Estimating effects of targeted public health interventions using the interventional disparity indirect effect among the exposed.

Disadvantaged groups are often defined by characteristics such as income or ethnicity. Reducing health disparities by directly manipulating such exposures may be infeasible. Instead, interventions can target mediators between these exposures and health outcomes. Indirect effects estimated using mediation analysis, interventional effects or the interventional disparity measure can quantify the expected impact of such disparity-reducing interventions. They capture the impact of changing the mediator distribution evaluated among the total population. This means keeping individuals in their exposu

Tree-based exploratory identification of predictive biomarkers in non-randomized data.

The idea of "stratified medicine" is an important driver of methodological research on the identification of predictive biomarkers. Most methods proposed so far for this purpose have been developed for the use on randomized data only. However, often data from clinical registries or non-randomized studies are the only available data source. For such data, methods for estimation of the average treatment effect are well established. Research on confounder adjustment when investigating the heterogeneity of treatment effects and the variables responsible for this is usually restricted to regression

Representation of racial and ethnic minorities in U.S. depression trials comparing psychotherapy vs. controls: A meta-analysis using state population benchmarks.

INTRODUCTION: Psychotherapies for depression are effective. To generalize to the diverse U.S. population, clinical trials must adequately represent racial/ethnic minority groups. Prior reviews of diversity of clinical trial participants are limited by incomplete reporting of race/ethnicity, use of national rather than state-level comparators, and pooling practices where a few diverse trials skew estimates. METHODS: We evaluated racial/ethnic representation in U.S. randomized trials comparing psychotherapy with an inactive control in depressed adults using meta-analysis with state-specific pop

Smoking and cluster headache: clinical, gender, and treatment insights from a cross-sectional cluster analysis.

BACKGROUND: The association between smoking and cluster headache (CH) has been consistently documented, with high rates of tobacco exposure observed among CH patients and several studies reporting links between smoking status and differences in disease onset, clinical features, and treatment response. However, the biological basis of this association remains uncertain, and no causal relationship has been established. To better characterize this interaction, the present study investigates the relationship between smoking and CH using three complementary analytical approaches. METHODS: In this m

Impact of continuing renin-angiotensin-aldosterone system inhibitors before surgery on intraoperative hypotensive events: a secondary analysis of the STOP-or-NOT Trial.

BACKGROUND: The multicentre STOP-or-NOT trial has shown that continuation of renin-angiotensin-aldosterone inhibitors (RAASis) before major noncardiac surgery did not increase the rate of postoperative complications, but led to a higher incidence of intraoperative hypotension. However, the risk of intraoperative hypotension could vary significantly between patients. We evaluated whether there is heterogeneity in the risk of intraoperative hypotension among patients who continue vs those who discontinue RAASi therapy before major surgery. METHODS: We conducted a secondary analysis of the STOP-

Heterogeneous causal mediation analysis using Bayesian additive regression trees.

Causal mediation analysis provides insights into the mechanisms through which treatments affect outcomes. While mediation effects often vary across individuals, most existing methods focus solely on population-average effects, overlooking individual-level heterogeneity. To address this limitation, we propose a Bayesian regression tree ensemble method that flexibly models nonlinear relationships and captures treatment-by-mediator interactions in the mediation process. Using hierarchical posterior sampling, our approach provides credible intervals with nominal coverage rates for inferring hetero

Differential Proteomic Profiling of Responders and Non-responders to Direct-Acting Antivirals Treatment in Chronic Hepatitis C Virus Infection.

Hepatitis C Virus (HCV), particularly genotype 3 (GT-3), is highly prevalent in India and is associated with faster progression to cirrhosis, hepatocellular carcinoma, and higher treatment failure rates. Although Direct-Acting Antivirals (DAAs) have revolutionized HCV therapy, 5-10% of patients fail to achieve sustained virological response (SVR). This proteomic study aimed to identify changes in the proteomic profile before and after treatment of both responders and non-responders to HCV treatment. Paired plasma samples from HCV GT-3 infected patients were collected before and 12 weeks after

Genetic Influences on Disease-Modifying Therapy Response in Multiple Sclerosis: Current Insights and Future Directions.

