Neurophysiology of stress urinary incontinence.

Stress urinary incontinence (SUI) involves involuntary leakage of urine in response to abdominal pressure caused by activities such as sneezing and coughing. The condition affects millions of women worldwide, causing physical discomfort as well as social distress and even social isolation. Until recently, SUI was approached by clinicians as a purely anatomic problem requiring behavioral or surgical therapy. Over the past several years, extensive basic and clinical research in the field of neurourology has enhanced our understanding of the complex neural circuitry regulating normal function of …

Medical management of urinary incontinence.

Treatment options for urinary incontinence include behavioral techniques, pharmacologic agents, and surgical procedures. Caregivers use pharmacotherapy heavily because of its availability, immediacy of results, and convenience. However, only pharmacotherapy for urge incontinence has advanced to the level at which several drugs that have undergone rigorous scientific testing using randomized controlled trials have received FDA approval; these are the antimuscarinic and anticholinergic/direct smooth muscle relaxant drugs. However, promising new drugs targeting other receptors are under investiga…

[Urinary stress incontinence in women].

The integral theory of Petros and Ulmsten has profoundly changed our understanding of the female pelvic floor. Anatomic laxity of the vaginal wall caused by pelvic floor defects induced at different damage zones is frequently not only responsible for stress urinary incontinence but also for pollakisuria, urgency, post-void residual and pelvic pain. A number of minimally invasive techniques have been developed to correct these defects. Applying a tension-free polypropylene tape around the mid-urethra has become an established method to correct the anterior ligaments. The infra-coccygeal sacrope…

Urinary incontinence in familial dysautonomia.

The aim of this study was to determine the prevalence of urinary incontinence in women with familial dysautonomia (FD). A telephone survey was conducted on 68 known surviving female FD patients over 13 years of age registered with the Dysautonomia Centers in the USA and Israel. The mean age of the surveyed group was 27.1+/-9.8 years and 99% of the patients were nulliparous. The overall reported prevalence of urinary incontinence was 82% (n=56). Of the patients with incontinence, 59% (n=33) reported stress incontinence, 11% (n=6) reported urge incontinence, and 30% (n=17) reported symptoms of b…

Urinary incontinence: newer pharmacotherapeutic trends.

PURPOSE OF REVIEW: Urinary incontinence is an under-reported epidemic with profound effects on quality of life. With recent pharmacologic advances, the belief that incontinence is an inevitable part of aging should be abandoned. Because many patients are unaware of curative options, they fail to report their symptoms and increase their risk of developing associated comorbid disease. Failure to diagnose and treat this disease in a timely manner increases society's economic burden associated with incontinence. RECENT FINDINGS: Pharmacologic treatment of urge incontinence previously had limited …

Urinary incontinence after radical prostatectomy.

Despite improvements in knowledge and technique, a growing number of patients experience incontinence after radical prostatectomy. This may be the result of damage to sphincteric structures, bladder dysfunction, an obstructive stricture, or some combination of these. After an appropriate interval to allow for improvement, the patient should undergo a thorough evaluation to assess the contribution of the various causes and should then be managed using a sequential treatment approach. Following restoration of adequate emptying, bladder dysfunction should be controlled first, if present, and pers

[Urinary incontinence--taboo during postmenopause].

Urinary incontinence affects 30-60% of all women in the postmenopause. More and more women want to remove the taboo attached to incontinence, because they do not accept the reduction in quality of life, social and domestic activities. Detrusor instability (urge incontinence) and/or urethral sphincter incompetence (genuine stress incontinence) are the principal mechanisms, which lead to involuntary urine loss in older women. It is quite evident that additional factors as hypoestrogenism and urogenital atrophy influence urinary incontinence. Since in the postmenopause urge incontinence combined …

Reconstructive surgery for urinary incontinence.

The clinical urologist often is faced with the referral of a patient with urinary incontinence refractory to conservative measures. Given the broad spectrum of causes of urinary incontinence, the clinician must base evaluation and therapeutic management on current principles of urinary tract pathophysiology. This article organizes the pertinent diagnostic considerations that must be addressed in guiding the clinician to the appropriate surgical treatment option.

Drug treatment of urinary incontinence.

