Cycling-Related Compressive Neuropathy of the Deep Ulnar Motor Branch in the Hand: Is Sonography a Valid Tool?

Repetitive vibratory trauma in cyclists is one of the most common causes of ulnar nerve compression at the wrist. This study describes five cases of clinically suspected compression syndrome of the deep terminal branch of the ulnar nerve (DBUN) defined by high-resolution ultrasound (HRUS) with a 18-MHz linear transducer. Reliable HRUS visualization of the DBUN was feasible in all patients. The affected nerves had focally swollen and hypo-echoic changed segments. HRUS is a good imaging tool for the evaluation of a suspicious DBUN neuropathy if no electrodiagnostic testing is available or in pat

The cubital tunnel syndrome caused by the intraneural or extraneural ganglion cysts: Case report and review of the literature.

Cubital tunnel syndrome is the second most common nerve compression syndrome in peripheral nerve compression disease. Although potential ulnar nerve entrapment can occur at multiple points along its course, such as the arcade of struthers, the medial intermuscular septum, the medial epicondyle, the cubital tunnel, and the deep flexor pronator aponeurosis, the most common site of entrapment is the cubital tunnel. However, cubital tunnel syndrome could also be caused by the occupying masses along the course of ulnar nerve, such as intraneural or extraneural ganglia. The cubital tunnel syndrome c

Diabetic Ulnar Compression Neuropathy.

Ulnar compression neuropathy is the second most frequent entrapment neuropathy of the upper extremity and is observed with increased prevalence among diabetes mellitus patients. The metabolic burden of diabetes mellitus, including chronic hyperglycaemia, microvascular dysfunction, and systemic inflammation, promotes nerve susceptibility to mechanical compression. This review summarises current evidence on the association between diabetes mellitus and ulnar compression neuropathy, highlighting epidemiological data, metabolic contributors, clinical manifestations, diagnostic tools, and managemen

Handlebar problems in bicycling.

Handlebar problems are common among serious bicyclists. Compression neuropathy, more commonly ulnar than median, is frequent but seldom produces permanent injury or deficit if promptly recognized and managed by attending to bicycle fit, energy-absorbing gloves and handlebar treatments, and cycling technique. Overuse symptoms may be seen if bicycle design and fit couple with repetitive motion to aggravate wrist tendons or connective tissues of the neck and shoulder. Mechanical and medical management are effective in relieving symptoms and preventing recurrence.

MR features of nerve disorders at the elbow.

MR imaging is a useful method for evaluating nerve disease. It can portray the normal anatomy and identify unsuspected space-occupying masses. Severe intrinsic nerve disease can be depicted. This article outlines the normal anatomy of the three major nerves that traverse the elbow joint: the ulnar nerve, the median nerve, and the radial nerve. Entrapment and compression neuropathies of each nerve are discussed in detail. Finally, the role of MR imaging in delineating each nerve abnormality is examined.

Lipomatous Mass Effect on the Brachial Plexus: A Case Report.

Peripheral nerve compression is a prevalent concern for primary care physicians and hand surgeons, with carpal tunnel syndrome (CTS) and median nerve compression at the wrist being some of the most commonly diagnosed conditions. However, for less common nerve entrapment syndromes, it is crucial for healthcare providers to recognize their symptoms and consider potential underlying issues, particularly those related to the brachial plexus. This case report highlights a 57-year-old male who presented with classic symptoms of left median and ulnar nerve compression in the setting of an enlarging l

Cubital Tunnel Release in a Patient With Parsonage-Turner Syndrome: A Case Report.

Parsonage-Turner syndrome (PTS) is a rare peripheral neuropathy characterized by the sudden onset of shoulder pain followed by muscle weakness and atrophy. This condition primarily affects the brachial plexus and may mimic or coexist with other compressive neuropathies. We present a case of a 39-year-old female with a prior diagnosis of PTS who developed progressive right-sided scapular and elbow pain, accompanied by numbness in the fourth and fifth digits of the right hand. Her symptoms had been ongoing for approximately three months. Physical examination revealed atrophy of the intrinsic mus

SKIN SURFACE TEMPERATURE CHANGES BEFORE AND AFTER MEDIAN NERVE DECOMPRESSION IN CARPAL TUNNEL SYNDROME.