Multiple sclerosis (MS) is a clinically and biologically heterogeneous, immune-mediated disease of the central nervous system, with substantial interindividual variability in disease course and response to disease-modifying therapies (DMTs). Over the past three decades, the MS therapeutic landscape has expanded considerably; however, treatment selection and switching remain guided primarily by clinical phenotype and imaging findings rather than molecular predictors of response. Despite extensive clinical trial evidence, prospectively identifying responders and non-responders to specific DMTs r

Toward Personalized Medicine in Type 1 Diabetes: Understanding How Patient Heterogeneity Influences Therapeutic Efficacy.

Pharmacologic interventions for type 1 diabetes (T1D) have advanced significantly in recent years with the advent of the first FDA approved therapy teplizumab for delaying symptomatic disease onset in 2022. Despite this progress, major hurdles remain in moving toward personalized medicine approaches for T1D. Here, we highlight the examples of heterogeneity in therapeutic responses to recent beta cell and immune interventions and what these studies can teach us about how to tailor therapy for maximizing benefit to patients at risk of or living with T1D. We examine the differences between propos

Hypertriglyceridemic pancreatitis subphenotypes and differential responses to low-molecular-weight heparin: analyses from a cohort and a randomized controlled trial.

BACKGROUND/OBJECTIVES: Hypertriglyceridemic pancreatitis (HTG-AP) is a heterogeneous disease, potentially diluting the apparent benefit of specific interventions. Characterizing this heterogeneity may help identify subgroups that benefit from particular treatments. We aimed to identify HTG-AP subphenotypes and explore differential responses to low-molecular-weight heparin (LMWH) across subphenotypes. METHODS: HTG-AP subphenotypes were identified using latent class analysis based on admission clinical and laboratory variables from a prospective cohort (n = 1483). Associations between subphenot

Clinical subphenotypes of sepsis based on mixed continuous and categorical data and differences in treatment effects: a cluster analysis of multicenter observational studies.

BACKGROUND: Sepsis is a heterogeneous syndrome; therefore, identifying subphenotypes is essential to determine the optimal treatment for each patient. Previous studies have mainly analyzed continuous variables and focused on the treatment effects in patient cohorts receiving a single intervention. In this study, we explored subphenotypes using mixed continuous and categorical data and examined multiple treatment effects for each subphenotype. METHODS: We used Fifty-two admission variables from two multicenter registries—the Tohoku Sepsis Registry and Focused Outcomes Research in Emergency Care

Early divergence of treatment response trajectories to adalimumab or its biosimilar in active ankylosing spondylitis: consensus clustering analysis of a randomized controlled trial.

BACKGROUND: Patients with active ankylosing spondylitis (AS) exhibit substantial heterogeneity in their clinical responses to tumor necrosis factor alpha inhibitors (TNFi). Consensus clustering, an unsupervised cluster discovery method, may identify AS subgroups with more homogeneous treatment response patterns to adalimumab (ADA), a widely prescribed TNFi. METHODS: We performed longitudinal consensus clustering based on 8 repeated measurements of 10 core response variables in 438 patients with active AS enrolled in a 24-week phase III randomized controlled trial of ADA or its biosimilar, IBI

Moderating Factors for the Effectiveness of Brief Psychological Intervention for People With Probable Personality Disorder: Secondary Analysis of Data From the SPS Trial.

Brief interventions have been proposed as a means of increasing access to psychological treatment for people with personality disorder. However, evidence of their effectiveness is limited to date. A recent large-scale trial of Structured Psychological Support (SPS) for people with probable personality disorder showed little evidence of patient benefit. However, questions remain about whether shorter term psychological interventions may be suitable for some patients. We therefore conducted a secondary analysis of trial data to examine whether baseline level of personality dysfunction, duration

The Social Ecology of Psychotherapy: Community-Level Influences on Adolescent Mental Health Treatment.

