CONCLUSIONS: Percutaneous mechanical thrombectomy seems to be a rapid and effective treatment of acute superior mesenteric artery embolism in median portion of its stem in absence of bowel necrosis. Follow-up of our patients showed excellent short- and long-term results.

Two patients with acute complete carotid bulb occlusion were treated using intra-arterial aspiration thrombectomy a mean of 3 hours after stroke onset. A slightly angulated 8-F guiding catheter designed for general interventional use was employed to aspirate the thrombus in the carotid artery. Manual aspiration through a 50-mL syringe resulted in effective removal of the thrombus followed by good patency of the internal carotid artery. Aspiration thrombectomy is a simple, feasible and effective rescue procedure for patients with acute complete carotid bulb occlusion.

During the past two years, in cases of infrapopliteal embolic occlusion not retrievable through the femoral approach, we have used distal tibial thrombectomy in preference to a direct approach via the popliteal artery. Distal tibial thrombectomy was done in seven extremities in six patients with excellent results. Only one extremity required subsequent amputation. There were no mortalities. This represents a limb salvage rate of 85%.