Carpal tunnel syndrome is the most common upper extremity compression neuropathy caused by compression of the median nerve at the wrist. Along with motor and sensory fibers, sympathetic fibers also pass through the median nerve, playing an important role in the regulation of blood flow to the skin which interacts between the body interior and its environment. The aim of this study was to examine the preand postoperative correlation between skin temperature changes in the area innervated by compressed median nerve compared to the skin areas innervated by radial and ulnar nerve and median nerve

Symptomatic epineural ganglion cyst of the ulnar nerve in the cubital tunnel: a case report and brief review of the literature.

An unusual case of pain and weakness in the hand and forearm due to a ganglion cyst of the ulnar nerve at the elbow is presented. The patient was managed initially as a case of cervical disc disease and cervical spondylosis and later as a case of carpal tunnel syndrome at an another institution. Cervical radiography and cervical magnetic resonance imaging scans were inconclusive. Neurosurgical referral revealed tenderness at the right cubital tunnel, weakness of the right hand and forearm muscles, and sensory deficit along the medial border of the forearm and the hand. The diagnosis of ulnar n

Ulnar tunnel syndrome.

As our understanding of the anatomy of the ulnar tunnel has increased, so too has our ability to clinically predict the specific sites of compression in ulnar tunnel syndrome. Anatomic studies have described in detail the course of the ulnar nerve as it passes through the ulnar tunnel and have helped correlate symptoms with anatomic location. Although the most common cause of compression is from a ganglion, other space-occupying lesions, such as tumors, anomalous muscles, or a thrombosed ulnar artery, are important to consider in the initial evaluation of the patient. While conservative manage

Results of the musculofascial lengthening technique for submuscular transposition of the ulnar nerve at the elbow.

BACKGROUND: In the absence of a randomized, prospective study comparing different surgical approaches for decompression of the ulnar nerve at the elbow, the choice of an approach relies on the individual surgeon's training and experience. The present report describes the results of a prospective, long-term evaluation of the musculofascial lengthening technique in a large series of patients. In these patients, the degree of ulnar nerve compression was staged with use of a numerical grading system that included measures of both motor and sensory function. METHODS: From 1985 through 1991, 121 co

Does the ulnar nerve enlarge after surgical transposition?

OBJECTIVES: The purpose of this study was to test the hypothesis that symptomatic transposed ulnar nerves have a larger average cross-sectional area (CSA) than symptomatic in situ ulnar nerves. METHODS: We conducted a retrospective review of the charts and sonograms of 68 patients who had failed ulnar nerve transposition compared to 48 patients with cubital tunnel syndrome who had not undergone surgical management. In addition, postoperative sonograms were compared with preoperative studies when available. Failure was defined as persistence or recurrence of symptoms of ulnar neuropathy postop

A Comprehensive Review of Cubital Tunnel Syndrome.

Cubital Tunnel Syndrome (CuTS) is the compression of the ulnar nerve as it courses through the cubital tunnel near the elbow at the location colloquially referred to as the "funny bone". CuTS is the most commonly diagnosed mononeuropathy after carpal tunnel syndrome. Cubital tunnel syndrome can manifest as numbness, tingling, or pain in the ring/small fingers and dorsoulnar hand. Repetitive pressure, stretching, flexion, or trauma of the elbow joint are known causes of CuTS. Chronic ulnar nerve compression and CuTS, when left untreated, can lead to atrophy of the first dorsal interosseus muscl

Acute handlebar syndrome: Two extremes of a challenging diagnosis.

BackgroundUlnar nerve mononeuropathy diagnosis can be challenging depending on where neural lesion is present. Repetitive trauma during cycling is a rare cause of ulnar neuropathy.Case descriptionWe describe two patients who developed the handlebar syndrome, an ulnar nerve palsy at Guyon's canal after cycling. The first patient had the syndrome after a short-distance ride and she was treated surgically, while the second patient developed the classical syndrome after a long ride and received conservative treatment. Surgical treatment of the first patient led to functional recovery.ConclusionHan
