated by performing a needle decompression or simple thoracostomy. Failures with needle decompression include incorrect identification of landmarks, body habitus, inserting the needle too deep or not deep enough. Recently, the use of simple thoracostomy has been increasing in frequency by both air medical and ground-based emergency medical services (EMS) systems. The objective of this study is to compare the effectiveness of needle decompression to simple thoracostomy in a theoretical tension pneumothorax created in a cadaveric model. METHODS: We used a p

To Watch Before or Listen While Doing? A Randomized Pilot of Video-Modelling versus Telementored Tube Thoracostomy.

BACKGROUND: New care paradigms are required to enable remote life-saving interventions (RLSIs) in extreme environments such as disaster settings. Informatics may assist through just-in-time expert remote-telementoring (RTM) or video-modelling (VM). Currently, RTM relies on real-time communication that may not be reliable in some locations, especially if communications fail. Neither technique has been extensively developed however, and both may be required to be performed by inexperienced providers to save lives. A pilot comparison was thus conducted. METHODS: Procedure-naïve Search-and-Rescue
