Reliability of control skin tests with common antigens in children undergoing tuberculin skin test.

The aim of this study was to determine the reliability of skin tests with common antigens as a measure of delayed-type hypersensitivity response in children undergoing tuberculin skin test (TST). Children from developing countries underwent TST as well as skin tests with diphtheria-tetanus vaccine (DT) and candida antigens prior to heart surgery. We included 52 children (mean age: 7.16 years). Forty-two (81%) and 20 of the 52 patients (38%) reacted to the DT and candida skin tests, respectively (P < 0.001 for the difference in response to these antigens). There was no difference in response to

The impact of recent tuberculin skin testing on paratuberculosis antibody responses.

There is a recognized interference between the single intradermal cervical comparative tuberculin (SICCT) test used for bovine tuberculosis surveillance and the paratuberculosis milk ELISA. Current UK guidance recommends a minimum interval of 42-60 d between a SICCT test and a paratuberculosis milk ELISA. However, recent research suggested that the post-SICCT antibody response may be greater in cows at high risk of Mycobacterium avium ssp. paratuberculosis (MAP) shedding (i.e, with an increased likelihood of shedding MAP in feces), leading to advisory guidance that high-prevalence herds could

Tuberculin skin testing and T-SPOT.TB in internationally adopted children.

BACKGROUND: Diagnosis of latent tuberculosis infection is a problem in children because of lack of a diagnostic standard and potential impact of previous Bacille Calmette-Guérin vaccination and exposure to environmental mycobacteria. Effectiveness and usefulness of interferon-gamma release assays in infants and younger children have yet to be clearly demonstrated. METHODS: Prospective cohort study including 109 children (4 months to 16 years) seen in an international adoption clinic at Nationwide Children's Hospital, Columbus, OH. Children were adopted from 14 countries, mostly (72.5%) from C

[Extent of tuberculin skin testing spot in bacteriologically verified tuberculosis].

BACKGROUND: Tuberculin skin test was evaluated in patients with bacteriologically verified tuberculosis notified in the Czech Republic during 2003 and 2004. METHODS AND RESULTS: Out of 1172 patients with bacteriologically verified tuberculosis altogether 28.8% were tuberculin-negative. The average value of tuberculin reaction in this cohort was 11.6 mm. Among 30-year-old patients with bacteriologically verified tuberculosis the number of tuberculin-negative individuals was statistically lower (14 %) in comparison with 70-year-old and older patients, where the prevalence of tuberculin-negative

Two-step tuberculin testing of passengers and crew on a commercial airplane.

OBJECTIVES: We investigated the risk of tuberculosis transmission from a person with highly infectious pulmonary tuberculosis to fellow passengers and crew members on a 14-hour commercial flight. The 2-step tuberculin testing was used to minimize the effects of the booster phenomenon. METHODS: Passengers and flight crew members identified from airline records were contacted by letter, telephone, or both to notify them of their potential exposure to Mycobacterium tuberculosis. The subjects were advised to undergo Mantoux tuberculin skin testing within the required time period to assess a conve…

Tuberculin screening: cost-effectiveness analysis of various testing schedules.

Because there is no tuberculin screening schedule currently recommended for adults, we used a Markov process in a cost-effectiveness analysis to determine an optimal strategy. We simulated the prognosis of a cohort of black 20-year-olds to evaluate the effects of various screening schedules with intradermal tuberculin and administration of isoniazid prophylaxis to those with positive results. The schedule with the lowest cost-effectiveness ratio is a single screening at 50 years of age, which costs $41,672 per quality-adjusted life year (QALY) gained. The cost-effectiveness ratio is nearly the

[The tuberculin-transamidinase test--a new method for the diagnosis of renal tuberculosis].

The possible use of the provocative standard test with subcutaneous administration of tuberculin combined with determination of the activity of transamidinase, a renal specific enzyme, in the serum and urine before and 24, 48, 72 and 96 hours after administration of tuberculin was studied for the first time in 63 patients with a purpose of diagnosing nephrophthisis. It was shown that a 2-fold or higher increase in the transaminidase activity in response to subcutaneous administration of 50 TU during the period from 24 to 72 hours could serve as a sign of nephrophthisis.

Prevalence of tuberculin skin test positivity in clinical population in New York City.

