Epidemiology and management of trauma patients in a Greek multispecialty hospital in the absence of a dedicated trauma center.

PURPOSE: In the absence of dedicated trauma centers, surgical emergency departments in hospitals assigned as trauma centers accept a huge load of trauma patients. In this audit, we aim to document and assess the epidemiologic data of trauma patients and their injuries in order to give a picture of the impact of trauma in the workload of a surgical department in the Greek healthcare system. METHODS: During a period of 2 years, we managed 6,041 trauma patients in the accident and emergency (A&E) department based on the Advanced Trauma Life Support (ATLS) protocols. We retrospectively reviewed t…

Demographic and injury trends for car crash casualties hospitalized in Level I Trauma centers over two decades: data from the National Trauma Registry.

BACKGROUND: During the past two decades, there have been many changes in automotive and medical technologies, road infrastructure, trauma systems, and demographic changes which may have influenced injury outcomes. The aim of this study was to examine injury trends among traffic casualties, specifically private car occupants, hospitalized in Level I Trauma Centers (TC). METHODS: A retrospective cohort study was performed based on data from the Israel National Trauma Registry. The data included occupants of private cars hospitalized in all six Level I TC due to a traffic collision related injur…

Early video-assisted thoracoscopic surgery (VATS) for non-emergent thoracic trauma remains underutilized in trauma accredited centers despite evidence of improved patient outcomes.

PURPOSE: Thoracic injury is a major contributor to morbidity in trauma patients. There is limited data regarding practice patterns of video-assisted thoracoscopic surgery (VATS) across trauma-accredited hospitals in the United States. We hypothesized that early VATS remains underutilized affecting patient outcomes. METHOD: We evaluated a cohort of patients who underwent non-urgent thoracic surgical intervention for trauma from the ACS-TQIP database in 2017 excluding patients who were discharged within 48-h or died within 72-h. We selected patients who underwent partial lung resection and deco

The confusion continues: results from an American Association for the Surgery of Trauma survey on massive transfusion practices among United States trauma centers.

BACKGROUND: Massive transfusion practices have undergone several recent developments. We sought to examine institutional practices guiding hemostatic resuscitation in the setting of massive hemorrhage. STUDY DESIGN AND METHODS: A 37-question online survey was sent to American Association for the Surgery of Trauma members. RESULTS: A total of 191 surgeons from 125 institutions completed the survey. Level I and II centers composed 70 and 18% of responding sites, respectively. A total of 123 institutions have a massive transfusion protocol (MTP); 54% report having an MTP for less than 5 years.

Encephalopathy and death in infants with abusive head trauma is due to hypoxic-ischemic injury following local brain trauma to vital brainstem centers.

BACKGROUND: Infants with abusive head trauma (AHT) have diffuse brain damage with potentially fatal brain swelling. The pathogenesis of the brain damage remains unclear. We hypothesize that brain damage in AHT is due to hypoxic-ischemic injury with hypoxic-ischemic encephalopathy (HIE) rather than primary traumatic brain injury (TBI) with traumatic diffuse axonal injury (tDAI). METHODS: We studied brain tissue of AHT victims. Primary outcome measure was the presence of primary traumatic versus hypoxic-ischemic brain injury. The diagnosis of tDAI followed a standardized semiquantitative diagno

Comparable outcomes for non-operative intracranial hemorrhage at Level III vs Level I/II trauma centers.

INTRODUCTION: Most patients with isolated, closed blunt intracranial hemorrhage (ICH) do not require neurosurgical intervention. However, outcome data for those managed entirely at Level III trauma centers remain incompletely characterized. METHODS: Using the 2017-2020 Trauma Quality Improvement Program (TQIP), we compared clinical outcomes for non-operative ICH patients treated at Level I/II versus Level III trauma centers. The primary endpoint was composite in-hospital adverse event counts per patient, modeled using negative binomial regression and reported as adjusted incidence rate ratios…

Safe local management of mild traumatic brain injury: Reducing unnecessary transfers to higher-level trauma centers.

