Ocular hypotony after trabeculectomy with mitomycin C.

Because mitomycin C reduces the resistance to aqueous outflow normally provided by postoperative subconjunctival fibrosis, we would expect to see more frequent hypotony after trabeculectomy with mitomycin C than after standard trabeculectomy. To evaluate the incidence of hypotony in trabeculectomy with intraoperative mitomycin C use, we performed a retrospective analysis on 52 eyes of 48 patients who underwent trabeculectomy with mitomycin C. Mitomycin C concentration was 0.4 mg/ml in all eyes, and treatment time ranged from 3 1/2 to seven minutes. We defined hypotony as intraocular pressure l…

Initial 5-fluorouracil trabeculectomy in young patients.

The effectiveness of initial trabeculectomy with adjunctive 5-fluorouracil (5-FU) for uncomplicated glaucoma in patients age 40 years or younger at the time of surgery was evaluated retrospectively in a consecutive series of 20 eyes of 20 patients. Fifteen patients had juvenile primary open-angle glaucoma, 4 had pigmentary glaucoma, and 1 had glaucoma associated with angle recession. Mean patient age was 26.8 +/- 9.7 years (range, 13 to 40 years) at the time of surgery. The mean total 5-FU dose administered was 27.8 +/- 8.8 mg (range, 15 to 45 mg). The intraocular pressure (IOP) decreased from…

Transconjunctival Nd:YAG laser revision of failing trabeculectomy.

We used a Q-switched Nd:YAG laser delivered transconjunctivally to restore filtration to a failing trabeculectomy of a 51-year-old black woman with primary open-angle glaucoma and uncontrolled intraocular pressure (IOP) on maximally tolerated anti-glaucoma medical therapy. Initially, the Nd:YAG laser was directed via a goniolens at the interface of the trabeculectomy flap and scleral bed. This attempt to restore filtration was unsuccessful. In two separate treatment sessions the Nd:YAG laser was directed transconjunctivally onto the visible trabeculectomy flap margins, followed by focusing the…

[Filtering trepano-trabeculectomy with sponge-like scar].

Filtering trepanotrabeculectomy (TTE) is a modification of the operation after J. Fronimopoulos. A triangular scleral flap is created which is approximately one-half as thick as the sclera. Trepanation is performed with a 2 mm trephine. The scleral edge of the trepanation opening is heat-cauterized. Filtering TTE was performed on 36 eyes - 17 with glaucoma simplex, 16 with congestive glaucoma, and 3 with secondary glaucoma. Fifty percent of the eyes thus treated had narrow chamber angles. Regulation of IOP was achieved in 96% of the cases. Intraoperative and postoperative complications were ra…

Sutureless Scleral Tunnel Trabeculectomy: Evaluation of Feasibility and Effectiveness versus Conventional Trabeculectomy for Management of Primary Open Angle Glaucoma.

PURPOSE: To evaluate the intraocular pressure-lowering effect of sutureless scleral tunnel trabeculectomy with Mitomycin-C (MMC) compared to the standard trabeculectomy with MMC in patients with primary open angle glaucoma (POAG). METHODS: This was a masked, randomized, controlled comparison trial involving 34 eyes of 34 patients with bilateral POAG. Patients were randomized to receive sutureless sclera flap trabeculectomy with MMC or conventional trabeculectomy with MMC. The main outcome for comparison was the IOP-lowering effect of both procedures. Complete surgical success was considered i…

[Comparison of the efficacy of trabeculectomy combined with Schlemm canal incision versus trabeculectomy alone in the treatment of primary open angle glaucoma].

OBJECTIVES: Glaucoma is the leading cause of irreversible blindness worldwide, with primary open-angle glaucoma (POAG) being the most common type. Trabeculectomy is currently the primary surgical treatment for POAG; however, its long-term efficacy remains suboptimal. This study aims to compare the therapeutic efficacy and safety of trabeculectomy combined with Schlemm canal incision (TCSCI) with trabeculectomy alone in order to provide new insights for POAG management. METHODS: In this retrospective case-control study, 47 patients (47 eyes) diagnosed with POAG at the Ophthalmology Center of X…

Bleb Plication for a Leaking Bleb After Trabeculectomy.

PURPOSE: The purpose of this work was to report a new method for the repair of ischemic bleb leaks. Bleb leakage is a serious complication of glaucoma filtering surgery with mitomycin C. Many surgical methods have been proposed to seal the leakage from a bleb; however, this novel plication method is effective, relatively easy to perform, and safe. METHODS: We describe the case reports of 2 patients with leakage from a bleb who were treated with the new method. The conjunctiva was lifted away from the sclera as extensively as possible around the ischemic conjunctiva toward the fornix using a b

The use of anterior segment imaging after trabeculectomy.

Evaluation of patients after trabeculectomy can be aided by the use of anterior segment imaging. Two commonly used forms of imaging are ultrasound biomicroscopy (UBM) and optical coherence tomography (OCT). Both modalities can be used to assess bleb morphology and how it affects treatment outcome, measure anterior chamber biometry, identify the presence of supraciliochoroidal fluid, cyclodialysis clefts and malignant glaucoma, and evaluate treatment modalities after trabeculectomy such as in laser suture lysis and needling. OCT, with its finer resolution, is able to view the bleb wall anatomy …
