Perforation of a vascular prosthesis by an eroded thoracic vertebral body.

A case of prosthetic graft rupture due to friction against the vertebral column, eroded by a previously repaired thoracic aortic aneurym, is described. Chiselling away the sharp bony edges of the eroded vertebrae, could have prevented this complication. To our knowledge, this complication has never been reported before.

The posterior surface of the lumbar vertebral bodies. Part I.

The degree of concavity (ie, the scalloping) on the posterior surface of the lumbar vertebral bodies has been evaluated quantitatively by means of a simple measuring device. The scalloping in the median sagittal plane was found to differ from that in the lateral plane, near the pedicular attachments. In the medial plane, an increase in scalloping from L1-L4 was noted, with a subsequent decrease at L5. Laterally, the concavity deepened from L1-L5, the values here being larger than those medially at all levels. The results are discussed with respect to the anatomy of the dural sac and its conten

Thoracic radiculopathy related to collapsed thoracic vertebral bodies.

Three cases are presented of thoracic radiculopathy related to collapsed thoracic vertebral bodies. In all cases proximal weakness of the legs was present, leading to the diagnosis of myopathy in two cases. Sensory symptoms were present in two cases. In one, anterior thigh paresthesias lead to a diagnosis of meralgia paresthetica. This diagnostic entity must be remembered if appropriate corroborative tests are to be performed. In cases of trauma this diagnosis should be considered if thoracic vertebral collapse is present. Conversely, an evaluation of proximal weakness should include a review

Paget's disease of a vertebral body seen on computerized tomography.

The author presents a case of Paget's disease effecting a lumbar vertebral body that was seen on a computerized tomogram. It had an interesting appearance and showed no contrast enhancement. There was a moderate compression fracture to this vertebral body and there was encroachment on the spinal canal. The author knows of no other similar case presentation.

Bilateral anterior lumbar vertebral body tethering: a feasibility cohort study.

PURPOSE: Anterior vertebral body tethering (VBT) is a viable option for children with idiopathic scoliosis. Treating double major curves with bilateral VBT may allow patients to avoid spinal fusion while improving the coronal Cobb angle. METHODS: A single center retrospective study was conducted to identify all patients who underwent bilateral VBT (lowest instrumented vertebra L3 or 4) with minimum 2-year follow-up. Clinical and radiographic parameters were collected, including complications and reoperations. Statistical analysis was performed utilizing Students' t-test. RESULTS: Seventy-thr…

Does screw alignment affect tether breakage in vertebral body tethering?

PURPOSE: Vertebral body tethering (VBT) is a non-fusion surgery for treating idiopathic scoliosis, with tether breakage as a common complication that occurs in 19–52% of patients. It has been reported in mechanical studies that screw malalignment > 1 mm may cause tether breakage. This is a clinical study aiming to assess the effect of screw alignment on tether breakage. METHODS: This was a retrospective multicenter cohort study of 115 patients with idiopathic scoliosis who underwent VBT. Clinical data and radiographic parameters such as Cobb angle, screw malalignment, and inter-screw angle wer…

Coronal decompensation following thoracic vertebral body tethering in idiopathic scoliosis.

PURPOSE: Post-operative coronal decompensation (CD) continues to be a challenge in the treatment of adolescent idiopathic scoliosis (AIS). CD following selective spinal fusion has been studied. However, there is currently little information regarding CD following Vertebral Body Tethering (VBT). Thus, the goal of this study is to better understand the incidence and risk factors for CD after VBT. METHODS: Retrospective review of a prospective multicenter database was used for analysis. Inclusion criteria were patients undergoing thoracic VBT, a minimum 2-year follow-up, LIV was L1 or above, ske…

Biomechanical modeling and assessment of lumbar vertebral body tethering configurations.

PURPOSE: Vertebral body tethering (VBT) is a fusionless spinal growth modulation technique, which shows promise for pediatric idiopathic scoliosis (IS) curve correction. This technique, mainly used for thoracic curves, is increasingly being used to treat lumbar curves in order to preserve spine flexibility. It remains necessary to adequately define the cord tension to be applied during the operation and the instrumented levels to biomechanically predict correction over time for the lumbar spine. METHODS: Twelve pediatric patients with lumbar IS, treated with lumbar-only or lumbar and thoracic…

Radiographic outcome after vertebral body tethering of the lumbar spine.

INTRODUCTION: Multiple studies have analyzed the outcome after thoracic Vertebral Body Tethering (VBT). The results seem reproducible with most studies reporting coronal correction rates around 50% and a tether breakage rate near 20% at two years follow-up. There is a paucity of data on lumbar VBT, and no study has yet analyzed the radiographic outcome after lumbar VBT in a double tether technique at two years follow-up, which was the aim of this study. METHODS: This is a retrospective, single surgeons' data analysis of all consecutive immature patients who have had VBT of the lumbar spine (t…

Evidence-based Indications for Vertebral Body Tethering in Spine Deformity.