OBJECTIVE: Effective treatment for depression and anxiety in adolescence is a public health concern. Although psychotherapies for depression and anxiety exist, youth psychotherapies are less effective when delivered in community settings compared to research settings. A social-ecological model, which considers individual-, interpersonal-, and community-level influences on wellbeing, posits that understanding multi-level factors can help us understand this drop in treatment effectiveness. This study employs a socioecological model to examine the influence of neighborhood factors on depression a

MRI-Derived Intra- and Peritumoral Heterogeneity for Predicting Response to Transarterial Chemoembolization in Patients with Hepatocellular Carcinoma.

RATIONALE AND OBJECTIVES: The compromised therapeutic efficacy of transarterial chemoembolization (TACE) in hepatocellular carcinoma (HCC) is closely associated with tumor heterogeneity. Magnetic resonance imaging (MRI)-derived quantification of intratumoral and peritumoral heterogeneity (ITH and PTH) may provide novel, noninvasive biomarkers. To develop and validate an MRI-based strategy leveraging quantitative ITH and PTH for pretreatment identification of patients with HCC achieving objective response to TACE monotherapy. Systemic biomarkers of treatment response to methotrexate in people with painful knee osteoarthritis: A biological substudy of the PROMOTE randomised controlled clinical trial.

OBJECTIVE: Stratification of therapeutic responses may help identify efficacious therapies for osteoarthritis (OA). In the PROMOTE randomised trial, participants with elevated baseline high-sensitivity C-reactive protein (hs-CRP) showed greater pain reduction after methotrexate treatment. We set out to interrogate a broader panel of serum/plasma inflammatory response markers relevant to methotrexate actions as potential biomarkers of therapeutic effect. Our objectives were to: (i) characterize changes in these systemic markers during methotrexate treatment; determine whether (ii) baseline leve

Clinical effectiveness of efgartigimod in a broad population of patients with generalized myasthenia gravis: subgroup analyses from a randomized, double‑blind, placebo‑controlled, phase 3 trial (ADAPT).

BACKGROUND: The ADAPT phase 3 trial (NCT03669588) showed efgartigimod was well tolerated and efficacious in acetylcholine receptor antibody-positive (AChR-Ab +) patients with generalized myasthenia gravis (gMG). This analysis utilized data from the ADAPT trial to investigate the efficacy and safety of efgartigimod in different patient subgroups. METHODS: ADAPT included a broad population of AChR-Ab + participants who received a stable dose of ≥ 1 (any) treatment for gMG and were randomized 1:1 to efgartigimod (10 mg/kg) or placebo (administered as four once-weekly infusions per cycle) for 26

Discordant signals: How outcome measurement shapes interpretation of ketamine response in treatment-resistant depression.

BACKGROUND: Depression rating scales capture heterogeneous symptom domains, raising questions about the comparability of treatment response across instruments. This issue is particularly relevant for rapid-acting interventions such as ketamine. METHODS: We conducted a secondary analysis of a naturalistic cohort (N = 71) of patients with treatment-resistant depression treated with intravenous ketamine. Changes in depressive symptoms were assessed using the Montgomery-Åsberg Depression Rating Scale (MADRS), Beck Depression Inventory-II (BDI-II), and Clinical Global Impression-Severity (CGI-S).

Patient treatment preference as a predictor of clinical outcomes in subjects with treatment-resistant depression: a comparative effectiveness research trial for antidepressant incomplete and nonresponders with treatment-resistant depression (ASCERTAIN-TRD).

Major depressive disorder (MDD) is a severe and debilitating illness. Despite the available treatments, clinical outcomes remain suboptimal, and hence, it is crucial to identify predictive factors for response. This is a secondary analysis investigating the relationship between treatment preference and response to treatment in the antidepressant incomplete and non-responders with treatment resistant depression (ASCERTAIN-TRD) trial (NCT02977299) comparing three treatment arms [aripiprazole augmentation, repetitive transcranial magnetic stimulation (rTMS) augmentation, switching to venlafaxine