Urinary incontinence is usually classified into four types: stress, urge, reflex and overflow incontinence. The first three types of incontinence are due to urine storage dysfunction and the latter is due to urine emptying dysfunction. To improve urine storage dysfunction treatment should be aimed at decreasing detrusor overactivity, increasing bladder capacity and/or increasing outlet resistance, whereas to improve emptying dysfunction, increasing detrusor contractility and/or decreasing outlet resistance are required. This review summarizes the various drugs currently in clinical use and new…

Surgical management of urinary incontinence.

Surgery for stress incontinence is selected according to specific problems in each patient. Patients with ISD can be identified and selected for sling surgery or collagen injection therapy. Other patients may be managed successfully by retropubic urethropexy, either open or laparoscopic. Diagnostic evaluation should be thoughtful and extensive, with good correlation of findings and presenting symptoms to apply therapy in a specific manner to each patient.

[Rehabilitation of female urinary incontinence].

Perineal rehabilitation is an appropriate alternative to surgery in the treatment of urinary female incontinence. The most important factors influencing the success of this technique is the ability of the patient to identify correctly the muscles of the pelvic floor, to strengthen this muscles using exercises, electrical stimulation and biofeedback, to contract voluntarily the pelvic floor musculature during stress or sensation of voiding for having a preventive effect on loss of urine, and also to change, if necessary, the micturitional and drinking customs. Some conditions are required to co

Nonsurgical treatment of urinary incontinence.

Genuine stress urinary incontinence can be treated by surgical or nonsurgical methods. Conservative treatments include pelvic muscle exercises, hormonal and nonhormonal pharmacologic therapy, and functional electrical stimulation with vaginal or anal electrodes. All of these methods improve or cure stress incontinence in a significant proportion of selected women, with less cost and morbidity. Patients with genuine stress incontinence generally should have a trial of conservative therapy before corrective surgery is offered. Behavioral and pharmacologic methods, alone and in combination, are u…

Surgery for stress urinary incontinence.

The literature on the surgery of stress urinary incontinence from November 1989 to December 1990 is critically reviewed. This period was one of consolidation and assessment with surprisingly few reports of the management of complications and few reports of long-term follow-up. Retropubic procedures continue to be the mainstay of treatment in patients in whom a gynecologic abdominal procedure is indicated. For patients who do not require laparotomy, needle suspension procedures are increasingly used. The Raz procedure is frequently used for patients with type III incontinence. Possible complica…

Surgery for stress urinary incontinence.

Stress urinary incontinence is a common problem, with various degrees of disability. Its etiology may be defective external or internal urethral sphincter mechanisms. Evaluation of the patient with symptoms of stress incontinence requires objective testing for diagnostic accuracy. Patient management may be individualized to conservative measures or to one of the various surgical approaches, which produce success rates between 80% and 95%. Retropubic suspension or long needle procedures should be considered for patients with stress incontinence and evidence of bladder neck detachment. Sling pro…

Treatment of stress urinary incontinence.

This review presents reported cure and improvement rates of stress urinary incontinence in women obtained by different treatment modalities. Apart from the urodynamic findings, histological and histochemical changes of the pelvic floor may be clinically relevant to treatment in the future. Long-term cure and improvement rates achieved by non-surgical treatment (physiotherapy, biofeedback, bladder training, electrostimulation) are commented on. These rates range from 40-60% for physiotherapy and electrostimulation but are considerably less after biofeedback and bladder training. Pharmacotherapy…

Treatment of recurrent urinary incontinence.

This method of elevating the vesical neck with two permanent nylon sutures is effective for correcting primary or recurrent stress urinary incontinence and even total urinary incontinence in over 90% of patients. The cystoscope guides placement of the nylon sutures, and the buttress supports the tissues to achieve precise functional closure of the bladder neck. The technique causes less blood loss and postoperative morbidity than many other operations because open pelvic surgery is avoided. There is less pain, and the patient can be discharged with supra-public urinary drainage on the second o…

Surgical correction of urinary incontinence.

Over the last 6 years, 114 patients have undergone surgery for urinary incontinence. The majority (79%) had neurologic dysfunction of the bladder because of spinal malformation (myelodysplasia, sacral agenesis, or trauma) and the remaining were a mixed group including exstrophy/epispadias, urethral valves, pelvic fractures, etc. The patients were grouped in six categories. Those with lower urethral resistance underwent bladder neck reconstruction with Young-Dees-Leadbetter procedure (five patients) or had placement of an artificial urinary sphincter (27 patients). Those with poor bladder compl…