Posterior spinal fusion has long been established as an effective treatment for the surgical management of spine deformity. However, interest in nonfusion options continues to grow. Vertebral body tethering is a nonfusion alternative that allows for the preservation of growth and flexibility of the spine. The purpose of this investigation is to provide a practical and relevant review of the literature on the current evidence-based indications for vertebral body tethering. Early results and short-term outcomes show promise for the first generation of this technology. At this time, patients shou…

Vertebral Body Avascular Osteonecrosis: An Unusual Late Complication of Tracheostomy.

Vertebral body erosion is a rare late complication of tracheostomy. Here we present the case of a 30-year-old female patient, in a vegetative state after severe brain injury, with a permanent tracheostomy and prolonged mechanical ventilation, who suffered from recurrent episodes of severe air leakage with oxygenation drop due to a puncture in the tracheostomy tube cuff. A neck computed tomography (CT) detected destruction of two vertebral bodies, C7 and Th1, and a bony fragment - a remnant of C7 penetrated the trachea and probably repetitively punctured the inflated cuff. A biopsy of the C7 ve…

Vertebral Body Sliding Osteotomy for Cervical Myelopathy With Rigid Kyphosis.

Cervical spondylotic myelopathy is surgically demanding when associated with rigid kyphosis. Posterior surgery cannot restore cervical lordosis, and adequate decompression is not possible with rigid kyphosis. Vertebral body sliding osteotomy (VBSO) is a safe and novel technique for anterior decompression in patients with multilevel cervical spondylotic myelopathy. It is safe in terms of dural tear, pseudarthrosis, and graft dislodgement, which are demonstrated at high rates in anterior cervical corpectomy and fusion. In addition, VBSO is a powerful method for restoring cervical lordosis throug…

Trabecular Architecture and Mechanical Heterogeneity Effects on Vertebral Body Strength.

PURPOSE OF REVIEW: We aimed to synthesize the recent work on the intra-vertebral heterogeneity in density, trabecular architecture and mechanical properties, its implications for fracture risk, its association with degeneration of the intervertebral discs, and its implications for implant design. RECENT FINDINGS: As compared to the peripheral regions of the centrum, the central region of the vertebral body exhibits lower density and more sparse microstructure. As compared to the anterior region, the posterior region shows higher density. These variations are more pronounced in vertebrae from …

Vertebral body osteonecrosis: proposal of a treatment-oriented classification system.

PURPOSE: To present a classification system for vertebral body osteonecrosis (VBON) based on imaging findings and sagittal alignment and consequently to propose treatment guidelines. METHODS: Chart review and classification of imaging and clinical findings. An analysis of literature about VBON has been evaluated to conceive the classification. The current data allows to correlate radiological findings with different stages of the pathophysiological process and consequently to propose a patient-tailored treatment plan. RESULTS: The classification identifies 4 stages: stage 0 (theoretical phas…

Blunt aortic injury secondary to fragmented tenth thoracic vertebral body.

We present a case of blunt traumatic aortic laceration following a motor vehicle crash. The aortic laceration was 4.5 cm above the coeliac axis and occurred because of an unstable tenth thoracic vertebral body. Open surgery was considered high risk, whereas an endovascular approach with an endoprosthesis placed at the exact anatomic location of the laceration was advocated.

Treatment of severe vertebral body compression fractures with percutaneous vertebroplasty.

OBJECTIVE: Evaluate the efficacy of percutaneous vertebroplasty for severe vertebral body compression fractures. METHODS: Over a period of 6 years and 8 months, 661 vertebroplasties were performed in 292 patients at our institution. Of these, 69 patients met our criteria for a severe vertebral body compression fracture defined as vertebral body collapse to less than one-third of the original height. Of the 69, 25 underwent single level vertebroplasty. Imaging features were then analyzed including location, extent of collapse, pattern of compression, pre- and post-kyphotic angle and adjacent d…

[Anterolateral instrumentation and vertebral body replacement for thoracolumbar segment fractures].

INTRODUCTION: Acute traumatic lesions of the thoracolumbar segment and spinal cord are a serious cause of disability and death. The diagnosis may be delayed and non-standardized or inappropriate with the resulting increase in problems; one of the great treatment advances has been the anterior and anterolateral surgical approach. OBJECTIVE: Perform a clinical-radiological assessment of patients with a diagnosis of thoracolumbar segment fracture who underwent anterolateral instrumentation and vertebral body replacement with a cage and graft, and compare their pre- and postoperative status. Complete posterior migration of intact vertebral body in spinal tuberculosis.

Spinal tuberculosis most commonly presents as a paradiscal lesion involving the disc space and adjacent vertebral bodies. Atypical forms of spinal tuberculosis have been described and are most often a result of posterior element involvement. The authors report a patient, who presented with complete posterior migration of an intact vertebral body, a complication of spinal tuberculosis that has not been reported till date. A 12-year-old girl with history of pulmonary tuberculosis presented with progressive paraparesis and back pain. Plain X-rays and MRI revealed that the L2 vertebral body had mi…