BACKGROUND: Patients with mild traumatic brain injury and traumatic intracranial hemorrhage are frequently transferred to higher-level trauma centers despite low rates of neurologic deterioration and operative intervention. Standardized brain injury algorithms prioritize safety but often escalate care for patients with small, nonoperative hemorrhages. In trauma systems with level III and IV centers lacking on-site neurosurgical coverage, identifying patients who can be safely managed locally remains challenging. METHODS: We performed a retrospective cohort study of adult patients treated betw…

Understanding the outliers: A qualitative analysis of leaders' perspectives on performance and safety in trauma centers.

INTRODUCTION: The factors explaining variability in trauma center performance across quality measures are not well understood. We aimed to characterize the influence of social, cultural, and organizational factors on trauma center performance. METHODS: We used an explanatory sequential design to study ACS TQIP-participating level I and II trauma centers that were ranked as high- and low-performing (first and last decile) across multiple representative quality indicators. The Chief Safety Officers, Directors of Quality and Safety, Trauma Medical Directors, and Trauma Program Managers within th…

Analyzing spinal cord injury patients: Directly admitted compared to individuals transferred into level I trauma centers.

BACKGROUND: Regaining a higher degree of function after spinal cord injury (SCI) requires timely diagnosis and intervention. The American College of Surgeons (ACS) guidelines recommend that SCI should be treated in a designated Level I trauma center. Patients may initially go to lower-level trauma centers, delaying care. This study assesses whether the ACS recommendation is achieved by analyzing interhospital transfer (IHT) of SCI patients. MATERIALS AND METHODS: Retrospective study using Pennsylvania Trauma Systems Foundation registry (2010 and 2020) to determine the trend of IHT of SCI pati…

The double 90 rule: A new strategy for resuscitation in non-academic level II trauma centers.

BACKGROUND: Efficient resuscitation after trauma and shorter time to definitive hemorrhage control help improve trauma outcomes. We aimed to improve the speed and efficiency of resuscitation for critically ill trauma patients in the emergency department by involving interventional radiology and a second surgeon. STUDY DESIGN: In 2017 our community, non-academic level II trauma center implemented the Double 90 rule-for trauma patients with 2 confirmed systolic blood pressures <90 mm Hg-which involves a second activation including the interventional radiology team, backup trauma surgeon, and op

Early rehabilitation in trauma centers: have speech-language pathology services progressed? A 3-year follow-up.

Trauma centers around the United States were surveyed to determine the extent and availability of early, aggressive, and appropriate speech rehabilitation services offered to trauma victims. Although significant increases were noted in specialty programs, augmentative systems usage, and follow-up services, there remains need for continued development in many aspects of early rehabilitation programs. As sophisticated medical management continues to salvage lives of America's prime resource, its young people, an added commitment is needed from all rehabilitation professionals to provide the righ…

Success rate variation between trauma video review and chart review of trauma intubations at a Level 1 trauma center.

BACKGROUND: First pass success (FPS) is an important quality metric for endotracheal intubations. FPS rates for trauma patients in the emergency department (ED) have been reported to be between 80% and 90% and are often based on self-reported data. We evaluated the number of intubation attempts seen on trauma video review (TVR) and compared them against charted attempts. METHODS: We used TVR to review a consecutive cohort of adult trauma intubations in the ED at a level 1 trauma center over 10 months. Four reviewing physicians with specialization in emergency medicine participated in video an

Typologies of hurricane trauma exposure: a person centered approach.

PURPOSE: Hurricane María (2017) and the subsequent large-scale migration of Puerto Ricans serve as a stark example of how extreme weather events, population displacement, and equity concerns impact the health of vulnerable populations. The aim of the present study is to examine the classes of hurricane-related experiences among Puerto Rican "María migrants" and to assess the relationship between different types of hurricane exposure and posttraumatic stress. METHODS: The present study was conducted with 319 Puerto Rican Hurricane María migrants on the U.S. mainland. Using a person-centered ap

Family-Centered Care for Children with Abusive Head Trauma.

Family adaptation to severe neurologic impairment often begins in hospital settings, where the medical care team begins to orient caregivers to alterations in the child's neurologic status. For children with abusive head trauma, there are added social dimensions that affect family adaptation throughout the inpatient stay and beyond. Principles of family-centered care are an important cornerstone of pediatric practice, yet family members whose children have severe neurologic impairment may struggle to adapt in outpatient and multidisciplinary settings, where they must assume a more active leade
