LIFT: Lifting the Impacted fetal head; the Fetal Pillow® and Tydeman® Tube trial: protocol for a prospective observational study to evaluate two disimpaction devices.

BACKGROUND: Impacted fetal head (IFH) at full dilatation caesarean section (CS) is an increasingly recognised obstetric emergency associated with significant maternal and neonatal morbidity. Despite its importance, there is limited evidence to guide management, particularly the use of disimpaction devices. METHODS: LIFT is a prospective observational trial comparing the use of two commercially available disimpaction devices-the Fetal Pillow® and the Tydeman® Tube -for the management of IFH at full dilatation caesarean section. DISCUSSION: The LIFT study will compare the performance of the Fe

"Addressing the urgent need for external cephalic version training and promotion: a survey on theoretical course and practical workshops participants" - a pilot study.

OBJECTIVES: External cephalic version (ECV) is a valuable technique for reducing the incidence of planned cesarean deliveries due to fetal malpresentation. The objective of the study was to evaluate the impact of a course conducted by specialists from a top-tier reference hospital on participants and their willingness to incorporate the acquired knowledge of performing external cephalic version (ECV) into their daily medical practice. METHODS: A survey-based study was conducted among 20 participants, including gynecology and obstetrics residents and specialists, who attended an ECV training c

Development of evaluation grids for obstetrical techniques in simulation training using the Delphi method.

BACKGROUND: Simulation-based training has become a cornerstone in obstetrics education, offering valuable opportunities to practice rare and complex procedures in a controlled environment. The importance of simulation in obstetrics and gynecology is well documented internationally, particularly in improving clinical skills and patient safety. In some countries, certain obstetric procedures, such as vaginal breech delivery or shoulder dystocia management, are infrequently performed, which highlights the challenge of ensuring that practitioners are adequately prepared for these rare events. Shoulder dystocia and associated manoeuvres as risk factors for perineal trauma.

INTRODUCTION AND HYPOTHESIS: Shoulder dystocia (SD) is an obstetric emergency that can be associated with serious neonatal morbidity and mortality. The aim of this study was to identify the incidence and risk factors for obstetric anal sphincter injuries (OASIS) in women who sustained SD at birth. METHODS: This was a retrospective observational study over a 5-year period whereby 403 cases of SD were identified. The primary outcome measure was to identify the incidence of OASIS in women with SD. We also evaluated the role of the manoeuvres used for the management of SD and aimed to identify po

Controversies surrounding the causes of brachial plexus injury.

In the past decades, obstetric personnel have attempted to reduce the frequency of brachial plexus injury (BPI) by attempting to use less traction during the delivery of the fetal anterior shoulder. Clearly, these attempts have been unsuccessful in preventing or reducing the frequency of BPI. We conducted a nonsystematic literature review of the reported controversies regarding the mechanisms of BPI causation and why this complication is not decreasing in frequency. The review indicates that maternal labor forces are the most likely cause of BPI.

Employing nifedipine as a tocolytic agent prior to external cephalic version.

BACKGROUND: External cephalic version (ECV) is accepted as a means of reducing the rate of breech presentation at term. Routine use of tocolysis prior to an ECV has been described to increase its success rate. The study's objective was to evaluate the efficacy of oral nifedipine as a tocolytic agent prior to ECV, and to compare it with intravenous ritodrine (IR). METHODS: Women with breech presentation at term considered suitable for ECV were given nifedipine prior to the procedure. The success rate was compared to a cohort of women who underwent an ECV attempt at our department during the ye

Digital rotation from occipito-posterior to occipito-anterior decreases the need for cesarean section.

OBJECTIVE: Our purpose was to study prospectively the efficacy of digital rotation in reducing the prevalence of persistent occipito-posterior position (POP) and its consequences. STUDY DESIGN: Sixty-one women with a singleton pregnancy were enrolled prospectively between July 2003 and July 2004. They were in the second stage of labor with the fetal head engaged in the occipito-posterior position. During the first period of the study women were allowed to continue labor without intervention (group I); during the second period, digital rotation was performed (group II). RESULTS: In group I 15

Terbutaline-triggered fetal arrhythmia prior to neonatal diagnosis of Wolff-Parkinson-White syndrome: A case report.

INTRODUCTION: Short-term maternal administration of betamimetics is a common obstetric practice with uses including tocolysis during antenatal corticosteroid administration for fetal lung maturity, intrapartum tachysystole, and prior to external cephalic version. While previous research has demonstrated adverse effects of prolonged use of maternal betamimetics, no prior documentation exists of fetal tachyarrhythmias beyond sinus tachycardia after administration of terbutaline. CASE: This case documents a transient fetal tachyarrhythmia consistent with presumed atrial flutter after maternal ad

Neural network approach for predicting outcomes of external cephalic version for breech presentation: a retrospective cohort study.

The procedure of external cephalic version (ECV) is an important option in the management of breech presentation. However, there is still a lack of effective methods to accurately predict the likelihood of ECV success on the basis of individual conditions. With the aim of better predicting the outcomes of ECV and subsequent delivery modes, this study developed neural network-based models. We conducted a retrospective cohort study of women with singleton pregnancies who underwent an ECV for breech presentation at a single, tertiary, university-affiliated hospital between January 2016 and Septem

Revisiting the management of term breech presentation: a proposal for overcoming some of the controversies.

BACKGROUND: The debate surrounding the management of term breech presentation has excessively focused on the mode of delivery. Indeed, a steady decline in the rate of vaginal breech delivery has been observed over the last three decades, and the soundness of the vaginal route was seriously challenged at the beginning of the 2000s. However, associations between adverse perinatal outcomes and antenatal risk factors have been observed in foetuses that remain in the breech presentation in late gestation, confirming older data and raising the question of the role of these antenatal risk factors in

Late preterm versus term external cephalic version: an audit of a single obstetrician experience.

PURPOSE: Recent literature evaluating the optimal timing for external cephalic version (ECV) in non-cephalic presentation is limited and hampered by methodological issues. We aimed to compare late preterm ECV [term (36-37 weeks of gestation] to term ECV ( > 37 weeks). METHODS: We conducted a retrospective cohort study of prospectively collected data of ECV procedures performed by a single operator during a 6 year period. Maternal, ECV procedure, delivery and fetal characteristics were compared between preterm ECV and term ECV. RESULTS: Overall, 547 (91.6%) of ECVs were term ECV while 50 (8.4

The effect of presentation and mode of delivery on neonatal outcome in the second twin.

OBJECTIVE: The purpose of this study was to determine if cesarean delivery of the nonvertex second twin improved neonatal outcome. STUDY DESIGN: We examined the maternal and neonatal records from 457 sets of twins delivered from 1985 to 1990. We compared 1- and 5-minute Apgar scores, umbilical artery and vein blood pH values, duration of neonatal hospitalization, the incidence and length of ventilation, intraventricular hemorrhage, birth trauma, and mortality rate between vertex and nonvertex second twins delivered either vaginally or by cesarean section. RESULTS: The presentation and mode o

Manual rotation and maternal positioning for managing fetal occiput posterior position during labor: a network meta-analysis.

BACKGROUND: The occiput posterior (OP) foetal position is a global obstetric challenge associated with increased maternal and neonatal complications. Manual rotation and specific maternal positioning (hands-and-knees, lateral position) are potential interventions for managing persistent OP position during labour; however, their comparative effectiveness remains uncertain. This systematic review and network meta-analysis aimed to evaluate the efficacy of these interventions versus standard care. METHODS: A systematic search of multiple databases was conducted from inception to January 3, 2025