The prevalence of latent tuberculosis infection (LTBI) in the various populations of New York City (NYC), a city with a high density of non-US-born persons, is unknown. We examined the prevalence of TST positivity in patients who received a tuberculin skin test (TST) between 1/2002 and 8/2004 at any of 10 NYC health department chest centers. A positive TST was defined as an induration reaction to tuberculin of ≥10 mm. In the study population of 41,022 individuals, prevalence of TST positivity was 24.4% (95%CI = 24.0, 24.8); four times higher among non-US-born persons than US-born (39.5% vs. 8.

Pseudoepidemics of tuberculin skin test conversions in the U.S. Army after recent deployments.

RATIONALE: The tuberculin skin test (TST) has many sources of error. These can lead to predominantly false-positive reactions when used in low-risk populations. The U.S. Army deploys to areas considered at high risk for tuberculosis (TB) infection, but often has limited contact with the local population. OBJECTIVES: We describe the investigation of eight pseudoepidemics of TST conversions in U.S. Army populations, five of which were associated with overseas deployments. METHODS: Outbreak investigations of these pseudoepidemics consisted of several components: evaluation of active and latent

Two-step tuberculin skin test and booster phenomenon prevalence among Brazilian medical students.

SETTING: Five medical schools in three cities in Rio de Janeiro State, Brazil, with different tuberculosis (TB) incidence rates. OBJECTIVE: To evaluate the prevalence of the booster phenomenon and its associated factors in a young universally BCG-vaccinated TB-exposed population. DESIGN: A two-step tuberculin skin test (TST) was performed among undergraduate medical students. Boosting was defined as an induration > or =10 mm in the second TST (TST2), with an increase of at least 6 mm over the first TST (TST1). The association of boosting with independent variables was evaluated using multiva

Comparison of tuberculin skin test response after three modalities of neonatal BCG vaccination.

The aim of this study was to determine the tuberculin skin test (TST) response to three different BCG modalities [0.1 ml Tokyo (n=104), 0.05 ml Pasteur-Mérieux (n=137) and 0.1 ml Pasteur-Mérieux (n=100)] in a cohort of healthy newborns 5 months after vaccination at birth in Santiago, Chile. Among the 341 infants, 91.2% had a response to TST, and the mean+/-SD TST reaction size was significantly larger in infants receiving the Tokyo strain than in those receiving the 0.05 ml or 0.1 ml Pasteur-Mérieux strains (4.4+/-2.0, 3.5+/-1.3 and 3.1+/-1.4 mm, respectively; P<0.0001). The mean+/-SD of the B

[The Tuberculosis Rapid Assay from DiaVita - No Alternative to the Tuberculin Skin Test].

New methods for diagnosing active tuberculosis (TB) and/or latent tuberculosis infection have been offered increasingly during recent years, also in Germany. The Tuberculosis Rapid Assay ('Tuberkulose Schnelltest [TB-ST]') from DiaVita is a serologic test method aiming to detect, within a few minutes, tuberculosis-specific antibodies in whole blood or serum of tuberculosis patients. Due to a lack of data, it is, however, hardly possible to assess this test on a scientific basis. With a relatively high specificity (according to DiaVita over 95 %) but inadequate sensitivity (approximately 60 - 8

Interpretation of the tuberculin skin test in Mycobacterium bovis BCG-vaccinated Singaporean schoolchildren.

Tuberculin skin test (TST) interpretation in Singapore is confounded by universal BCG vaccination at birth and by a revaccination policy for schoolchildren who are tuberculin nonreactors (TST < 10 mm, using 1 TU PPD RT 23) at 12 or 16 yr old, with not more than two BCG vaccinations given to any child. School health records for birth cohorts 1978 to 1984 indicate that 82.8% were revaccinated at age 12 yr. By collation with the national database of tuberculosis (TB) notifications, we examined the risk of TB disease in these cohorts for the 4 yr subsequent to TST reading at ages 12 and 16 yr resp

[HIV infection, tuberculosis and tuberculin test in Switzerland. The Swiss HIV Cohort Study].

BACKGROUND: The usefulness of the tuberculin skin test (TST) for HIV-infected subjects may be questioned because false-negative results are increasingly common with advanced immune deficiency. The objectives of this research were thus to describe the use and the results of TST among patients with HIV infection in Switzerland and to measure the usefulness of isoniazid preventive chemotherapy and the incidence of tuberculosis (TB) relative to TST and CD4 cell count at the time of TST. DESIGN: Prospective multicentric cohort study of HIV infected patients; comparison of patients tested and non-t
